Effects of Structured Resisted Exercises on Cognition Level Among Patients with Mild Cognitive Impairment

Objective: To evaluate the effects of structured resistance exercises on cognition level among patients with mild cognitive impairment. Methods: A quasi experimental trial was conducted on thirty patients with mild cognitive impairment (MCI) from September 2020 to February 2021 at Ibn e Siena hospital, Multan. The total sample was randomly divided into two equal groups containing fifteen patients each; Group-A (Conventional pharmacological treatment) and Group-B (Resisted exercises). Group-A participants were treated with conventional pharmacological treatment cholinesterase inhibitors along with regular physical exercise while Group-B participants were treated with resistance exercises along with conventional pharmacological treatment. The standardized tools were used for data collection including Standardized mini mental state examination (SMMSE), Montreal cognitive assessment (MOCA), Trial making test A (TMT-A) and Trial making test B (TMT-B). Data was entered and analyzed by using SPSS 21. Results: Independent samples T-test showed statistically significant difference after intervention for measures of cognitive performance. There was significant difference (p<0.01) between pre and post intervention score of SMMSE (20.60 ± 1.75 and 23.20 ± 1.69) and MOCA (17.60 ± 1.35 and 21.93 ± 1.57). There was also significant difference between (p<0.01) pre and post intervention score of TMT-A (1.47 ± 0.34 & 1.23 ± 0.04) and TMT-B (2.51 ± 0.04 and 2.08 ± 0.04). Conclusion: Resistance exercises increases the cognitive levels of Mild Cognitive impairment patients. Key Words: Cognitive dysfunction, Exercises, Dementia.

HIV-ASSOCIATED NEUROCOGNITIVE DISORDER (HAND) PADA PASIEN DENGAN HIV TANPA INFEKSI OPORTUNISTIK

Latar Belakang: Infeksi Human Immunodeficiency Virus (HIV) telah menjadi epidemi di seluruh dunia termasuk di Indonesia. Salah satu komplikasi infeksi HIV pada sistem saraf pusat (SSP) berupa gangguan fungsi kognitif yang disebut HIV-associated neurocognitive disorder (HAND). Replikasi HIV dalam jangka waktu panjang terjadi pada astrosit dan mikroglia, yang dapat menurunkan fungsi neuronal. HAND terkait dengan aktivitas virus dan mediator inflamasi sel imun pada SSP sehingga menyebabkan kerusakan neuron otak. Kasus: Pasien perempuan, 28 tahun, suku Bali, mengeluh mudah lupa yang dialami sejak 2 tahun lalu. Pasien masih dapat melakukan aktivitas sehari-hari secara mandiri walaupun keluhan lupa terkadang dirasakan mengganggu. Pasien memiliki riwayat infeksi HIV sejak bulan September 2015 dengan CD4 16 sel/µl dan mendapat terapi ARV fixed-dose combination dengan regimen tenofovir, lamivudine, dan efavirenz. Pemeriksaan neurobehavior dijumpai atensi baik, gangguan memori terutama new learning ability, memori tunda, asosiasi berpasangan, gangguan visuospasial dan eksekutif, ADL dan IADL mandiri, MMSE: 24, MoCA Ina: 14, Clock Drawing Test: 3, Trial making test A baik, Trial making test B terganggu, International HIV Dementia Scale (IHDS): 10.5, Skala penilaian depresi Hamilton: 15. Hasil CT Scan kepala dalam batas normal. Diskusi: Dari hasil pemeriksaan, pasien dikategorikan dalam HAND tipe Asymptomatic Neurocognitive Impairment (ANI). Kadar CD4 diketahui berhubungan dengan derajat kerusakan neuron otak dan kadar CD4 nadir rendah (≤ 200 sel/µl) merupakan faktor risiko terjadinya gangguan kognitif pada pasien dengan HIV. Pemberian terapi kombinasi ARV dapat menunjukkan peningkatkan performa fungsi kognitif dan fungsional. Simpulan: Infeksi HIV secara langsung pada SSP dapat menyebabkan gangguan neurokognitif dan inisiasi pemberian terapi ARV dini merupakan usaha pencegahan terjadinya perburukan lebih lanjut.

Залежність виникнення післяопераційної когнітивної дисфункції від глибини анестезії у хворих після коронарного шунтування

Мета. Проаналізувати залежність виникнення післяопераційної когнітивної дисфункції (ПОКД) від глибини анестезії у хворих після оперативного втручання з приводу ішемічної хвороби серця (ІХС) - коронарного шунтування в умовах штучного кровообігу. Матеріали і методи. Шунтування коронарних судин виконано 79 пацієнтам з ІХС, яких розподілили на дві групи за єдиним критерієм - в залежності від глибини анестезії під час перфузії. До 1-ї групи увійшли пацієнти, у яких під час анестезії показник біспектрального аналізу (BIS) утримувався на рівні менше 40, до 2-ї – на рівні від 40 до 55. До оперативного втручання, на 7-й день та через 3 міс після нього в обох групах застосовували нейрокогнітивні тести: шкалу оцінки психічного статусу (Mini Mental state examination - ММSЕ), а також тести за методикою Grooved pegboard і за методикою Trial Making Test Part A. Результати. Застосування тесту MMSE продемонструвало, що пацієнти 1-ї групи на 7-й день після операції набирали на 7,8% (14,2; 5,6), а через 3 міс – на 3,8% (7,3; 3,4) менше балів у порівнянні з кількістю балів, яку вони набирали до операції. Пацієнти 2-ї групи на 7-й день після операції набирали на 4,3% (-8,3; 4,3) менше балів, а загальна кількість балів, яку вони набирали через 3 міс, не відрізнялася від загальної кількості балів, набраної ними до операції (-5,3; 0). Застосування тесту візуальної уваги (Trial making test Part A) показало, що пацієнти 1-ї групи на 7-й день після операції витрачали на виконання тестового завдання на 13,8% (11; 16,5), а через 3 міс - на 3,3% (0; 5,7) більше часу у порівнянні з доопераційними показниками. У 2-й групі на 7-й день після операції пацієнти витрачали на виконання тесту на 3% (-2,4; 5,7) більше часу, а через 3 міс - на 2,6% (3,7; 2,9) менше часу, ніж до операції. Результати тестування за методикою Grooved pageboard були такими. Пацієнти 1-ї групи на 7-й день після операції витрачали на виконання тесту на 8% (5,5; 10,9), а через 3 міс - на 4,8% (2,5; 9,4) більше часу, ніж до оперативного втручання, пацієнти 2-ї групи – відповідно 7,1 (5,2; 12,3) та 3,6% (2,4; 9,8). Висновки. Результати клінічних досліджень у хворих після виконання коронарного шунтування показали, що під час штучного кровообігу показники BIS на рівні 40 – 55 відповідають оптимальній глибині анестезії, що є запорукою зменшення частоти виникнення ПОКД у ранньому періоді та через 3 міс після оперативного втручання. Фактором ризику виникнення ПОКД під час шунтування коронарних артерій в умовах штучного кровообігу є показники BIS менші 40.

Sensibilidad de algunas pruebas estandarizadas para evaluar el funcionamiento de la atención ejecutiva en niños de siete años

Introducción. La utilización de pruebas estandarizadas para la evaluación de las habilidades atencionales, es una práctica habitual dentro de contextos clínicos y educativos. Puesto que el concepto de atención resulta algo ambiguo, este estudio intenta examinar la relación entre algunas de estas pruebas y la atención ejecutiva, tal y como es conceptualizada en las aproxi-maciones más recientes de la neurociencia cognitiva. Método. Se administraron algunos subtests del WISC-R, la parte A y B del Trial Making Tests y la versión de tarjetas de la tarea Stroop a un grupo de niños de siete años. A continuación los niños llevaron a cabo una versión computerizada de la tarea Stroop, la cual, según algunos estudios de neuroimagen, estaría asociada con el circuito neural implicado en la aten-ción ejecutiva. Resultados. Los niños puntuaron diferencialmente únicamente en los subtests de Figuras In-completas, en Dígitos y en la parte B del Trial Making Test, produciendo diferentes patrones de interferencia en el efecto Stroop. Además, las puntuaciones obtenidas en ambas versiones de la tarea Stroop no correlacionaron. Discusión. Los resultados parecen indicar que estas pruebas son sensibles al funcionamiento de la atención ejecutiva en niños de edad escolar. Por otra parte, la ausencia de correlación entre las dos versiones de la tarea Stroop sugiere que en la versión de tarjetas los mecanismos atencionales podrían ser diferentes a los implicados en la versión informatizada.

Predictors of Cognitive Dysfunction among Patients with Moderate to Severe Chronic Kidney Disease

Cognitive dysfunction including dementia is a common complication of chronic kidney disease (CKD) that has just been recently appreciated. It has negative outcomes in the management of patients with CKD. This study explored the possible biochemical and clinical features of patients with CKD that can predict the occurrence of cognitive impairment in patients with moderate to severe CKD. We evaluate patients with stages 3-5 CKD for the occurrence and predictors of cognitive impairment. Multiple areas of cognitive function were tested in this single-center study using Community Screening Interview for Dementia (CSID) and Trial-Making Test A (TMTA)/Trial-Making Test B (TMTB). Cognitive impairment was correlated with patients’ routine biochemical, hematological, and selected clinical parameters. We observed a negative correlation between cognitive impairment and patient’s serum calcium (r 5 0.240; p 5 0.033) and estimated Glomerular filtration rate (eGFR) (r 5 0.379; p 5 0.0006). Therefore, eGFR is an accurate predictor of cognitive dysfunction in patients with moderate to severe CKD. Early evaluation of cognitive function in CKD is indeed advised for optimal outcome in the management of patients with CKD.

Fonctions exécutives, troubles bipolaires et trouble déficit de l’attention et hyperactivité. Quel lien ?

Le trouble déficit de l’attention avec hyperactivité est fortement associé au trouble bipolaire chez l’adulte. La perturbation des fonctions exécutives est présente dans les troubles bipolaires. Le but de cette étude est d’évaluer les fonctions exécutives, grâce à quatre tests neuropsychologiques (Stroop, Trial Making Test, Wisconsin, Fluences verbales), chez des patients portant le diagnostic de trouble bipolaire selon les critères du DSM-IV associé au trouble hyperactivité déficit de l’attention déterminé par l’entretien diagnostic, la DIVA (Diagnostisch Interview Voor ADHD) et un auto-questionnaire de TDAH (Wender Utah Rating Scale : WURS). L’étude montre que l’altération des fonctions exécutives est plus importante chez les patients souffrant de comorbidité d’où la spécificité des thérapeutiques visant les troubles cognitifs caractéristiques.

Lack of a chemobrain effect for adjuvant FOLFOX chemotherapy in colon cancer patients: A pilot study.

e14018 Background: Chemotherapy improves the survival rate of stage III colon cancer patients. The combination of oxaliplatin, 5-fluorouracil and leucovorin (the FOLFOX4 regimen) has emerged as the standard of care. Cognitive changes and disfunction after cancer chemotherapy are increasingly reported as a disturbing side-effect in cancer survivors. This prospective study evaluates potential alterations in cognitive function in Folfox4-treated patients. Methods: We evaluated 57 consecutive colorectal cancer patients who received adjuvant chemotherapy with FOLFOX4. Patients underwent a complete battery of neuropsychological tests at three different times: before (T0), at the end (T1) and six months after treatment (T2). Changes in neuropsychological test scores at T0, T1 and T2 were compared with a two tail t-test (T1-T0, T2-T1, T2-T0), using the repeated measure ANOVA model. Results: We have analyzed cognitive impairment (Mini Mental State Examination, MMSE), visuo-spatial memory, information processing speed (Trial Making Test-A and Trial Making Test-B), verbal memory (Rey Auditory Verbal Learning Test), emotional distress (Psychological Distress Inventory), anxiety (State and Trait Anxiety Inventory) and depression (Beck Depression Inventory). We found no cognitive impairment in time considered, as we found a median MMSE score of 27,51 ± 1,21 at T0, 27,63 ± 0,65 at T1 and 27,26 ± 1,17 at T2. We noticed some transient variations in tests evaluating verbal memory and in information processing speed performances. The only significative scores modifications were those related to anxiety and depression. Conclusions: We found no significant effect on cognitive function related to chemotherapy, the only little modification is about some emotional performance during chemotherapy. These findings may be explained by the central role of the psychological adaptation process, which occurs during the period from diagnosis to completion of treatment and is characterized by anxiety and adjustment depression. Our results seem to rule out any significant cognitive impairment due to adjuvant Folfox4 chemotherapy in colon cancer patients.
