Verletzung der A. profunda femoris mit transfusionspflichtiger Blutung durch sekundäre Dislokation des Trochanter minor nach Osteosynthese einer pertrochantären Femurfraktur

Zusammenfassung Ein 95-jähriger Patient erlitt eine transfusionspflichtige Blutung nach Osteosynthese einer pertrochantären Femurfraktur mittels proximalem Femurnagel. Das bei der Erstmobilisation sekundär dislozierte Fragment des Trochanter minor perforierte einen Seitenast der A. profunda femoris. Die Blutung konnte mittels Embolisation gestillt werden, und der Patient wurde wenige Tage später mit stabilem Hämoglobinwert entlassen. Wiederholt sinkende Hämoglobinwerte trotz mehrmaliger Bluttransfusion weisen auf eine aktive Blutung hin. Die genaue Identifikation der Blutungsquelle nach Osteosynthese proximaler Femurfrakturen ist entscheidend für die korrekte Therapie.

The Impacts of Elastic Band Training on the Posterior Abdominal Wall Muscles of Olympic-Style Weightlifters

Purpose: In this study, it was aimed to examine the impacts of elastics band trainings, which were applied for 12 weeks on the Olympic-style weightlifters, on their sportive performances and psoas, ilacus, and quadratus lumborum muscle volumes.Materials and Methods: This research study was conducted on 4 groups, comprising of 12 men (elastic band training group (n=6), control group (n=6)), and 12 women (elastic band training group (n=6), control group (n=6)) Olympic-style weightlifters. The athletes in the elastic band training groups were engaged in standing hip flexion exercises, clamshell exercises, lying hip flexion exercises, and side bend exercises. Before and after the elastic band training applied to the Olympic-style weightlifters for 12 weeks, (1.5 Tesla) MRI device was used for the axial images of the regions starting from the 12th thoracic vertebrae to the trochanter minor of femur. The volume calculations of the muscles were conducted via the Cavalieri method.Results: After the elastic band training, it was determined that the total volume of posterior abdominal wall muscles of the men and women weightlifters in the elastic band training groups and the total weights lifted by them were higher compared to those of the control group individuals (p<0,001).Conclusion: As the result, it can be stated that including the posterior abdominal wall muscle developing trainings into the general programs of the weightlifters in the Olympic weightlifting sport can both enable an increase in the volumes of the muscles in this region and positively influence the performance of the athletes.

Patient-Specific Restoration of Biomechanics in Total Hip Arthroplasty

ZusammenfassungEine exakte Rekonstruktion der Biomechanik ist ein wesentlicher Bestandteil in der Hüftendoprothetik für eine gute postoperative Beweglichkeit und Stabilität des Kunstgelenks. Die präoperative Planung des Beinlängen- und Offsetausgleichs wird üblicherweise auf größenskalierten konventionellen Röntgenbildern durchgeführt, die jedoch durch die zweidimensionale Projektion der Gefahr von Fehlberechnungen unterliegen. Insbesondere das femorale Offset wird dabei häufig unterschätzt. Bei der Rekonstruktionsgenauigkeit von Beinlänge und Offset sollten Unterschiede von über 1 cm postoperativ vermieden werden. Analysen der Gangkinematik zeigen jedoch bereits Veränderungen im Gangmuster bei Beinlängen- und Offsetunterschieden von über 5 mm. Neben einer Vielzahl anderer Methoden eignen sich zur intraoperativen Umsetzung der Beinlängen- und Offsetrekonstruktion sowohl die bildfreie Navigation als auch die Fluoroskopie. Eine Abschätzung der Schafttorsion der einliegenden Endoprothese ist neben des Goldstandards CT auch auf konventionellen Röntgenaufnahmen mithilfe der Bestimmung des projizierten CCD-Winkels möglich. Als prädiktiver Parameter eignet sich die Isthmus-Ratio (iRatio), um bereits präoperativ nativradiologisch einen Hinweis der intraoperativ zu erwartenden Schafttorsion zu erhalten. Die Größe des Trochanter minor hilft, eine Abschätzung der nativen Torsion des Femurs treffen zu können.

Finite element analysis of the impact of screw insertion distal to the trochanter minor on the risk of iatrogenic subtrochanteric fracture

Iatrogenic subtrochanteric fractures are rarely encountered after cannulated screw fixation of femoral neck fractures; however, when they do occur, there can be several complications. Many orthopedic surgeons have concerns about the potential for iatrogenic subtrochanteric fractures after screw fixation distal to the trochanter minor; therefore, some surgeons are typically reluctant to perform this procedure. This study focused on the risk of an iatrogenic subtrochanteric fracture after treating femoral neck fractures with cannulated screws. The main purpose of the study was to understand iatrogenic subtrochanteric fractures and evaluate the effects on these fractures of an inverted triangular–shaped configuration for placement of the cannulated screws. A femur bone with an femoral neck fracture at a 40° incline to the horizontal plane was generated along with a representation of a three-dimensional finite element model, and three inverted triangular–shaped configurations for placement of the cannulated screws were investigated using finite element analyses. Statistical results indicated that the occurrence risk of ISF increases when the screw is located distal to the trochanter minor. Moreover, the risk of occurrence of intertrochanteric fracture increases when the screw is located medial to the trochanter minor because of local concentrated stress on the surface of the screw canals. To avoid the vulnerability of the subtrochanteric region, it was found that proximal placement of the screws using the inverted triangular–shaped configuration could yield better results. In addition, the results of this study provide suggestions on improved screw configurations.
