Morphological substantiation of the clinical efficacy of platelets rich plasma was carried out in the treatment of 22 men with androgenetic alopecia from the 1st to the 4th stage according to the Norwood-Hamilton scale were included. All patients received intradermal injections of platelets rich plasma 0,15 ml per injection. The course of treatment consisted of 4 procedures with an interval of 4 weeks. Clinical efficacy was assessed by the dynamics of morphometric indices of hair growth. Histological examination was carried out on horizontal sections, stained with hematoxylin and eosin, the morphology of the hair was counted at four levels. Evaluation of morphometric growth parameters conducted before treatment and 4 months after the onset of it. It was established that the therapy of platelet-rich plasma has a pronounced clinical efficacy, consisting in a significant (p=0,00025) increase in hair density by 11% and average hair diameter by 10% (p=0,00766), a 14% decrease in the share of hair follicles (p=0,00959). Histologically, the increase in hair density was significant at the level of the bulb of the hair follicles by 148% (p=0,0034) and at the level of the sweat glands by 65% (p=0,0326), and also by the tendency to increase their number at the level of the sebaceous glands. This was combined with a significant decrease in the proportion of telogen hair at 47% (p=0,0153). Thus, the positive clinical effect of plasma-rich plasma therapy in men suffering from androgenetic alopecia is based on reliable morphofunctional changes in the hair follicles.

Androgenic alopecia (AGA) is a type of progressive hairloss, where there is androgen mediated conversion of susceptible terminal hairs into vellus hairs, in genetically predisposed individuals. To compare efficacy of Topical 5% Minoxidil alone versus Topical 5% Minoxidil with Autologous Platelet Rich Plasma (PRP) therapy in patients with Androgenetic Alopecia.This is aProspective study conducted in Department of Dermatology GMERS Medical College, Gandhinagar, Gujarat. A convenience sample of 62 men in the age group of 20-40 with Grade 2-5 AGA according to Hamilton- Norwood Grading were selected and was divided into 2 groups of 31 each. Presitting digital photographs and dermoscopic photos were taken. Autologous PRP was prepared using 18 ml of patients blood after double spin centrifugation and injected by Nappage technique. Results were assessed at the baseline and at the end of each sitting on the basis of change in hair density, photographic evaluation and patient’s self satisfaction. Highly significant increase in hair density was achieved after 4 months of treatment. At T4 (Fourth Session of treatment) Group B showed higher hair density (42.97± 8.96) as compared to Group A (36.94 ± 11.57) which was statistically significant at P = 0.03Group B showed better improvement as compared to Group A.PRP treatment has a positive therapeutic effect on male Androgenetic alopecia without major side effects.

Features of platelet-rich plasma in the complex therapy in patients with androgenic alopecia are analyzed. A comparative evaluation of the clinical efficacy of the three treatment methods was carried out: topical applications of a5% solution of minoxidil, intradermal injection of platelet-rich plasma, and combination of these techniques. A solution of minoxidil was applied to the dry skin of the scalp 1 ml twice a day without washing throughout the observation period. The course of treatment consisted of 4 procedures with an interval of 4 weeks. Clinical efficacy was assessed by the dynamics of morphometric indices of hair growth. It was established that after complex therapy in the form of minoxidil applications and injections of plasma rich plasma, the hair density increased by 32% (p=0,00004), the diameter of the hair shafts by 26% (p=0,00004). At the same time, the share of vellus hair decreased by 30% (p=0,00082), and the proportion of telogen hair decreased by 39% (p=0,00008). The results of using complex therapy significantly exceeded the clinical effect of platelet-rich plasma and topical applications of a 5% solution of minoxidil. The data obtained allows to suggest that plasma enriched with platelets and minoxidil potentiate each other’s effects, and their complex application seems promising for the treatment of androgenetic alopecia.

Platelet-rich plasma (PRP) therapy has been considered as a promising treatment for androgenetic alopecia (AGA). The aim of the study was comparative evaluation of the clinical efficacy of PRP-therapy, minoxidil, and their combination in the treatment of men with AGA and to evaluate the effects of PRP on the proliferation of hair follicle (HF) cells in skin biopsy. Materials and Methods: The study involved 69 men who were divided into 3 groups who received PRP therapy, minoxidil, and their combination. The clinical efficacy of the therapy was evaluated by the dynamics of morphometric of hairs. To assess cell proliferation antibodies to β-catenin, CD34, Ki67, and to Dkk-1 were used. Results. PRP treatment was more effective than minoxidil therapy (p = 0.005). Complex therapy turned out to be more effective than minoxidil monotherapy (p < 0.0001) and PRP monotherapy (p = 0.007). After applying PRP the absolute and relative values of the β-catenin and CD34 expression area increased; an increase in Ki67+ index was also significant. Conclusions: PRP can be considered as a treatment option for AGA. Combined PRP and minoxidil use seems promising for the treatment of AGA. PRP increase in the proliferative activity of HF cells and improves hair morphology in patients with AGA.

Abstract Background Although some studies in the literature report that autologous and homologous platelet-rich plasma (PRP) can be employed in the treatment of androgenic alopecia (AGA), no study, to the authors’ knowledge, has examined the estrogen concentration of prepared PRP. Objectives The authors aimed to determine the presence of estrogen in PRP and to investigate the effect of estrogen concentration of PRP on AGA treatment. Methods Between 2017 and 2018, 30 male patients with hair loss complaints were included in this prospective study. Autologous PRP was injected in patients in Group 1. Homologous PRP with high estrogen levels was injected in the patients in Group 2. PRP was injected in both groups 4 times at 0, 1, 3, and 6 months. The obtained photographs were evaluated and hair densities of each patient at controls were calculated. Results The mean estrogen level measured in PRP was statistically significantly higher in Group 2. In both groups, the increase in hair density was observed from the first month, but this increase was statistically significantly higher in all controls in Group 2. In Group 2, there was a statistically significant increase in the 1st and 3rd months compared with the previous control, but there was no difference between the 6th and 12th months and the 3rd month. Conclusions Increased hair density is greater and earlier in the group receiving estrogen-rich PRP than in the group utilizing autologous PRP. The authors think that estrogen-rich PRP may be employed in the treatment of AGA in the presence of an appropriate donor. Level of Evidence: 2

Measurements were made of hairs and sweat glands in the skins of a number of different Bovidae. The principal differences between the Bovinae and the one member of the Cephalophinae (the duiker) studied were in the dimensions of the skin and its component organs and in hair density; secondary hairs were not present, although in the American bison small primary follicles which appeared to provide a woolly undercoat were seen. In contrast, secondary hair foIlicles were found in the skins of all the Caprinae, Antilopinae and most of the Hippotraginae studied. The sweat glands of the Bovidae varied in shape from simple sacs as seen in the oryx to the coiled tubes of the wildebeest, and differed markedly in size, the largest being found in the wisent.

Background: Androgenetic alopecia is characterized by a progressive miniaturization of hair follicles in a pattern distribution in genetically predisposed individuals. The efficacy of conventional therapies is variable, therefore there is a need for adjuvant and newer treatment modalities to provide faster and better outcomes. Objectives: Evaluation of the efficacy and tolerability of a combined therapy: preformed growth factors vehiculated through iontophoresis in patients with androgenetic alopecia and associated telogen effluvium, to obtain faster hair regrowth. Materials and Methods: Treatment was performed between June 2018 and June 2019 on 60 patients with androgenetic alopecia and associated telogen effluvium. Each patient underwent 4 sessions in total, each session was performed every 3 weeks. Global photography and trichoscopy were collected at every session of therapy. All patients filed out a self-assessment questionnaire. Results: Results were very promising, with improvement of hair density and thickening of the hair shaft diameter in most of patients seen with both global photography and trichoscopy. All patients were satisfied of the clinical result and reported a complete reduction in hair loss. No serious adverse side effects were reported. Conclusions: The use of growth factors associated with iontophoresis technique is a useful treatment for treating and preventing androgenetic alopecia. In addition, in case of associated telogen effluvium, this technique allows for an early stop of hair shedding, especially when cosmetic procedures do not provide satisfactory results in patients.
