Total Migration of the Lumbo-peritoneal valve from D4 to V4: Case Report

The valve devices are designed for the treatment of hydrocephalus, Ventriculoperitoneal shunt (VPS) and lumbopperitoneal shunt (LPS), but they have two complications: infectious and mechanical. Among the latter, we find the dysconnection and migrations of parts of the valves either in the intracranial ventricular cavities or in the natural cavities of the patient's body (heart, colon, bladder, lung, vena cava, etc.) even externalization. through natural orifices.

A curious case of electric wire in urethra

We present a case of discharging urethral fistula in a sexually active young male, who was found to have an impacted electrical wire in the urethra. Polyembolokoilamania is a condition, where one inserts foreign bodies via natural orifices, either for autoerotism, in inebriated states or due to psychological conditions. A wire was inserted into the patient’s urethra and it remained impacted for a year, leading to pain, dysuria and discharging urethral fistula. The wire was removed and the fistulous tract was excised. Patients, if diagnosed with autoerotism needs follow up with psychiatrist to address the underlying condition.

Attitude and perception toward anthrax among cattle owners in selected rural communities in Bangladesh

Anthrax is a zoonotic disease caused by spore forming Bacillus anthracis. Throughout the world, it causes illness in livestock, wildlife and sometimes secondarily infects humans and causes a major public health threat. The study was conducted to assess cattle owners’ awareness, perceptions and attitudes toward anthrax. Data on awareness of anthrax, clinical signs of anthrax in animals and human, its routes of transmission and methods of prevention, the families’ consumption habits of anthrax-infected carcasses, and other family activities that increase exposure to anthrax were collected using an interviewer-administered questionnaire. Overall, the level of awareness amongst the farmers for anthrax was 71.5%. All of the farmers were aware that anthrax occurs in cattle, and 54% indicated the presence of unclotting blood oozing from natural orifices as a consistent finding in cattle that died of anthrax, whereas 71.2% of them indicated the presence of skin lesions as the most common sign of the disease in humans. The good efficacy of human anthrax treatment (62%), slaughter of moribund cattle and selling of meat from cattle found dead to unsuspecting consumers (68%), reluctance to lose animals (28%), and forgetting about anthrax (46.4%) were mentioned as the major reasons for consuming anthrax-infected carcasses. Of the respondents 79.2% of cattle owners indicated that they would not consume meat from cattle found dead, because they were discouraged by veterinary authorities, therefore introducing meat inspection services is likely to have a positive impact in preventing human anthrax outbreaks in selected areas of Bangladesh. Disc Battery - An Unusual Vaginal Foreign Body in a Child

Disc battery ingestion and esophageal injury is well known in children. Insertion of a disc/lithium battery into body’s natural orifices is rarely reported. We present a case of self-insertion of a lithium battery into the vagina by a 2 ½ year old female. Vaginoscopy was performed and the battery was retrieved which had corroded and caused vaginal ulceration. Post-operative outcome was favorable. Treating physicians must be aware of the hazardous effects of insertion of lithium batteries as it may cause significant damage in a short period.

Reducing invasiveness of laparoscopic surgery using natural orifices and abdominal wall defects for extraction of the specimen

Introduction: Due to significant technical evolution complex surgeries can be performed laparoscopically nowadays. However, laparotomy is needed frequently for the extraction of the specimen, which decreases the advantages of laparoscopy. Aim: The aim of the authors was to analyse and present their experience on the use of natural orifices and abdominal wall defects for extraction of the surgical specimen. Method: From 2009 the authors used natural orifices (stomach, vagina, rectum) when viscerotomy was an obligate part of laparoscopic surgery and, in a special gastrointestinal laparoscopic operation, the gate of the inguinal hernia for specimen extraction. Results: In 3 patients benign lesions of the stomach were extracted using gastroscope. In 6 patients with bowel endometriosis, in whom the wall of the vagina was completely infiltrated, the resected bowel was extracted transvaginally, and in 5 patients transrectal extraction of the specimen was performed. In 2 patients the inguinal hernia was used for the surgical specimen extraction after laparoscopic sigmoid resection, and in one patient a single-port was inserted into the gate of the hernia during laparoscopic cholecystectomy. Complications occurred only after transvaginal specimen extraction (rectovaginal fistula in 2 patients). Conclusions: Use of natural orifices and abdominal wall defects for surgical specimen extraction further decreases the invasiveness of laparoscopic surgery, if indications made appropriately. Orv.
