With the growing demand for body, contouring, non-invasive devices for reducing localized fat have become increasingly popular and have grown exponentially in the last decade. The Lipocavitation technique has been used recently for this purpose and has been evaluated as a method for the selective destruction of adipose tissue. This work has as main objective, to bring up the practice, the mechanism of action in the body and the effects of the use of lipocavitation in the treatment of localized adiposity. This is an exploratory study of bibliographic review, with consultation in the databases of the VHL (Virtual Health Library), PUBMED and MEDLINE and SciELO. The results showed that the lipocavitation technique used in aesthetics to reduce adipocytes allows the destruction of localized fat, facilitating its elimination and contributing to the reduction of localized measures. With the current technological advances, a variety of techniques can be used, or even combined, allowing innovation to professionals, bringing improvement and well-being to each individual.

Background: Cryofrequency consists of the combination of cryotherapy and radiofrequency, acting simultaneously in the treatment of flaccidity and localized adiposity. Objective: To investigate the effects of cryofrequency at the dermal and hypodermic levels. Materials and methods: This is a case study, composed of one woman of 39-year-old, with interest to perform abdominoplasty surgery, presenting adiposity and flaccidity located in infra-umbilical region. Its treatment consists of eight cryofrequence applications. Results: There was reduction of 7 cm on supra-umbilical plicometry and 6 cm on infra-umbilical; a decrease of 2.7 kg of body weight. The ultrasound data show a decrease of 0.6 mm, in the thickness of the adipose layer. In the histological analysis: thicker epidermis, with more layers and dermis with greater number of fibroblasts and inflammatory cells, greater quantity of neoformed collagen tissue. Conclusion: The application of cryofrequence promoted reduction of adipose tissue and increased production of collagen. Level of Evidence: Level III, case-control study.

Localized adiposity is not only a common aesthetic issue but also a health risk factor. Pharmacopuncture can be a therapeutic option for the imbalance of regional fat distribution. The tuber of Pinellia ternata has been prescribed as antitussive and expectorant as a traditional Korean medicine. This study investigated the effects of pharmacopuncture with P. ternata water extract (PT) on localized adiposity. Male C57BL/6J mice were fed on a high-fat diet (HFD) for 6 weeks. 100 μL of 10 mg/mL of PT was injected into the left-side inguinal fat pad, while saline was injected into the right-side inguinal fat pad as self-control. Treatments were performed 3 times per week for 4 weeks. The inguinal fat weight was analyzed by dual-energy X-ray absorptiometry. PT pharmacopuncture significantly decreased the weight of the inguinal fat pad. The adipocyte size was reduced with increases of lipolytic enzymes and lipophagy-related factors by PT pharmacopuncture. There was marked inhibition of lipid accumulation content in 3T3-L1 adipocytes by PT treatment. The expressions of adipose triglyceride lipase (ATGL), hormone-sensitive lipase (HSL), autophagy-related gene (ATG) 5, ATG7, and LC3 were markedly increased by PT treatments in vivo and in vitro. This study suggests that pharmacopuncture of Pinellia ternata has ameliorative effects on adiposity by lipid catabolic effects via activating both lipolysis and lipophagy in a localized region.

Introduction: The excess of body adiposity and the hormonal action can lead to the accumulation of adipose tissue in certain parts of the body. The electrolipophoresis is a technique used to treat localized fat, by applying low frequency electrical currents that act directly on the adipocytes and lipids accumulated subcutaneously or epicutaneously. This electrical stimulation causes physiological modifications in the adipocyte, among them the increase in blood flow and cellular metabolism leading to lipolysis. Objective: This study aimed to prove, through the measurement of triglycerides in the blood, which is the most effective method of electrolipophoresis practical application in the promoted lipolysis. Methods: The old female patients aged from 49 to 62 underwent evaluation that measured the body mass index (BMI); perimetry performed with the use of measuring tape of the waist and abdomen region and the realization of abdominal (vertical) skinfold measurement with the use of adipometer. After the evaluation, the volunteers were divided into two groups in which one of them received subcutaneous application of electrolipophoresis equipment (needles) and the other one received epicutaneous electrolipophoresis application (plates). They were positioned and oriented about the procedure and received the practical application of electrolipophoresis (DGM equipment) twice a week for ten sessions of 60 minutes each. The dosage of triglycerides was performed in the first and last session. Results: There was a reduction in all evaluations performed, and a subcutaneous application shows more effective in the treatment of localized fat. Conclusion: With these findings was observed a reduction in abdominal measurement and decreased of triglycerides in the blood thus showing that treatment using plates or electrodes is effective in localized lipodystrophy. Despite the significant reduction of localized adiposity, further studies with more participants and sessions are needed to make this treatment, widely used in clinical practice of Dermato-Functional Physiotherapy, most reliable.

Background: Cryofrequency is a characteristic used in clinical practice to reduce localized and flaccid adiposity, and promotes heat in our tissues by maintaining a surface. Objective: To investigate the effects of Cryofrequency in the reduction of adiposity located on the flank region. Methods: This is an experimental study, consisting of 30 female volunteers between 20 and 40 years old with flank fat. The applications were performed once a week, totaling eight applications, using the Andrus Criofrequência  - ADOXY MEDICAL, with parameters of 350 w of power, monopolar mode on the region of right and left flanks, with an area of 10 cm in height and 20 cm in width, for 8 minutes per area. The evaluations were performed before and after 4 and 8 Cryofrequency applications. Results: There was a reduction of the adiposity located in the flank region, with a significant decrease in the values of perimetry, plicometry and ultrasonography, and positive results in the questionnaire evaluating satisfaction and criofrequence reactions. Conclusion: Cryofrequency was effective for the treatment of localized adiposity, generating a positive satisfaction among the evaluated volunteers.

Background: Among the techniques used in clinical practice to reduce localized adiposity, cryolipolysis has been highlighted, which is a non-invasive method capable of reducing the thickness of the adipose tissue without damage to the surrounding tissues. However, several associations of techniques suggest an intensification of the results, associating cryolipolysis with other treatments, and the most common is ultracavitation (UCV) or radiofrequency (RF) and massage. Objective: To investigate the effects of cryolipolysis associated with UCV and RF on localized adiposity. Methods: This is a blinded and controlled clinical trial. The sample was composed of 30 females divided into 2 groups of 15: one group received cryolipolysis application in the abdominal region, and performed 1 session of drainage weekly; while another group performed the cryolipolysis and then 8 applications of UCV associated with RF and modeling massage (protocol named Advanced Redux Method – ARM), once a week. The parameters used in cryolipolysis were: temperature: -7ºC; suction pressure: 30 kpa; and application time: 50 min. For the UCV, the parameters were time of 8 minutes, 30w of potency and intensity of 10 w/cm2 . After cryolipolysis, a follow-up of 2 months was performed to verify the changes related to weight, perimeter and thickness of the adipose tissue, measured by ultrasonography. The reassessments were performed 30 and 60 days after the cryolipolysis. Results: It was observed that the ARM promoted a greater reduction of adiposity located in the abdominal region, in the plicometry, perimetry and ultrasonography variables (p<0.05). In the analysis of the questionnaires the ARM group showed lower edema, pain, ecchymosis after application when compared to the group associated only with the drainage. The percentage of satisfaction was also higher in the ARM group. Conclusion: Therefore, it can be observed that the association of UCV, RF and modeling massage through the ARM promoted a higher reduction in adiposity than the group that exclusively performed the cryolipolysis treatment with lymphatic drainage.
