Differential Attainment is a phenomenon, recognised globally, where certain cohorts of people tend to have poorer career outcomes based on factors other than capability, academic effort, or motivation. Although by no means unique to or exclusive to, it is indeed well described and monitored in the health education/ training, and affects professionals throughout their entire career journey from admission to retirement. It is a marker of an unfair system and affects individuals as well as organisations adversely. It is the responsibility of all organisations, policymakers and regulators to urgently understand the causes, find solutions, and support those that are disadvantaged as a result of this inequality. The British Association of Physicians of Indian Origin addressing its declared mission of achieving excellence through promoting equality and diversity, along with its collaborative partners and a panel of international experts has created an Alliance for Equality in Health Professions. This Alliance will spearhead a thematic analysis of the entire spectrum of differential attainment in medical professions. This paper describes the protocol for the thematic synthesis of evidence, the priority setting partnerships for undertaking a critical integrative analysis, and the process for combining evidence from experts with lived experiences of grassroots professionals to produce meaningful solutions, actions, and policy enablers. The output will be published as a series of papers in Sushruta and culminate in a seminal report in 2021 when BAPIO turns 25.

The study dealt with tax evasion in the medical profession, where the problem was the existence of many cases of tax evasion, especially tax evasion in the income tax of medical professions. The aim of the study is to try to shed light on the phenomenon of tax evasion and the role of the tax authority in the development of controls and means that reduce the phenomenon of tax evasion. The most important results of the low level of tax awareness and lack of knowledge of the tax law and the unwillingness to read it and the sense of taxpayers unfairness of the tax all lead to an increase in cases of tax evasion and in suggested tightening control and follow-up on the offices of auditors, through the investigation and auditing The reports of certified accountants and the use of computers for this purpose in order to raise the degree of confidence in these reports and bring them closer to the required truth and coordination and cooperation with the Union of Accountants and Auditors and inform them about each case of violations of the auditors and accountants N because of its great influence in the rejection of the organization of the accounts and not to ratify fake accounts lead to show taxpayers accounts on a non-truth in order to tax evasion.

The chapter claims that a profession progresses through interaction and conflict with adjacent professions. Using the professional development of physiotherapy in Norway as an example, the chapter illustrates how inter-professional disputes are central to the ways division of labour and responsibility are shaped. The chapter shows how the physiotherapists’ struggle to gain public authorisation, and to become a part of the national health services, were entwined with the medical professions aspirations to control and subordinate other professions working within the field of health and medicine. The chapter shows how physiotherapists battled the medical profession by seeking active support from the state.

In Thailand, the topic of medical ethics and laws related to medical professions has been one part of the national competency assessment criteria. The objective of this article was to design legal issues into the medical curriculum and to share experiences of creative legal study. Legal contents were inserted into 10 subjects and taught for year 1 to year 6 medical students. Students were divided into multi-groups or received individual tasks and then, shared their knowledge and idea for solving legal problems. The results showed they could interpret and create novel ideas for legal and ethical reconstruction, including the topic of the principle of laws, criminal laws, civil and commercial laws, public health laws, organ donation/transplantation, end of life decisions, and legal liability for the medical profession. Finally, the creative legal study can be used as a novel approach to support creativity among medical students.

This chapter examines the history of critiques that have been made of psychiatric practice in specific times and specific places. Though psychiatry is well-established as part of the medical profession and requires completion of a medical education all doctors receive, psychiatrists are often viewed as distinct from other doctors, and psychiatry continues to be viewed negatively in the public eye. Psychiatrists themselves have been partly to blame for this; the profession originally spent decades attempting to differentiate itself from the other medical professions. Since it is not possible to cover every aspect of the history of critiques of psychiatry, the chapter focuses on two major problems that continue to influence how psychiatry is practiced and perceived today: the development of asylums as a form of confinement for those who are mentally ill, and the development of psychiatric nosology and diagnosis. By exploring the continuum of mental illness and the idea of “normal versus abnormal,” the chapter offers psychiatrists a framework for how they can think about their work going forward.

Abstract This article is a voice in the discussion of legal problems related to understanding the concept of a medical profession. The considerations were divided into three main parts. In the first one, the concept of “medical profession” was reconstructed and problems in the classification of particular professions to the category of medical professions were pointed out. Not every medical profession is characterized by the quality of public trust, although due to the good that medical professions are dealing with, such a state would most likely be the most desired one. In the second part of the text, based on the jurisprudence of the Constitutional Tribunal, the features determining the recognition that a given profession is a profession of public trust were indicated. These remarks were related to medical professions. The content of the third part is a consequence of the fact that it is difficult to precisely establish a catalog of medical professions. The practical significance of the lack of systemic solutions in defining a medical profession is given in the example of tax law, in the scope of the exemption from the Goods and Services Tax.

This paper describes the social profile of 200 physicians, practicing medical profession in Bangalore City of the State of Karnataka. The paper explores the personal, family and educational background of the physicians. This would enable us not only to have a social profile of the medical practitioners in the social system, but may also be of relevance in explaining their professional performance. An important finding of the study reveals that the proportion of those coming from higher social origins being marginally higher at 55 percent. It could be indicating to a trend in which the share of less privileged in higher ranking professions is on the rise. An attempt is made to understand the implications of social origin for other traits. The findings reveal that social origin varies significantly with rural urban background, basis for medical education, age at thinking of medical professions and educational grades of the physicians. However, it varies independent of gender, age and educational achievements of the physicians studied.

Practitioners in the legal, accounting, architectural, and medical professions are showing an increasing interest in marketing - particularly in advertising. Some professions such as accounting have already reached finality on whether advertising should be permitted. The medical profession has not given this matter any serious thought and this exploratory study aims to provide some early guidelines by analysing the perceptions of medical practitioners and in particular, the possible implications of lifting advertising restrictions. The empirical results indicate that physicians realise that patients often do not have adequate information at their disposal before choosing the practitioner of their choice. They do not seem to believe, however, that advertising is a means of overcoming this problem. Specifically, older and more experienced physicians feel that advertising will benefit neither the profession nor the public. It is surmised that the status quo will be maintained for the short and medium term.

The COVID-19 pandemic has exposed the devastating impact of inequalities that have plagued societies for generations. The timing of the events spiraling from the unlawful killing of an apparently innocent black man in the United States of America led to an uprising of sorts across many countries. It touched the psyche of the people in the UK and came at a time when the British Association of Physicians of Indian origin reaches its 25th anniversary (1996-2021). One of the fundamental visions and values that the organisation was created on, was to promote excellence through equality and diversity. Therefore in its silver jubilee year, BAPIO launches an over-arching Alliance for Equality in Healthcare Professions. The Alliance is tasked to conduct a review of the evidence of differential attainment across the spectrum of healthcare careers, engage with stakeholders from the grassroots to the responsible organisations and finally generate an expert consensus on recommendations for the changes necessary to tackle such inequalities.

Differential attainment exists in all professions and is a manifestation of systemic factors creating an unequal environment where individual careers and aspirations may be thwarted. Although this inequality which affects some groups of students unfairly, has been recognised over the last 2 decades, it remains a significant issue. This scoping review explores the causes and contributors in relation to undergraduate medical education. Using thematic analysis, the authors present the case for tackling the disparity in education and training. There are evidence-based solutions for individuals, organisations and at a societal level. The recommendations from this review will be discussed and debated in the series of workshops, as part of the ‘Bridging the Gap’ series from the Alliance for Tackling Inequalities in Health, chaired by the British Association of Physicians of Indian origin. The output of the consensus-building workshops and thematic synthesis with the accompanying qualitative research will be presented in the final report due in 2021.

The article describes four important non-medical areas in the profession of a physician: legal, administrative, economic and logological. These areas are complementary to the core knowledge in all medical professions, particularly of a physician. The aim of the article is to indicate some specific problems faced by a contemporary physician which, despite not being part of medical knowledge, are impossible to avoid.
