Abstract Purpose The aim of the study was to compare long-term results after 1 year in patients with single-sided deafness (SSD) who were fitted with different hearing aids. The participants tested contralateral routing of signals (CROS) hearing aids and bone-anchored hearing systems (BAHS). They were also informed about the possibility of a cochlear implant (CI) and chose one of the three devices. We also investigated which factors influenced the choice of device. Methods Prospective study with 89 SSD participants who were divided into three groups by choosing BAHS, CROS, or CI. All participants received test batteries with both objective hearing tests (speech perception in noise and sound localisation) and subjective questionnaires. Results 16 participants opted for BAHS-, 13 for CROS- and 30 for CI-treatment. The greater the subjective impairment caused by SSD, the more likely patients were to opt for surgical treatment (BAHS or CI). The best results in terms of speech perception in noise (especially when sound reaches the deaf ear and noise the hearing ear), sound localization, and subjective results were achieved with CI. Conclusion The best results regarding the therapy of SSD are achieved with a CI, followed by BAHS. This was evident both in objective tests and in the subjective questionnaires. Nevertheless, an individual decision is required in each case as to which SSD therapy option is best for the patient. Above all, the patient's subjective impairment and expectations should be included in the decision-making process.

Imbalance among patients with vestibular hypofunction has been related to inadequate compensatory eye movements in response to head movements. However, symptoms of imbalance might also occur due a temporal mismatch between vestibular and other balance-related sensory cues. This temporal mismatch could be reflected in a widened temporal binding window (TBW), or the length of time over which simultaneous sensory stimuli may be offset and still perceived as simultaneous. We hypothesized that decreased vestibular input would lead to a widening of the temporal binding window. We performed whole-body rotations about the earth-vertical axis following a sinusoidal trajectory at 0.5 Hz with a peak velocity of 60°/s in four normal subjects. Dichotic auditory clicks were presented through headphones at various phases relative to the rotations. Subjects were asked to indicate whether the cues were synchronous or asynchronous and the TBW was calculated. We then simulated decreased vestibular input by rotating at diminished peak velocities of 48, 24 and 12°/s in four normal subjects. TBW was calculated between ±1 SD away from the mean on the psychometric curve. We found that the TBW increases as amplitude of rotation decreases. Average TBW of 251 ms at 60°/s increased to 309 ms at 12°/s. This result leads to the novel conclusion that changes in temporal processing may be a mechanism for imbalance in patients with vestibular hypofunction.

Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by deficits in social communication and interaction, and restricted interests and behavior patterns. These characteristics are considered as a continuous distribution in the general population. People with ASD show atypical temporal processing in multisensory integration. Regarding the flash–beep illusion, which refers to how a single flash can be illusorily perceived as multiple flashes when multiple auditory beeps are concurrently presented, some studies reported that people with ASD have a wider temporal binding window and greater integration than typically developed people; others found the opposite or inconsistent tendencies. Here, we investigated the relationships between the manner of the flash–beep illusion and the various dimensions of ASD traits by estimating the degree of typically developed participants’ ASD traits including five subscales using the Autism-Spectrum Quotient. We found that stronger ASD traits of communication and social skill were associated with a wider and narrower temporal binding window respectively. These results suggest that specific ASD traits are differently involved in the particular temporal binding processes of audiovisual integration.

PurposeWe sought to determine whether speech perception and language skills measured early after cochlear implantation in children who are deaf, and early postimplant growth in speech perception and language skills, predict long-term speech perception, language, and neurocognitive outcomes.MethodThirty-six long-term users of cochlear implants, implanted at an average age of 3.4 years, completed measures of speech perception, language, and executive functioning an average of 14.4 years postimplantation. Speech perception and language skills measured in the 1st and 2nd years postimplantation and open-set word recognition measured in the 3rd and 4th years postimplantation were obtained from a research database in order to assess predictive relations with long-term outcomes.ResultsSpeech perception and language skills at 6 and 18 months postimplantation were correlated with long-term outcomes for language, verbal working memory, and parent-reported executive functioning. Open-set word recognition was correlated with early speech perception and language skills and long-term speech perception and language outcomes. Hierarchical regressions showed that early speech perception and language skills at 6 months postimplantation and growth in these skills from 6 to 18 months both accounted for substantial variance in long-term outcomes for language and verbal working memory that was not explained by conventional demographic and hearing factors.ConclusionSpeech perception and language skills measured very early postimplantation, and early postimplant growth in speech perception and language, may be clinically relevant markers of long-term language and neurocognitive outcomes in users of cochlear implants.Supplemental materialshttps://doi.org/10.23641/asha.5216200

Purpose Speech perception can be described as the transformation of continuous acoustic information into discrete memory representations. Therefore, research on neural representations of speech sounds is particularly important for a better understanding of this transformation. Speech perception models make specific assumptions regarding the representation of mid vowels (e.g., [ɛ]) that are articulated with a neutral position in regard to height. One hypothesis is that their representation is less specific than the representation of vowels with a more specific position (e.g., [æ]). Method In a magnetoencephalography study, we tested the underspecification of mid vowel in American English. Using a mismatch negativity (MMN) paradigm, mid and low lax vowels ([ɛ]/[æ]), and high and low lax vowels ([ i ]/[æ]), were opposed, and M100/N1 dipole source parameters as well as MMN latency and amplitude were examined. Results Larger MMNs occurred when the mid vowel [ɛ] was a deviant to the standard [æ], a result consistent with less specific representations for mid vowels. MMNs of equal magnitude were elicited in the high–low comparison, consistent with more specific representations for both high and low vowels. M100 dipole locations support early vowel categorization on the basis of linguistically relevant acoustic–phonetic features. Conclusion We take our results to reflect an abstract long-term representation of vowels that do not include redundant specifications at very early stages of processing the speech signal. Moreover, the dipole locations indicate extraction of distinctive features and their mapping onto representationally faithful cortical locations (i.e., a feature map).
