Over the last 15 years, BGC Engineering Inc. has developed and implemented a geohazards Integrity Management Program (IMP) with 12 major pipeline operators (consisting of gas and oil pipelines and of both gathering and transmission systems). Over this time, the program has been applied to the assessment of approximately 13,500 individual hydrotechnical and geotechnical geohazard sites spanning approximately 63,000 km of operating pipelines in Canada and the USA. Hydrotechnical (watercourse) and geotechnical (slope) hazards are the primary types of geohazards that have directly contributed to pipeline failures in Canada. As with all IMPs, the core objectives of a geohazard management system are to ensure a proactive approach that is repeatable and defensible. In order to meet these objectives, the program allows for varying levels of intensity of inspection and a recommended timescale for completion of actions to manage the identified geohazards in accordance with the degree of hazard that the site poses to the pipeline. In this way, the sites are managed in a proactive manner while remaining flexible to accommodate the most current conditions at each site. This paper will provide a background to the key components of the program related specifically to existing operating pipeline systems, present pertinent statistics on the occurrence of various types of geohazards based on the large dataset of inspections, and discuss some of the lessons learned in the form of program results and program challenges from implementing a geohazard integrity management system for a dozen operators with different ages of systems, complexity of pipeline networks, and in varied geographic settings.

PurposeThis study used a qualitative approach on five case studies in Brazilian industrial companies. The research used interviews, document analysis and on-site visits to collect and analyze data. The companies were selected based on the following criteria: operating in the industrial sector, updating their quality management system (QMS) process to ISO 9001: 2015 and agreeing to participate in this study.Design/methodology/approachThis article aims to investigate the strategies of industrial companies adopted for ISO-9001:2015 certification in light of the six major advances concerning the previous version. Thus, QMS of other organizations can incorporate identified lessons learned, whether certified or not.FindingsThe main finding of the research is the systematization of a set of lessons learned in the experiences of implementing the six significant advances of ISO 9001 concerning the previous version by industrial companies in the State of São Paulo in Brazil. These lessons can and should be used by other organizations to improve their QMSs.Practical implicationsThe practices identified in this empirical research can serve as benchmarking to assist quality managers from other companies in QMS certification based on ISO 9001: 2015 or even those not certified but interested in updating their QMSs. Therefore, lessons learned can significantly minimize efforts to improve your projects, processes, products and services. These findings can also help industrial companies improve their production efficiency and effectiveness through quality improvement.Originality/valueThe main novelty of the research is the consolidation of theoretical and practical analysis of the main changes in the latest version of the ISO 9001 standards. The efforts to fulfill those changes result in lessons learned. The “lessons learned” will form a new block of knowledge that will subsidize theoretical (new research) and practical (formulation of a new ISO 9001 standard and helps quality managers improve their systems).

This chapter is a report on using a course management system Desire2Learn® to facilitate the implementation of the quality assurance standards recommended by Quality Matters. After a brief introduction to the Quality Matters standards, the chapter describes how the Desire2Learn learning environment can be structured to build an interconnected framework to promote objective-oriented, criterion-referenced, dynamic assessment. A pilot study was conducted to explore how learning activities can be assessed in alignment with measurable learning objectives, with reference to criterion-based rubrics. The chapter discusses issues found through the study and lessons learned in using the competency structure of Desire2Learn to enforce objective-oriented assessment.
