Aim. To optimize the treatment of dacryocystitis in the newborns, to reduce the frequency of recurrent nasolacrimal duct catheterization. Methods. Examined were 207 children suffering from catarrhal (the first group - 57 children, 73 affected eyes), purulent (the second group - 139 children, 158 eyes) and phlegmonous (the third group - 11 children, 12 eyes) dacryocystitis. A control group was also formed - 12 children without inflammatory eye diseases. Conducted was a clinical examination, determined was the lysozyme activity of the tears, analyzed was the effectiveness of medicinal and surgical treatment, and conducted was a microbiological investigation with the study of sensitivity of the pathogenic microorganisms to antibacterial drugs. Results. In catarrhal dacryocystitis of the newborns identified were seven species of microorganisms, most commonly found were S. epidermidis (more than 50%) and S. aureus (12.4%), in 19.2% of cases no pathogen was identified. The etiology of purulent dacryocystitis was more diverse - 23 species of pathogens, the leading pathogens were S. aureus (15.2%), S. pneumoniae and S. epidermidis (13.9% each), E. faecium (8.2%). In the phlegmonous form of the disease revealed were 4 types of pathogens, most commonly S. epidermidis (45.5%) and S. aureus (27.2%). In 9.1% of cases no pathogen was identified. The causative agents of catarrhal and purulent dacryocystitis of the newborns showed the greatest sensitivity to pikloxydine (100% and 93.3%), chloramphenicol (86.7% and 79.7%) and ciprofloxacin (87.0% and 72.2%). Revealed was the reduction of lysozyme activity of the tears both in the affected and in the unaffected eyes of children with dacryocystitis. During the analysis of the results of surgical treatment the frequency of relapses in the first group (catarrhal dacryocystitis of the newborns) was 8.3%, while in the second group (purulent dacryocystitis) it was significantly higher - 13.7%. Conclusion. In order to reduce the frequency of relapses conservative treatment should be conducted with respect to the microbial etiology, forms of the disease, and sensitivity of pathogens to antimicrobial agents before catheterization of the nasolacrimal duct.

42 children at age from 1 to 15 years (36 - with relapse of congenital nasolacrimal duct (NLD ) stenosis and 6 - with canalicular stenosis) bougienage NLD was done with Ritleng stent intubation. The control group included 25 children 1-5 years old with relapsed congenital stenosis of the NLD, the treatment of which was limited to probing the NLD. In the first group full functional effect was observed in 21 children (58.3 %), in 9 (25.0 %) - was light epiphoria, in 1 (2.8 %) - moderate, and in 5 (13.9 % ) treatment was not effective. In the second group a full functional effect was obtained for each child. In the control group lacrimal drainage was restored fully in 16 (64.0 %) children, in 2 (8.0 %) - moderate and in 7 (28.0 %) - severe epiphora was remained. Using of silicone intubation with the Ritleng lacrimal system allows to continuously retain their foramen in stenotic site without causing an inflammatory response and bedsores lacrimal punctum and canaliculus.

Purpose of the work. improving the results of surgical treatment of patients with primary intra-abdominal infiltrates and abscesses. Material and methods. From 2006 to 2019, 191 patients with primary intra-abdominal infiltrates and abscesses were treated. The patients' age ranged from 16 to 85 years. There were 96 male patients (50.26%), 95 female patients (49.74%). Results. The patients were divided into 3 subgroups depending on the underlying disease. The first group included 74 (38.74%) patients with destructive appendicitis, of which 39 (20.42%) were in the control group, and 35 (18.32%) were studied. The second group included 48 (25.13%) patients suffering from perforated gastric ulcer and 12 duodenal ulcer, of which the control group was 26 (13.61%), and the studied group was 22 (11.52%). The third group included 69 (36.13%) patients with cholecystitis, of which 37 (19.37%) were in the control group, and 32 (16.76%) were studied. All patients were operated on. Conclusions. Surgical treatment is individualized depending on the disease, so with destructive appendicitis from 74 (38.74%) laparotomic in 42 (21.99%), laparoscopic in 32 (16.75%), and in 12 (6.28%) with conversion; perforated gastric ulcer and 12 duodenal ulcer in 48 (25.13%) open laparotomy; with cholecystitis from 69 (36.13%) in 48 (25.13%) laparotomic and in 21 (11.00%) laparoscopically. The use of water-jet technologies in 64 (33.51%) patients made it possible to minimize damage to the serous membrane and cleanse the peritoneum from acquired formations.

Purpose: comparative study of trabeculectomy results with various models of domestic “Glautex” drainage in the surgical treatment of primary open angle glaucoma (POAG). Patients and methods. The results of surgical treatment of 98 (105 eyes) patients with POAG aged 50–83 years who underwent trabeculectomy with various Glautex drainage models were analyzed. There were 43 men (43.9 %) and 55 (56.1 %) women. The second (II) stage of POAG was diagnosed in 49 eyes (46.7 %), the third (III) stage in 56 (53.3 %). All patients were divided into 3 groups. The first (main) group consisted of 34 (37 eyes) patients who underwent trabeculectomy in combination with the implantation of Glautex DDA drainage model. The second (main) group included 29 (30 eyes) patients with trabeculectomy and implantation of the SDA model of this drainage. The third group was the control group and consisted of 35 (38 eyes) patients with the classical method of surgery without using any drainage. Results. There was a significant 71.7 % decrease in IOP compared with baseline data in the 1st patients group after antiglaucomatous surgery, 72 and 74 % decrease was in the 2nd and 3rd groups respectively (p < 0.05). An increase in IOP was noted predominantly in the control and in the second study group within 1 month of follow-up. Normalization of ophthalmotonous pressure was achieved by using needling in 13.3 % (4/30) patients in the second group, in 7.9 % (6/38) cases in the control group. The absolute hypotensive effect in the 1st group was noted in 75.7 % of cases; the relative hypotensive effect was in 8.1 %; total failure was in 16.2 %. In the 2nd group the absolute hypotensive effect of the surgery was in 73.3 % of 30 cases, the relative was in 6.7 %, total failure was in 20 %. In the control group (38 eyes), absolute success was in 63.1 %, the relative hypotensive effect was in 13.2 %, and the total failure was in 23.7 %. Conclusion. Trabeculectomy with Glautex drainage and with various models in case of primary open-angle glaucoma provided a sufficiently high relative hypotensive effect in 82 % of cases in a year after the surgery. The achieved surgery outcomes with this drainage in the studied periods did not depend on applied model: DDA or SDA (83.8 and 80 % respectively), but was higher than the classical trabeculectomy (76.3 %). However, in case of the SDA model, needling was required in 13.3 % of cases in the early postoperative period.

A 3-year-old, castrated male golden retriever was presented for evaluation of recurrent ocular discharge of 4 months’ duration from the left eye. Dacryocystorhinography was performed and demarcated a cystic dilatation of the left nasolacrimal duct with obstruction of the duct distal to the cystic cavity. Surgical exploration of the left maxillary sinus was performed to confirm the diagnosis and reestablish drainage into the nasal cavity. Recovery was uneventful, and the dog has been asymptomatic for &gt;36 months postoperatively. This report documents the third published case of surgical treatment for cystic dilatation of the nasolacrimal duct.

Background. The frequency of deep cervical burns in children is four times higher than that of deep face burns. Currently, there is no consensus on the methods for surgical treatment of deep burns in cervical areas; meshed skin autografts continue to be used. Aim. To evaluate the benefits of early surgical treatment of deep сervical burns in children between the third and fifth days from the moment of injury. Materials and methods. Case-control study. Surgical treatment was performed in 81 children with deep cervical burns. The main group with early surgical treatment included 46 children and underwent surgical treatment at 3.37 0.14 days from the moment of injury; the control group received autograft during stage treatment for 35 children at 27.17 0.18 days. The treatment results were evaluated by the following indicators: the number of dressing changes, the period of skin restoration, and the area of graft success. In the long term, functional and cosmetic treatment results were evaluated. Results. In the study and control groups, 7.93 0.45 and 18.75 0.61 dressings were required to complete the treatment, respectively (p 0.001). The skin restoration periods were 16.54 0.68 and 36.94 0.89 days, respectively (p 0.001). The graft success areas were 99.50% 0.13% in the main group and 93.91% 2.68% in the control (p 0.001). During the staged surgical treatment, one patient showed a loss of 90% of the graft, which required regrafting. Other complications in the treatment process have not been noted. When assessing long-term cosmetic results using the Vancouver Scar Scale, the average score was 4.0 0.26 points in the main group and 7 0.28 points in the control (p 0.001). The presence of post-burn cicatricial contracture in the main group was noted in 12 (26%) people and the absence in 34 (74%) children. In the control group, 20 (57%) patients required surgical removal of post-burn deformity, and 15 (43%) children did not need further surgical interventions. Conclusions. Early surgical treatment of deep cervical burns in children on the third and fifth days from injury allows not only to accelerate the process of restoration of the skin but also to directly affect the cosmetic and functional results in a better way.

Over the past two years (2019, 2020), we have studied 660 milk samples from cows with various forms of mastitis, delivered from 50 dairy farms and complexes. At the same time, 1240 microbiological studies were performed, as a result of which 248 cultures of pathogenic and conditionally pathogenic microorganisms were isolated, namely: 146 (58,8%) cultures of Staphylococcus aureus, 33 – (13,3%) conditionally pathogenic staphylococci, 31 – (12,5%) Streptococcus agalactiae, 13 – (5,2%) Streptococcus dysgalactiae, 25 – (10%) of the family of Enterobacteriaceae. For 2019 and 2020 the allocation percentage of Staphylococcus aureus was 58,8%, which exceeds the allocation percentage of conditional pathogenic staphylococci 4,4 times, agalactiae 4,7 times and dysgalactiae Streptococcus 11,3%, enterobacteria, 5,9 times, respectively. After examining the areas of growth retardation of Staphylococcus aureus to antimicrobial agents, both intracisternal and intramuscular, on the example of nine dairy farms in 2019 and 2020, we revealed different resistance of the pathogen in each of the farms we examined. When analyzing the sensitivity of Staphylococcus aureus to antimicrobial drugs, including complex ones, their active substance was taken into account. As a result of the conducted studies, it was found that in all farms, the greatest sensitivity of Staphylococcus aureus was noted to norfloxacin (35,3 mm), neomycin (33,2 mm), amoxicillin (34,5 mm), benzylpenicillin-procaine (33,6 mm), marbofloxacin (34,5 mm). The lowest sensitivity of the microbe was found to cloxacillin (26,6 mm), dioxidin (22,5 mm), and spiramycin (25,5 mm). Thus, Staphylococcus aureus has different resistance to antibiotics, and the use of antimicrobial agents with low effectiveness can lead to a chronic form of mastitis. In this regard, and to achieve effective treatment, it is necessary to identify the causative agents of inflammation of the mammary gland of cows, as well as to analyze their sensitivity to the antibacterial drugs used in the farm. Turkish dermatologist, H. Beheet described the disease as recurrent triad of iritis, oral aphthous lesions and genital ulceration. Auto immune disease is the recent focus on the unknown etiology which is still being discussed. Among the other immunosupressive drugs, CyA included in it's treatment newly. One of the important side effects of this drug is gingival hyperplasia which has a direct relation with the presence of teeth and periodontal tissue. We are interested in the ultrastructure of immunocompetent target cells that were affected by CyA in BD.Three groups arranged in each having 5 patients with BD. Control group was the first and didn’t have CyA treatment. Patients who had CyA, but didn’t show gingival hyperplasia assembled the second group. The ones displaying gingival hyperplasia following CyA therapy formed the third group. GMC of control group and their granules are shown in FIG. 1,2,3. GMC of the second group presented initiation of supplementary cellular activity and possible maturing functional changes with the signs of increased number of mitochondria and accumulation of numerous dense cored granules next to few normal ones, FIG. 4,5,6.

This study was conducted as an attempt to discover the effect of applying Demonstration Method on students’ achievement in speaking skill. It was an experimental research. The subject was students of Grade XII, Vocational High School (Sekolah Menengah Kejuruan: SMK), which consisted of 79 students. The research was divided into two groups: experimental and control groups. The instrument used to collect the data was speaking test. To obtain the reliability of the test, the writer applied Kuder Richardson 21 formula. The result of the reliability was 0.7, and it was found that the test was reliable. The data were analyzed by using t-test formula. The analysis showed that the scores of the students in the experimental group were significantly higher than the scores of the students in the control group at the level of significant m = 0.05 with the degree of freedom (df) 77, t-observed value 8.9 > t-table value 1.99. The findings indicate that using Demonstration Method significantly affected the students’ achievement in speaking skill. So, English teachers are suggested to use Demonstration Method in order to improve students’ achievement in speaking skill.

С течением времени подходы к изучению резистентности к антибиотикам трансформировались от сосредоточения на выделенных в виде чистой культуры патогенных микроорганизмах к исследованию резистентности на уровне микробных сообществ, составляющих биотопы человека и окружающей среды. По мере того, как продвигается изучение устойчивости к антибиотикам, возникает необходимость использования комплексного подхода для улучшения информирования мирового сообщества о наблюдаемых тенденциях в этой области. Все более очевидным становится то, что, хотя не все гены резистентности могут географически и филогенетически распространяться, угроза, которую они представляют, действительно серьезная и требует комплексных междисциплинарных исследований. В настоящее время резистентность к антибиотикам среди патогенов человека стала основной угрозой в современной медицине, и существует значительный интерес к определению ниши, в которых бактерии могут получить гены антибиотикорезистентности, и механизмов их передачи. В данном обзоре мы рассматриваем проблемы, возникшие на фоне широкого использования человечеством антибактериальных препаратов, в свете формирования микрофлорой кишечника резервуара генов резистентности. Over the time, studies of antibiotic resistance have transformed from focusing on pathogenic microorganisms isolated as a pure culture to analysis of resistance at the level of microbial communities that constitute human and environmental biotopes. Advancing studies of antibiotic resistance require an integrated approach to enhance availability of information about observed tendencies in this field to the global community. It becomes increasingly obvious that, even though not all resistance genes can geographically and phylogenetically spread, the threat they pose is indeed serious and requires complex interdisciplinary research. Currently, the antibiotic resistance of human pathogens has become a challenge to modern medicine, which is now focusing on determining a potential source for bacterial genes of drug resistance and mechanisms for the gene transmission. In this review, we discussed problems generated by the widespread use of antibacterial drugs in the light of forming a reservoir of resistance genes by gut microflora.
