ABSTRACT The use of clomiphene to assess ovarian function in women has been monitored by the determination of plasma oestradiol and progesterone concentrations. The assay of both hormones was carried out on one plasma sample after separation of oestradiol and progesterone by TLC. The oestradiol was determined by double competitive binding to rabbit uterine supernatant (Robertson et al. 1971) and the progesterone by competitive binding to corticosteroid binding globulin. After clomiphene stimulation the oestradiol levels in the follicular and luteal phases exceeded those found in normal cycles; this was also true of progesterone concentrations during the luteal phase. The methods of assay were also used to follow changes in plasma hormone levels during the early stages of pregnancy in hitherto infertile women. These cases included a twin pregnancy and a pregnancy which terminated in abortion at 7 weeks.

ABSTRACT Oestradiol and progesterone were measured in plasma of infertile women treated with human menopausal gonadotrophin (HMG) and human chorionic gonadotrophin (HCG). A total of 27 courses of treatment from 4 women were investigated. Plasma oestradiol was determined by a double competitive binding method and progesterone by competitive binding to corticosterone binding globulin. A relationship exists between the levels of plasma oestradiol in the follicular phase and the occurrence of ovulation as determined by plasma progesterone measurements in the luteal phase. Ovulation was not detected unless the oestradiol levels were in excess of 30 ng/100 ml plasma on the 7th day of treatment. The value of monitoring women undergoing gonadotrophin treatment by this procedure is discussed.

ABSTRACT An effect of D-Trp6-LH-RH (superactive LH-RH agonist) in the pituitary and ovarian function was examined. Four regularly cycling baboons were used for this study. After determination of control values of plasma levels of LH, oestrogen and progesterone during the entire menstrual cycle, D-Trp6-LH-RH was infused subcutaneously for 7 days during the early luteal phase in these four baboons. An infusion of D-Trp6-LH-RH increased plasma LH and oestrogen, but it failed to alter the plasma level of progesterone. From these results, it seems unlikely that 1) D-Trp6-LH-RH has a luteolytic effect, and 2) an increased ovarian oestrogen causes luteolysis in the baboon.

SummaryTwo quantitative procedures for autoprothrombin C are described. In one of these purified prothrombin is used as a substrate, and the activity of autoprothrombin C can be measured even if thrombin is in the preparation. In this procedure a reaction mixture is used wherein the thrombin titer which develops in 20 minutes is proportional to the autoprothrombin C in the reaction mixture. A unit is defined as the amount which will generate 70 units of thrombin in the standardized reaction mixture. In the other method thrombin interferes with the result, because a standard bovine plasma sample is recalcified and the clotting time is noted. Autoprothrombin C shortens the clotting time, and the extent of this is a quantitative measure of autoprothrombin C activity.

Abstract. Sex hormone binding globulin (SHBG) levels were studied for possible effects of oestradiol-17β on SHBG. No change in SHBG plasma was recorded during normal menstrual cycles or during treatment with oestradiol-17β to menopausal women. However, gonadotrophin treatment to amenorrhoeic women to induce ovulation resulted in high oestradiol concentrations and a pronounced increase in SHBG was found during the luteal phase of these cycles. A marked increase of SHBG was also recorded in a woman with pronounced fluctuations of oestradiol during treatment with levonorgestrel sc implants for contraception. In conclusion, effects on SHBG were only found when extraordinarily high levels of plasma oestradiol were recorded.

Abstract Study question Is there a relationship between the serum progesterone and estradiol levels and certain morphological characteristics of human endometrium during the mid-luteal phase? Summary answer Serum progesterone is associated with the stromal edema and the abundance and size of basal vacuoles in the endometrium of women during the mid-luteal phase. What is known already Progesterone and estrogen are essential hormones that are necessary to prepare the endometrium for pregnancy. Their serum concentrations during the mid-luteal phase are important criteria for prediction of successful embryo implantation. In addition, a variety of endometrial morphological markers, such as the presence of pinopodes, subnuclear and supranuclear vacuoles, glandular secretion, and stromal edema have been applied for determination of the window of implantation and endometrial receptivity. However, the relationship between these endometrial morphological characteristics and serum levels of progesterone and estradiol is still scarcely studied. Study design, size, duration This is an observational study of 98 women, 25 to 46 years of age (mean 37 years), who had a blood sample and an endometrial biopsy during the mid-lutheal phase (LH + 7) in a natural cycle. The study was conducted between August 2020 and November 2020. Participants/materials, setting, methods Serum progesterone and estradiol were measured by electrochemiluminescence immunoassay (ECLIA) on the Cobas e411 analyser (Roche Diagnostics, Germany). The following endometrial morphological characteristics were assessed using light microscopy: (1) basal vacuoles (mean size and percentage of vacuolated glandular cells) (2) apical vacuoles (mean size and percentage of vacuolated glandular cells), (3) pinopodes (percentage of luminal epithelium covered in pinopodes), (4) glandular intraluminal secretion (6-level scoring system), (5) stromal edema (6-level scoring system). Main results and the role of chance The serum progesterone levels ranged between 0.39 and 145.3 ng/ml, with a median of 24.36 ng/ml. The serum estradiol levels varied between 26.91 and 842.89 pg/ml with a median of 124.75 pg/ml. The percentage of cells with basal vaculoles ranged from 0 to 90%, with a median of 38.57%, apical vacuoles (0–50%, 16.83%), pinopodes (0–80%, 23.87%), glandular intraluminal secretion (0–80%, 28.57%), and stromal edema (1–6, 1.42). To examine the association between the serum progesterone and estradiol and the studied endometrial morphological characteristics, the Spearman’s Rho Correlation coefficient for non-paramentric data was used. No correlation was found between serum estradiol levels and the studied morphological variables (p > 0.05). In contrast, the serum progesterone concentration showed a significant negative correlation with the percentage of glandular epithelial cells with basal vacuoles (R= - 0.28; p = 0.03), the mean size of the basal vacuoles (R= - 0.24; p = 0.5) and a significant positive correlation with the stromal edema (R = 0.34; p < 0.01). Limitations, reasons for caution The study was limited in sample size. Wider implications of the findings: The results of this study revealed that serum progesterone is more strongly associated with the occurrence of certain endometrial morphological characteristics during the mid-luteal phase than serum estradiol. These findings are valuable for development of new methods for accurate determination of the window of implantation. Trial registration number Not applicable

AbstractObjective. The aim was to determine the chorionic and amniotic types in multifetal pregnancies with transvaginal ultrasonography at very early stage of gestation.Study design. Twenty-one spontaneous multifetal pregnancies were scanned transvaginally before 8 weeks' gestation (four of them from 4th week). The chorionic and amniotic type was determined ultrasonographically. All twin gestations had postpartum pathologic evaluation of the placenta and histologic determination of the chorionic and amniotic type.Results. Ultrasonographic evaluation of the 21 pregnancies demonstrated 20 twin and 1 triplet gestation. Four of the twin pregnancies were monochorionic-diamniotic. Triplet was monochorionic-triamniotic (spontaneously aborted in 8th week of gestation). In all 20 twin pregnancies, transvaginal ultrasonography correctly predicted the chorionic and amniotic type before 8 weeks of gestation.Conclusion. Transvaginal ultrasonography allows a reliable, simple and rapid determination; the dichorionic twin pregnancy in 4 weeks, monochorionic in 5 weeks, and differentiation of mono-or diamniotic in 7 weeks of gestation.

Abstract We describe here a direct a radioimmunoassay (RIA) for the determination of 6-sulfatoxymelatonin (aMT6s) in plasma, with iodinated aMT6s as tracer. The aMT6s antiserum was raised in rabbit by immunization with a bovine serum albumin conjugate, giving negligible cross-reactivities for related compounds. The low limit of detection (15 pmol/L) allowed a direct assay that required only a 100-microL plasma sample. Dilutions of plasma and of synthetic aMT6s gave the same parallel response in the RIA. A preliminary study showed a circadian variation in healthy volunteers, with mean concentrations ranging from 52 (at 1600-2100 h) to 378 pmol/L (at 0400 h), whereas this rhythm was abolished in pinealomectomized patients. After administration of melatonin orally, or by infusion, the aMT6s concentrations in plasma concorded with previous data on aMT6s production and pharmacokinetics, with aMT6s being cleared from plasma more slowly than melatonin. This assay should have practical application in the development of new pharmaceutical formulations that minimize the hepatic metabolism of melatonin.

ABSTRACT Menstrual cycle profiles of salivary progesterone were derived from daily samples obtained from 75 postmenarcheal girls aged between 12 and 17 years. Data were analysed to establish the proportion of girls exhibiting ovulatory cycles in each of several age groups following menarche or in groups based solely on chronological age. 'Luteal'-phase profiles of salivary progesterone, which were similar in shape to those characteristic of mature premenopausal women, were taken to indicate the probable occurrence of ovulation. The percentage of cycles in those subjects judged to be ovulatory, grouped according to gynaecological age, rose from 14% in the first year to 56% in the fourth year. Those classified according to chronological age, and where sufficient data permitted reliable estimates of the percentage of subjects ovulating, rose from 11% at 12 years of age to 64% for girls aged 16. These data are in reasonable concordance with published data using plasma progesterone and urinary methods for establishing ovulatory incidence, and indicate the value of salivary progesterone measurements in studies of ovarian function in adolescence.
