Abstract We report on the case of a 40-year-old male who was admitted to the clinic with a large ulcer on his left buttock, 3 days after an intramuscular benzathine penicillin injection. The patient was diagnosed with Nicolau syndrome, a rare vascular complication in which a lesion develops after intramuscular injection. Symptoms are intense pain at the injection site, erythema, and livedoid dermatitis, which leads to necrosis of skin, subcutaneous tissue and muscle tissue. It was described by Nicolau after intramuscular injections of bismuth salt for syphillis therapy. Nicolau syndrome is rare, but its symptoms are devastating and healthcare professionals must be aware of this clinical entity, since intramuscular injections are common procedures for administration of drugs.

Purpose We report a patient with basal cell carcinoma presenting with severe myiasis in a large ulcer involving the upper and lower eyelid. Methods Myiasis is an infestation of vertebrate animals by larvae of certain fly species. About 70 larvae were removed manually. A biopsy of the tissue underneath demonstrated a basal cell carcinoma. The myiasis was produced by the fly Lucilia sericata, currently used for treating chronic nonhealing ulcers. Because of the stage of the carcinoma, an orbital exenteration was carried out. Results The patient died 2 days later because of cardiopulmonary failure not related to the myiasis. Conclusions We present a case of a severe orbital myiasis focusing on its management and life-threatening nature.

Genital tuberculosis (TB) in women is a chronic disease with low-grade symptoms. Genital tract tuberculosis is usually secondary to extragenital TB. The fallopian tubes are most commonly affected, and along with endometrial involvement, it causes infertility in such patients. Involvement of the cervix and the vulva is very rare. We present one such rare case of vulvar tuberculosis presented with a large ulcer diagnosed on histopathology and treated with antitubercular chemotherapy.

A 60-year-old, otherwise healthy, male farmer presented to our Dermatology Department with a large ulcer on his lower right leg. The lesion had started as a small papule 6 months before, which became eroded and transformed into a rather rapidly progressive ulcer. On careful inspection, numerous larvae were found moving within the wound. The larvae were analyzed and found to be Lucilia sericata (the green bottle blowfly). The lesion was diagnosed histopathologically as squamous cell carcinoma. The myiasis was treated by submerging the wound in a dilute permanganate potassium solution.

Riga-Fede disease is a rare, benign disorder characterized by reactive ulceration of the oral mucosa associated with repetitive dental traumatism. It was first described in children with neurologic disorders and is very rare in adults. This case report describes the occurrence of a large ulcer of the tongue, resembling squamous cell carcinoma, in an adult with hemiparesis. The lesion cleared after neurologic recovery. This case highlights the importance of considering this disorder in the differential diagnosis of oral mucosal ulcerations.

This case report is on a very rare case of giant anal canal ulcer with rectal prolapse causing total faecal incontinence. An elderly patient in her eighties presented to the emergency department with complaints of a mass protruding thorough anus along with fresh rectal bleeding. Examination revealed a large ulcer measuring 6 cm × 8 cm involving the entire anal canal with rectal prolapse. Here, we discuss the management of this patient with a rare cause of giant anal canal ulceration.

Anisakis simplex B larvae and adults occurred in the forestomach of 7 of 10 Phocoena phocoena found drowned in cod nets in the winters (November–January) of 1977–1978, 1980–1981, 1981–1982, and 1983–1984, along the Scottish east coast. A cluster of third- and fourth-stage larvae was found in one large, crater-like ulcer in the wall of the ventral "hemisphere" of the forestomach in five porpoises. A sixth infected animal had one large ulcer bearing a larval nematode cluster, plus, at a separate site in the ventral hemisphere, an aggregation of six very small ulcers, three of which contained larvae. The remaining infected porpoise harboured a large multiple ulcer at a single site in the ventral hemisphere, each of its five craters bearing a cluster of larvae. Adult male and female nematodes were apparently "free" in the forestomach lumen. One third-stage larva of Contracaecum sp. was the only other parasite found in the gastrointestinal tract of the 10 porpoises. Whiting (Merlangius merlangus) was the dominant prey item, and may have been the source of A. simplex B infection.
