Algeria Islamists may boost influence at May election

Significance Since the elections of 1991, which led to a decade of civil war, parliamentary polls have generally taken place in a tense atmosphere, with protests, riots and boycotts. As the health of 80-year-old President Abdelaziz Bouteflika remains precarious, the regime seeks to demonstrate stability and security to the population and the international community by conducting the elections as smoothly as possible. Impacts Secular opposition parties are fragmented and unable to ally or challenge the governing FLN and RND. Islamist parties are likely to increase their presence in parliament but are unlikely to receive more than one-third of the 462 seats. Islamic values may play a more important role in educational, social and judiciary matters (eg, family rights).

Significance These results indicate that the incumbent A Partnership for National Unity-Alliance for Change (APNU-AFC) coalition has narrowly won re-election, returning David Granger as president for another term. However, opposition parties have contested the results, with the international community also voicing concerns about the conduct and outcome of the elections. Impacts The contested election count is likely to lead to an overhaul of GECOM staffing and procedures once the current dispute is settled. Companies are set to hold off new investments in Guyana until the incoming government and policy stance are confirmed. The fraud allegations will also focus attention on May elections in neighbouring Suriname, whose ruling party is also seeking re-election. Control over expected windfall oil revenues will make this election result especially hotly contested.


Subject Constitutional reform in Singapore. Significance Parliament is expected in August to receive recommendations on electoral and constitutional changes that will affect opposition politics and executive powers. The government raised the initiatives in parliament in January; Prime Minister Lee Hsien Loong said they would be beneficial to Singapore and its government. However, the measures are also likely to benefit the People's Action Party (PAP), which has ruled Singapore since independence in 1965. Impacts Presidential elections are unlikely to become more competitive. Opposition parties would gain parliamentary exposure if the reform recommendations are enacted. Government scrutiny of corporations' activities in the community and local politics will intensify.


Subject Bukele and congress. Significance Salvadoran troops on February 9 occupied the Legislative Assembly at the invitation of President Nayib Bukele. The occupation was intended as a high-profile stunt to pressure legislators into approving Bukele’s request for extra security funding. In reality, it revived memories of the country’s brutal civil war, sparking domestic and international criticism, undermining Bukele’s domestic and international image and probably bringing his honeymoon period to an abrupt end. Impacts The military is now less likely to take part in large-scale security operations given concerns raised by its visible presence. Bukele’s new anti-corruption body is set to receive less funding and powers via congressional grant. Bukele will maintain his antagonistic stance ahead of the 2021 elections, where he hopes to boost his party’s legislative presence.


Significance Violent protests engulfed the capital city last week as police clashed with demonstrators demanding President Joseph Kabila honour presidential term limits. The president's intransigence has driven the violent reaction, but divisions within the opposition are also fuelling the violence. Impacts Recent violence may have ended the prospect of opposition unity in the near term. The DRC is unlikely to experience a third civil war but security may deteriorate, especially in Kinshasa. The international community may broker peace talks led by the African Union. The increasing instability will almost certainly shatter the DRC's fragile economic recovery.


Subject The Central African Republic's Truth, Justice, Reconciliation and Reparations Commission. Significance The Central African Republic (CAR) parliament in late February passed a bill establishing the Truth, Justice, Reconciliation and Reparation Commission (TJRRC). The TJRRC is the main justice initiative envisaged under the February 2019 peace agreement between the government and 14 armed groups. However, the TJRRC will face significant political hurdles to delivering on its lofty premise. Impacts It is unlikely that victims will receive reparations without significant technical and financial backing from the international community. Due to its Bangui-centric approach, the TJRRC may neglect abuses committed by the government and armed groups in outlying regions. During the election period, the TJRRC could be employed as a political threat against opposition parties.


Subject Islamic State group's expansion prospects Significance Over the past sixteen months, the international community has focused its campaign to counter the Islamic State group (ISG) primarily on Iraq and Syria. This approach overlooks ISG's accelerating regional affiliate programme, which gives ISG strategic resiliency outside of its "caliphate" within the two main countries. A strategy to defeat ISG cannot succeed without addressing its formal affiliates in Libya, Egypt, Afghanistan and elsewhere. Impacts ISG's affiliate in Egypt will rekindle ties with extremist groups on mainland Egypt in order to launch more attacks. ISG will encourage new supporters globally to launch spectacular terrorist attacks. Russia's escalation in Syria will aid ISG's expansion by degrading the Syrian opposition and ensuring the continuation of the civil war.

ABSTRACTVaborbactam (formerly RPX7009) is a member of a new class of β-lactamase inhibitor with pharmacokinetic properties similar to those of many β-lactams, including carbapenems. The pharmacokinetics and safety of vaborbactam were evaluated in 80 healthy adult subjects in a first-in-human randomized, placebo-controlled, double-blind, sequential single- and multiple-ascending-dose study. A total of 10 dose cohorts were enrolled in the study, with 6 subjects randomized to receive 250 to 2,000 mg of vaborbactam and 2 subjects randomized to receive placebo in each cohort. Maximum concentrations for vaborbactam were achieved at the end of the 3-h infusion. Vaborbactam exposure (Cmaxand area under the concentration-time curve [AUC]) increased in a dose-proportional manner following multiple doses. There was no evidence of accumulation with multiple doses, consistent with the terminal half-life of ∼2 h. Both the volume of distribution (Vss) and plasma clearance were independent of dose. For the 2,000-mg dose, the plasma clearance was 0.17 ± 0.03 liters/h, the AUC from 0 h to infinity (AUC0–∞) was 144.00 ± 13.90 mg · h/liter, and theVsswas 21.80 ± 2.26 mg · h/liter. Urinary recovery was 80% or greater over 48 h across all dose groups. No subjects discontinued the study due to adverse events (AEs), and no serious AEs (SAEs) were observed. All AEs were mild to moderate and similar among the vaborbactam- and placebo-treated subjects, with mild lethargy as the only unique AE reported with the high dose of vaborbactam. Overall, this study revealed the safety, tolerability, and pharmacokinetic profile of vaborbactam and formed the basis for advancement into patient studies in combination with meropenem, including treatment of patients with carbapenem-resistantEnterobacteriaceae(CRE) infections. ABSTRACTChloroquine combined with primaquine has been the standard radical curative regimen forPlasmodium vivaxandPlasmodium ovalemalaria for over half a century. In an open-label crossover pharmacokinetic study, 16 healthy volunteers (4 males and 12 females) aged 20 to 47 years were randomized into two groups of three sequential hospital admissions to receive a single oral dose of 30 mg (base) primaquine, 600 mg (base) chloroquine, and the two drugs together. The coadministration of the two drugs did not affect chloroquine or desethylchloroquine pharmacokinetics but increased plasma primaquine concentrations significantly (P≤ 0.005); the geometric mean (90% confidence interval [CI]) increases were 63% (47 to 81%) in maximum concentration and 24% (13 to 35%) in total exposure. There were also corresponding increases in plasma carboxyprimaquine concentrations (P≤ 0.020). There were no significant electrocardiographic changes following primaquine administration, but there was slight corrected QT (QTc) (Fridericia) interval lengthening following chloroquine administration (median [range] = 6.32 [−1.45 to 12.3] ms;P< 0.001), which was not affected by the addition of primaquine (5.58 [1.74 to 11.4] ms;P= 0.642). This pharmacokinetic interaction may explain previous observations of synergy in preventingP. vivaxrelapse. This trial was registered at ClinicalTrials.gov under reference number NCT01218932.

AbstractHaving been supressed and denied their rights by successive Syrian governments over the years, Syrian Kurds are now asserting ade factoautonomy. Since the withdrawal of the Syrian President's forces from the ethnically Kurdish areas in the early months of the current civil war, the inhabitants have declared a self-rule government along the lines of the Kurdistan regional government in northern Iraq. For Syrian Kurds, the creation of a small autonomous region is a dream fulfilled, albeit one unrecognized by the international community. Some 15% to 17% of the Syrian population is Kurdish. Whether they can achieve statehood will depend on a reading of international law and on how the international community reacts. There are certain aspects which differentiate Kurdish self-rule in Syria from its counterpart in Iraq.


Significance The opposition has rejected the results and is preparing legal challenges to Museveni’s victory. Impacts NRM internal cohesion will fray as Museveni becomes a more polarising figure in national politics. The NRM will increasingly try to co-opt opponents to neutralise the growing momentum and collaboration among opposition parties. The government will likely launch developmental efforts to tackle youth unemployment and thus avert unrest.
