Nervous control of mammalian salivary glands

In the production and flow of saliva, sympathetic and parasympathetic nerves generally cooperate, although variations between the different salivary glands are considerable, particularly in the sympathetic innervation. In the submandibular gland of the dog, sympathetic impulses cause secretion via β-adrenoceptors, and since sympathetic motor effects are elicited via α-adrenoceptors it is possible to study separately motor and secretory effects in this gland. Such experiments indicate that myoepithelial contractions serve to accelerate the salivary flow and to support the secreting acinar cells and prevent back-flow of fluid from the luminal system into the glandular tissues. The contractions are elicited reflexly from the oral mucosa together with secretion. A potentiation interaction between sympathetic and parasympathetic nerves occurs in the formation of the primary saliva. In parotid glands of rabbits and rats such an interaction has been demonstrated in the secretion of amylase.

Mouse submandibular glands show an androgen-dependent sexual dimorphism, reflected in higher concentrations in males than in females of bioactive peptides, such as epidermal growth factor (EGF), nerve growth factor, and renin in the cells of the granular convoluted tubules (GCT). Biochemical studies have demonstrated androgen receptors in submandibular gland and other androgen-responsive organs in mouse. We have determined the cellular localization of these receptors using steroid autoradiography. Fifteen adult gonadectomized male mice were injected intravenously with 0.13 microgram or 0.26 microgram [3H]-dihydrotestosterone (SA 135 Ci/mM); some animals were pre-treated with cyclocytidine to stimulate secretion by GCT cells. Animals were killed 15 min, 1, 2, or 3 hr after isotope injection. Steroid autoradiographs were prepared, and some were stained immunocytochemically for EGF. Of the different cell types of submandibular gland, the acinar cells most frequently and intensely concentrated [3H]-DHT; GCT cells also concentrated the hormone, as did a small number of striated duct cells. In the other major salivary glands, the only cells that concentrated the androgen were interlobular striated duct cells in sublingual gland. In prostate, anterior pituitary, and brain a large number of cells concentrated androgen, as has been previously reported. Androgen binding by the GCT cells was a predictable finding, since androgen-induced alterations in composition and form of these cells are well documented. The intense androgen concentration by the acinar cells was an unexpected finding and suggests a hitherto unknown androgen regulation of these cells. An incidental finding was intense concentration of [3H]-DHT in the nuclei of the endothelial cells of the post-capillary venules of the cervical lymph nodes.

Autotransplantation of the submandibular gland is effective for severe keratoconjunctivitis sicca. However, most transplants show decreased secretion shortly after the operation, which leads to obstruction of Wharton’s duct. The hypothesis that decreased catecholamine release due to denervation contributes to hypofunction in the early phase was tested in transplanted glands in rabbits. We found that salivary flow, expression of β1- and β2-adrenoceptor, and the maximum binding capacity were markedly decreased in the transplanted glands. Isoproterenol significantly reversed the decreased secretion, enhanced the expressions of β1- and β2-adrenoceptor, and ameliorated the atrophy of acinar cells. The contents of cAMP and phospho-ERK 1/2 were increased after isoproterenol treatment. These results indicate that lack of β-adrenoceptor stimulation is involved in early dysfunction of the transplanted gland. Isoproterenol treatment moderates structural injury and improves secretory function in the transplanted submandibular gland through up-regulating β1- and β2-adrenoceptor expression and post-receptor signal transduction.

Pendahuluan: Sialolithiasis adalah penyakit umum kelenjar saliva. Gejalanya termasuk pembengkakan kelenjar yang terlibat, terutama selama makan, dan nyeri tekan, yang mungkin mereda tetapi dapat kambuh kembali. Sialolith terjadi terutama di kelenjar submandibula (80-90%) dan pada tingkat yang lebih rendah di kelenjar parotid (5-20%). Sialolith bisa tunggal atau jamak. Multipel sialolith di kelenjar submandibula jarang terjadi. Tujuh puluh dari delapan puluh persen kasus memiliki sialolith tunggal, hanya sekitar 5% pasien yang memiliki tiga atau lebih sialolith. Faktor etiopatogenesis terkait dengan pembentukan sialolith adalah obstruksi, penurunan laju aliran saliva, dehidrasi, infeksi kelenjar saliva, dan terganggunya kelarutan kristaloid. Tujuan penulisan laporan kasus ini untuk menjelaskan etiopatogenesis dan terapi kasus multipel sialolithiasis kelenjar submandibula. Laporan kasus: Seorang wanita 24 tahun datang dengan pembengkakkan dan nyeri pada submandibula kanan. Radiografi panoramik menunjukkan massa radiopak terdefinisi dengan baik dalam submandibula kanan. Interpretasi ultrasonografi menunjukkan massa tak homogen hypoechoic dengan kalsifikasi ganda. Pengangkatan kelenjar submandibula dilakukan dengan pendekatan ekstraoral. Laporan kasus ini menunjukkan Gambaran sebanyak sembilan sialolith di kelenjar submandibula, yang dihilangkan dengan pendekatan ekstraoral. Simpulan: Etiopatogenesis dari pembentukan multipel sialolithiasis pada duktus kelenjar, yaitu faktor mekanis, inflamasi, kimiawi, dan infeksi. Diperkirakan bahwa alkalin serta saliva kental yang mengandung banyak sel mukus, memiliki persentase kalsium fosfat lebih tinggi seperti pada kelenjar saliva submandibula yang mendukung pembentukan sialolith. Pengangkatan kelenjar submandibula beserta sialolith dilakukan sebagai standar baku perawatan dan dapat menghindari kekambuhan. Pasien kontrol kembali satu minggu pasca operasi dengan kondisi baik dan dijadwalkan untuk pemeriksaan radiografis ulang enam bulan kemudian untuk memastikan tidak terjadinya pembentukan sialolith baru di saluran kelenjar saliva.Kata kunci: Multipel, sialolithiasis, kelenjar submandibula. ABSTRACTIntroduction: Sialolithiasis is a common disease of the salivary glands. Symptoms include the glands inflammation, especially during eating, and tenderness, which may subside but may recur. Sialoliths occur mainly in the submandibular glands (80-90%) and to a lesser extent in the parotid glands (5-20%). Sialolith can be singular or plural. Multiple sialoliths in the submandibular gland rarely occur. Seventy out of eighty per cent of cases have a single sialolith. Only about 5% of patients have three or more sialoliths. The etiopathogenetic factors associated with sialolith formation are obstruction, decreasing salivary flow rate, dehydration, salivary gland infection, and impaired crystalloid solubility. The purpose of this case report was to describe the etiopathogenesis and treatment of multiple cases of submandibular gland sialolithiasis. Case report: A 24-year-old woman presented with inflammation and pain in the right submandibular. Panoramic radiograph shows a well-defined radiopaque mass in the right submandibular. Ultrasound interpretation revealed a hypoechoic homogeneous mass with multiple calcifications. Removal of the submandibular gland was carried out with an extraoral approach. This case report showed the appearance of as many as nine sialoliths in the sub-mandibular gland, removed by an extraoral approach. Conclusion: Etiopathogenesis of the formation of multiple sialolithiasis in the glandular duct are mechanical, inflammatory, chemical, and infectious factors. It is thought that alkaline and thick saliva, which contains many mucus cells, has a higher percentage of calcium phosphate than in the submandibular salivary glands, which support the formation of sialoliths. Submandibular gland removal along with the sialoliths is performed as the treatment standard, which can avoid recurrence. The control visit is carried out one week postoperatively in good condition, and the patient is scheduled for another radiographic examination six months after to ensure that no new sialoliths occurred in the salivary gland.Keywords: Multiple, sialolithiasis, submandibular gland.

Gross cystic disease fluid protein-15 (GCDFP-15)/prolactin-inducible protein (PIP) is present at moderate levels in human submandibular and sublingual glands and is barely detectable in human parotid gland. The rodent homologue, PIP, has previously been identified in adult submandibular and lacrimal glands. Here we present the molecular characterization of rat PIP and show that this protein is a product of neonatal and adult rat submandibular, sublingual, and parotid glands. cDNA clones encoding rat PIP were isolated and sequenced. The deduced amino acid sequence of rat PIP shows 56% overall identity and 80% similarity with mouse PIP. By SDS-PAGE, secreted rat PIP has an apparent Mr of 17,000, with a minor proportion present as Mr 20–22,000 N-glycosylated forms. PIP was localized in rat salivary glands by immunogold silver staining. PIP was identified in acinar cells of developing and mature submandibular and parotid glands and at very low levels in sublingual gland serous demilunes. Typically, rat submandibular gland secretory proteins are produced by either acinar cell progenitors (Type III cells) or mature acinar cells. The expression pattern observed for PIP is similar to that previously reported for salivary peroxidase, an important component of nonimmune mucosal defense.

The Human Anatomy is an ancient Science, which uses the human body as the main material of study. However, structural differences between individuals that make up the species are quite common in the population and always emerge as a source of reports that aim to demonstrate and clarify these differences. These structural changes are called anatomical variations and may be presented externally or internally in any of the body systems, with no functional impairment to the individual. The salivary glands are exocrine glands that secrete saliva directly into the mouth through their ducts. This secretion has the functions of keeping mucous membranes moist, cleaning teeth, lubricating, dissolving and starting the food digestion process. Most of the saliva is secreted by the major salivary glands, the parotid glands and the submandibular glands, the latter being the focus of this case report. The purpose of this paper is to report a case of finding an accessory submandibular gland on a cadaver from the Human Anatomy Laboratory of the University Center of Rio Grande do Norte located in Natal, Rio Grande do Norte, Brazil. The discovery was made during a dissection of an adult male body in 2012, where an accessory submandibular gland was found in the right antimere. Thus, by identifying such variations, its clinical importance can be observed for the purpose of diagnostic imaging, surgery and anatomical teaching applied to clinic, given the scarcity of published reports, thus providing better understanding those working directly or indirectly on the subject.

Background: Enlargement in the extraoral region with the absence of abnormal dental and periodontal structures are sometimes seen in dental practice, sometimes followed by xerostomia. Enlargement of the acute nonsuppurative salivary glands has been associated with several types of viruses. The purpose of this paper is to review salivary gland diseases associated with non-HIV and HIV viral infections.Discussion: Non-HIV viruses which were detected in the salivary glands including Paramyxovirus, cytomegalovirus (CMV), Hepatitis C virus (HCV), human papilloma viruses (HPV), Epstein-Barr virus (EBV), human herpes simplex virus (HHSV-8), and coxsackie virus. HIV-associated salivary gland disease typically presents with xerostomia and/or intraglandular lymph nodes, and diffuse infiltrative lymphocytosis syndrome (DILS). The most common viral infection conditions in salivary gland disorders are mumps and HIV. Enlargement and inflammation of the glandular structures will affects the control of salivary secretion by nerves. Parasympathetic nerves block conducted signals to the salivary glands, so the salivary flow isdecreased.Conclusion: There is association between viral infection and diseases of thesalivary gland. By knowing sequelae viruses on the salivary gland, dentists are expected to understand the clinical condition and therapeutic that should be given to the patients.

The localization of dopamine stores and the expression and localization of dopamine (DAT) and vesicular monoamine transporters (VMAT) type-1 and -2 and of dopamine D1-like and D2-like receptor subtypes were investigated in rat submandibular, sublingual, and parotid salivary glands by HPLC with electrochemical detection, as well as immunochemical and immunohistochemical techniques. Male Wistar rats of 2 mo of age were used. The highest dopamine levels were measured in the parotid gland, followed by the submandibular and sublingual glands. Western blot analysis revealed DAT, VMAT-1, VMAT-2, and dopamine receptors immunoreactivity in membrane preparations obtained from the three glands investigated. Immunostaining for dopamine and transporters was developed within striated ducts. Salivary glands processed for dopamine receptors immunohistochemistry developed an immunoreaction primarily in striated and excretory ducts. In the submandibular gland, acinar cells displayed strong immunoreactivity for the D2 receptor, while cells of the convoluted granular tubules were negative for both D1-like and D2-like receptors. Parotid glands acinar cells displayed the highest immunoreactivity for both D1 and D2 receptors compared with other salivary glands. The above localization of dopamine and dopaminergic markers investigated did not correspond closely with neuron-specific enolase (NSE) localization. This indicates that at least in part, catecholamine stores and dopaminergic markers are independent from glandular innervation. These findings suggest that rat major salivary glands express a dopaminergic system probably involved in salivary secretion. The stronger immunoreactivity for dopamine transporters and receptors in striated duct cells suggests that the dopaminergic system could regulate not only quality, but also volume and ionic concentration of saliva.

Evidence is presented to show that the electrical activity of the salivary glands of Philine aperta is controlled by two largely independent systems which elicit different responses from salivary acinar cells. The excitatory junction potentials (EJPs) recorded from salivary cells result from the activity of a pair of identified buccal ganglion neurones. Each of these salivary effector neurones innervates only the ipsilateral gland. The effector neurones are driven to fire by synaptic input which is timed to occur during the retraction phase of the feeding cycle. Gland cell excitatory post-synaptic potentials (EPSPs) and action potentials appear to be mediated by a small group of peripheral neurones located at the base of each salivary gland. These cells give rise to a tract of fibres which cross to the contralateral gland and which may be responsible for communicating EPSP/action potential activity between the glands. The possible functions of the EJP and EPSP/spiking activities are discussed.

Background:Sjögren’s syndrome (SjS) is a chronic systemic autoimmune disease targets primarily the salivary and lacrimal glands, the severe dryness of the mouth and eyes are common manifestations in patients. Therefore, daily life could be affected by these manifestations in patients with SjS.Objectives:The aim of the study was to assess associations among daily activity impairment and scores of EULAR Sjögren’s Syndrome Patient Reported Index (ESSPRI) and major salivary gland ultrasonography (SGUS) in primary SjS.Methods:In this cross-sectional study, 41 patients with primary SjS (F/M:39/2; mean age: 52,1±10,5) were included. The mean disease duration was 9.5±6,6 years in the group.Data were collected by clinical examinations and a questionnaire regarding two patients reported outcome measures (PROMs). Firstly, Work Productivity and Activity Impairment (WPAI) questionnaire assessed paid and unpaid work during the last seven days. Scores of WPAI subgroups as absenteeism, presenteeism, overall work impairment as well as daily activity impairment were calculated by using 6 items. Secondly, dryness, fatigue and pain in ESSPRI scale were evaluated by visual analogue scale (VAS; 0-10 points) in SjS. High scores in both PROMs indicates that disease manifestations affect patient ‘life poorly.In addition, structural damage of parotid and submandibular salivary glands were examined by using Milic and Hocevar USG scoring methods. Unstimulated whole salivary flow rate (U-WSFR; as ml/min) were also used to interpret the functional status of major salivary glands. High SGUS score and low U-WSFR reflects that disease activity affects major glands poorly.Results:Daily activity impairment was calculated as 63,9±31,1 in patients with primary SjS. High scores in ESSPRI-dryness, ESSPRI-fatigue and ESSPRI-pain were also observed in the group (7,5±2,4; 6,4±2,8 and 6,1±3,1, respectively). Daily activity impairment was correlated with scores of ESSPRI-dryness (r:0,55 p=0.000), ESSPRI-fatigue (r:0,38 p=0.014) and ESSPRI-pain (r:0,56 p=0.000) as well as parenchymal inhomogeneity USG scores of right and left parotid glands (r:0,49 p=0.032; r:0,51 p=0.025).U-WSFR (0.20±0.20 ml/min) was moderately correlated with parenchymal inhomogeneity USG scores of major salivary glands (p<0.05). ESSPRI-dryness score was significantly higher in patients with low U-WSFRs (≤ 0.1 ml/min) than the others (87,5±16,3 vs 68,3±25,1, respectively)(p=0.021).Conclusion:Firstly, subgroup scores of ESSPRI and low U-WSFR associated to daily activity impairment in patients with primary SjS. Secondly, parenchymal inhomogeneity scores of both parotid glands could give an important clue to clinicians for the disease-related damage. Finally, WPAI with 6-item could be thought as an useful tool to understand the effect of the disease manifestations on patients’ daily life.Disclosure of Interests: :None declared

Objective.This study evaluated oxidative damage caused to the salivary glands in streptozotocin-induced diabetes (DM).Materials and Methods.Rats were divided into 4 groups: groups 1 and 2, control rats, and groups 3 and 4, DM rats. 8-Hydroxy-2′-deoxyguanosine (8-OHdG), protein carbonyl (PC), 4-hydroxynonenal protein adduct (4-HNE), oxidized and/or MDA-modified LDL-cholesterol (oxy-LDL/MDA), 8-isoprostanes (8-isoP), and oxidative stress index (OSI) were measured at 7 (groups 1 and 3) and 14 (groups 2 and 4) days of experiment.Results.The unstimulated salivary flow in DM rats was reduced in the 2nd week, while the stimulated flow was decreased throughout the duration of the experiment versus control. OSI was elevated in both diabetic glands in the 1st and 2nd week, whereas 8-isoP and 8-OHdG were higher only in the parotid gland in the second week. PC and 4-HNE were increased in the 1st and 2nd week, whereas oxy-LDL/MDA was increased in the 2nd week in the diabetic parotid glands.Conclusions.Diabetes induces oxidative damage of the salivary glands, which seems to be caused by processes taking place in the salivary glands, independently of general oxidative stress. The parotid glands are more vulnerable to oxidative damage in these conditions.
