Serum Phosphorus and Albumin in Patients Undergoing Peritoneal Dialysis: Interaction and Association With Mortality

Hyperphosphatemia and hypoalbuminemia confer worse clinical outcomes, whether these risk factors interact to predispose to mortality is unclear. In this prospective cohort study, 2,118 patients undergoing incident continuous ambulatory peritoneal dialysis (CAPD) were enrolled and categorized into four groups based on the changing point regarding mortality at 1.5 mmol/L for serum phosphorus and 35 g/L for serum albumin. Risks of all-cause and cardiovascular mortality were examined independently and interactively in overall and subgroups. There was no association between serum phosphorus with all-cause and cardiovascular mortality, but significant interactions (p = 0.02) between phosphorus and albumin existed in overall population. Patients in subgroup with high phosphorus and low albumin were at greater risk of all-cause (HR 1.95, 95%CI 1.27–2.98, p = 0.002) but not cardiovascular mortality (HR 0.37, 95%CI 0.10–1.33, p = 0.13), as compared to those with low phosphorus and high albumin. In contrast, patients with both low parameters had a higher risk of all-cause (HR 1.75, 95%CI 1.22–2.50, p = 0.002) and cardiovascular mortality (HR 1.92, 95%CI 1.07–3.45, p = 0.03). Notably, an elevated risk of both all-cause and cardiovascular mortality was observed in those with low serum albumin, irrespective of phosphorus levels, suggesting low albumin may be useful to identify a higher-risk subgroup of patients undergoing CAPD with different serum phosphorus levels.

MiR 208a Regulates Mitochondrial Biogenesis in Metabolically Challenged Cardiomyocytes

Metabolic syndrome increases the risk for cardiovascular disease including metabolic cardiomyopathy that may progress to heart failure. The decline in mitochondrial metabolism is considered a critical pathogenic mechanism that drives this progression. Considering its cardiac specificity, we hypothesized that miR 208a regulates the bioenergetic metabolism in human cardiomyocytes exposed to metabolic challenges. We screened in silico for potential miR 208a targets focusing on mitochondrial outcomes, and we found that mRNA species for mediator complex subunit 7, mitochondrial ribosomal protein 28, stanniocalcin 1, and Sortin nexin 10 are rescued by the CRISPR deletion of miR 208a in human SV40 cardiomyocytes exposed to metabolic challenges (high glucose and high albumin-bound palmitate). These mRNAs translate into proteins that are involved in nuclear transcription, mitochondrial translation, mitochondrial integrity, and protein trafficking. MiR 208a suppression prevented the decrease in myosin heavy chain α isoform induced by the metabolic stress suggesting protection against a decrease in cardiac contractility. MiR 208a deficiency opposed the decrease in the mitochondrial biogenesis signaling pathway, mtDNA, mitochondrial markers, and respiratory properties induced by metabolic challenges. The benefit of miR 208a suppression on mitochondrial function was canceled by the reinsertion of miR 208a. In summary, miR 208a regulates mitochondrial biogenesis and function in cardiomyocytes exposed to diabetic conditions. MiR 208a may be a therapeutic target to promote mitochondrial biogenesis in chronic diseases associated with mitochondrial defects.

Enhancement The Growth of Snakehead (Channa striata) with Addition of Dragon Fruit Peel Flour to The Diet

Snakehead (Channa striata) is a fish that grows relatively slowly, but is a typical Sumatran fish with high albumin content and economic value. This makes this fish a very prospect to be developed. Meanwhile, dragon fruit peel is one of the wastes that can be used for diet raw materials and contains vitamins and minerals that have the potential to be useful for increasing growth in fish. This study aims to determine the growth performance of snakehead fed by adding dragon fruit peel flour. This study used test animals, namely snakehead fish juvenile with the size of 1.06 ± 0.02 g/fish. The experiment of fish reared in aquarium measuring of 35cm x 45cm x 90 cm, filled with water as high as 15 cm without aeration. The experiment diets consisted of 5 types, namely feed added with 0% dragon fruit flour (control); 0.5; 1.0; 1.5 and 2.0% of feed. Each treatment was repeated three times. Snakehead was reared for 60 days and fed three times a day on an ad satiation basis. The parameters observed in this study were the survival rate and growth performance of snakehead fish. The results showed that the survival rate between treatments was not significantly different, namely in the range of 97.76 - 100%. Meanwhile, the specific growth rate and feed efficiency of snakehead which added 2% to the diets were the highest, namely 8.66 ± 0.04%/day and 67.83 ± 2.31%/day. The conclusion of this study was the addition of dragon fruit peel flour at 2% enhanced the growth performance of snakehead.

Kandungan Protein Roti Tawar Dengan Subtitusi Tepung Ikan Gabus (Channa Striata)

Background : Malnutrition problem is still the top priority in Indonesia and one of focuses in the Medium Term Development Plan (RPJMN) for 2020-2024. Riskesdas 2018, show the percentage of nutritional status of stunting under five age in Indonesia 2018 was 25.7%, in West Sulawesi Province was the second highest contributor for stunting children in Indonesia with prevalence 41.8%. One of the preventive efforts that can be done is through the development of food products. In this study, the development of white bread with the substitution of Channa Striata fish flour was expected to enrich the nutritional content of bread, especially protein. In this study, using Channa Striata fish because it has a higher protein content than other foodstuffs known as sources of protein such as eggs, chicken and beef. Apart from protein, Channa Striata fish also has a high albumin content which can be used in wound healing. This aims of this study to determine the protein content of white bread with the substitution of Channa Striata fish meal by 5% and 10% Methods : This type of research is an experimental study to determine the protein content in white bread with the substitution of Channa Striata fish meal by 5% and 10%. The analytical method used to assess protein content is the kjeidhal method, which is a simple method for determining total nitrogen in amino acids, proteins, and nitrogen-containing compounds. The test was carried out at Health Laboratory South Sulawesi. Results : The results showed that the protein content of white bread with a substitution of 5% was 9.03 grams, and a substitution of 10% was 10.38 grams. Conclusion : The conclusion of this study was even more substitution of Channa Striata fish flour used, will increase protein content in the white bread.

Serum albumin and beta-amyloid deposition in the human brain

ObjectivesTo investigate the relationships of serum albumin with in vivo Alzheimer disease (AD) pathologies, including cerebral β-amyloid (Aβ) protein deposition, neurodegeneration of AD-signature regions, and cerebral white matter hyperintensities (WMH), in the human brain.MethodsA total of 396 older adults without dementia underwent comprehensive clinical assessments, measurement of serum albumin level, and multimodal brain imaging, including [11C] Pittsburgh compound B-PET, 18F-fluorodeoxyglucose-PET, and MRI. Serum albumin was categorized as follows: <4.4 g/dL (low albumin), 4.4 to 4.5 g/dL (middle albumin), and >4.5 g/dL (high albumin; used as a reference category). Aβ positivity, AD-signature region cerebral glucose metabolism (AD-CM), AD-signature region cortical thickness (AD-CT), and WMH volume were used as outcome measures.ResultsSerum albumin level (as a continuous variable) was inversely associated with Aβ deposition and Aβ positivity. The low albumin group showed a significantly higher Aβ positivity rate compared to the high albumin group (odds ratio 3.40, 95% confidence interval 1.67–6.92, p = 0.001), while the middle albumin group showed no difference (odds ratio 1.74, 95% confidence interval 0.80–3.77, p = 0.162). Neither serum albumin level (as a continuous variable) nor albumin categories were related to AD-CM, AD-CT, or WMH volume.ConclusionsLow serum albumin may increase the risk of AD dementia by elevating amyloid accumulation. In terms of AD prevention, more attention needs to be paid to avoid a low serum albumin level, even within the clinical normal range, by clinicians.

OPTIMALISASI PEMBUATAN BROWNIES IKAN GABUS

Cork fish contains a lot of albumin compared to other freshwater fish, male cork fish albumin content of 6,7%, lower than female cork fish that have albumin 8,2%, are general brownies are made from flour, however, some have added fish to making brownies such as catfish and tilapia, but no one has added cork fish to make brownies, this study aims to determine to difference in sensory characteristics, the quality and content of albumin as well as the community's acceptance of brownie products. The design of this research is descriptive experimental design consisting of 2 factors: the proportion of the addition of cork fish consisting of 3 levels (60 grams, 80 grams, 100 grams), with the cork steaming process and the process without steaming cork fish. The results of this study are the average characteristics of brown, not fish-scented, not fishy and soft-textured. Brownie products that have significant differences both in terms of color, aroma, taste and texture are brownie samples with the addition of 100 grams of raw and 100 grams of steamed. The albumin content test for the addition of cork fish brownies which had the highest albumin content was found in the 100 gram raw cork fish addition brownie sample that was equal to 459.29% w / w. The results of the quality of brownie products are in accordance with SNI with the quality of water content <40% and ash content <3%. The acceptance of brownie products is that most people choose brownies with the addition of steamed cork 60 grams. The conclusion is cork fish brownies are acceptable to the public and contain high albumin.

Acute kidney injury after platinum-based transcatheter arterial chemoembolization and transarterial infusion chemotherapy in patients with hepatocellular carcinoma

Backgrounds The incidence of acute kidney injury and the association between acute kidney injury and prognosis have been reported about transcatheter arterial chemoembolization using anthracycline. However, the incidence of acute kidney injury after platinum-based transarterial chemoembolization or transarterial infusion chemotherapy remains unclear. The aim of this study was to investigate association between acute kidney injury after platinum-based transcatheter arterial chemoembolization/transarterial infusion chemotherapy and prognosis in patients with hepatocellular carcinoma. Methods We retrospectively analysed 270 sessions in 129 patients who underwent platinum-based transcatheter arterial chemoembolization/transarterial infusion chemotherapy. Acute kidney injury was diagnosed according to the criteria established by the International Club of Ascites. The incidence of acute kidney injury, risk factors for serum creatinine elevation and association between acute kidney injury and prognosis were assessed. Results Fifteen cases of acute kidney injury (5.6%, 15/270) developed in 14 patients (10.8%, 14/129). Ascites (coefficient: 0.059, P = 0.006), low estimated glomerular filtration rate (coefficient: −0.008, P = 0.029), diabetes (coefficient: 0.072, P &lt; 0.001) and high albumin–bilirubin grade (albumin–bilirubin grade 2: coefficient: 0.053, P = 0.004; and albumin–bilirubin grade 3: coefficient: 0.103, P &lt; 0.001) were significantly associated with an elevation in serum creatinine levels after transcatheter arterial chemoembolization/transarterial infusion chemotherapy. The development of acute kidney injury was associated with poor prognosis (hazard ratio: 3.18, 95%CI: 1.411–7.171, P = 0.005). Patients with acute kidney injury had a significantly lower survival rate than patients without acute kidney injury (log-rank test; P = 0.034). Conclusions The incidence of acute kidney injury after platinum-based transcatheter arterial chemoembolization/transarterial infusion chemotherapy was consistent with that after transcatheter arterial chemoembolization using anthracycline, and the development of acute kidney injury was associated with poor prognosis. Ascites, diabetes, low estimated glomerular filtration rate and high albumin–bilirubin grade were risk factors for serum creatinine elevation after platinum-based transcatheter arterial chemoembolization/transarterial infusion chemotherapy.

1723. Human Serum Albumin Regulates the Growth of Candida auris in vitro

Background Candida auris is commonly detected in human ear secretions. However, C. auris occasionally causes bloodstream infections even in immunocompetent patients resulting in poor prognosis. It was speculated that C. auris growth within the blood might be regulated by proteins in the bloodstream. Thus, in this study, the potential role of blood proteins in the regulation of C. auris growth was investigated. Methods Five Candida species (C. albicans, C. auris, C. glabrata, C. parapsilosis, and C. tropicalis) were incubated overnight. Colony suspensions for each species were prepared and adjusted to OD 1.0 at absorbance 0.1. Then, human serum albumin (HSA) and bovine serum albumin (BSA) were diluted (2.5 g/dL–0.002 g/dL) and mixed with the suspensions. Mixed samples were adjusted to 100 μL and incubated on MHA plates at 35°C for 2 days. Then, 50 μL of the combined sample was extracted and streaked onto Yeast extract-Peptone-Dextrose (YPD) agar. The remaining 50 μL sample was analyzed using an XTT assay. Further testing was then conducted on the effects of a specific blood protein albumin on Candida. Thereby, C. albicans and C. auris were cultured following the procedure above and stained with Annexin V and PI. Results The growth of C. auris mixed with a high albumin concentration (2.5~0.15 g/dL) was regulated compared with that of other Candida species (P < 0.01) (Figures 1 and 2); however, the growth of C. auris mixed with a lower albumin concentration was similar to that of other species. The wash-out study showed that C. auris growth and survival in the high albumin concentration was not different than that of other species. Conclusion HSA and BSA regulated C. auris growth which led to increased necrosis of C. auris. Conversely, growth of the other Candida species was not regulated. Therefore, albumin might be involved in the growth and necrosis of C. auris. As the highest concentration at which albumin regulated C. auris growth was similar to that found in human serum, it is possible that serum albumin might help prevent C. auris from entering the bloodstream via the ear or skin.
