A STUDY OF ETIOLOGICAL AND CLINICAL PROFILE OF PATIENTS WITH LMN CRANIAL NERVE PALSY IN A TERTIARY CARE CENTER AT THANJAVUR MEDICAL COLLEGE AND HOSPITAL

BackgroundNeurological presentation with isolated/multiple cranial nerve palsy is normal and its different causes incorporate infection, autoimmune, neoplastic, and inflammatory pathologies. The etiological range may rely on geological areas. We attempted this study to investigate the clinical range and etiological profile of numerous cranial nerve palsy.Materials and methodsThis planned observational study was led from January 2020 to January 2021. All the patients with cranial nerve palsy coming to the neurology OPD were taken for examinations. Essential targets were to characterize anatomical disorder/cranial nerve combinations and to set up etiology. The primary goals were to examine related components. Patients with neuromuscular junction disorder, anterior horn cell disease, myopathies, first and second cranial nerve dysfunction were excluded from the study. All patients went through an organized convention of clinical assessment, examinations, and few particular examinations as per clinical protocol for analysis.ResultsCavernous sinus was the commonest anatomical condition of different cranial nerve paralyses and tuberculous disease was the commonest cause in this investigation.7th cranial nerve was the common isolated nerve involved with idiopathic ethology, diabetes was the most common cause overall found with third nerve involvement.

Skull Base Metastasis of Breast Cancer With Oculomotor and Trochlear Nerve Palsy

Skull base metastatic tumors are rare. Breast cancer in particular can cause bone metastases after a long period of time. A 70-year-old woman presented with multiple cranial nerve palsy. Magnetic resonance imaging revealed a lesion that extended from the orbit to the base of the skull, and the patient was referred to our department. Ophthalmological evaluation showed left visual acuity impairment, left oculomotor nerve palsy, and left trochlear nerve palsy. Endoscopic biopsy performed 5 years after the completion of breast cancer treatment revealed skull base metastases. In unilateral multiple cranial nerve palsy, the possibility of skull base metastases should be considered.

Chronic Inflammatory Demyelinating Polyradiculopathy with Multiple Cranial Nerve Palsy: An Atypical Pediatric Case

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is relatively rare in children. Moreover, atypical presentation remains a diagnostic challenge for physician. Early recognition and proper immunomodulatory therapy is important for favorable outcomes with prevention of progressive course. We report a case with atypical CIDP. A rare case of tuberculoma masquerading as CP Angle neoplasm

Tuberculoma involving the cerebellopontine angle is very rare. Preoperative neuroradiological features of such lesions may mimic neoplastic lesions. Our case presented with cerebellar features and multiple cranial nerve palsy. Neuroimaging mimicked CP angle neoplastic lesion. Antitubercular therapy and steroids resulted in significant clinical improvement and marked radiological reduction in size of the lesion. In our subcontinent a treatable infective cause like tuberculosis should be ruled out in CP angle lesions. Although rare but definitely a possibility to be considered.
