Male partners/fathers are key support persons for many childbearing women and their involvement in pregnancy, childbirth and the postpartum/postnatal period has beneficial effects on a wide range of outcomes related to maternal and child health and family wellbeing. Social support is implicated in the relevant causal pathway, but has received largely tangential attention in the public health literature. This discussion paper aims to reframe men's participation in maternity care as an opportunity to enhance their readiness and ability to provide social support to women, contributing to the debate on the definition and rationale for male partner involvement, and paving the way for further empirical work. I begin by presenting a theory of change illustrating the causal pathway leading from male partner participation, through the key intermediate step of social support, to improved health and wellbeing for women and children. I proceed by arguing that many people desire male partner participation in maternity care; however, in practice, this is often limited owing to cultural, social and institutional barriers. I use examples from the intervention literature to demonstrate how participation in care can boost men's motivation to support women and enhance their ability to do so by increasing their knowledge and skills. Finally, I draw up general implications for further male partner involvement programmes, suggesting that in order to achieve meaningful and sustainable gains, attention to design is crucial in order to avoid reinforcing patriarchal gender norms. Programmes should be implemented alongside other efforts to improve quality and promote woman-centred care. This article is part of the theme issue ‘Multidisciplinary perspectives on social support and maternal–child health’.

Abstract Background Having a partner is associated with better prognosis in patients with cardiovascular disease. However, the influence of partners on modification of patients' lifestyle-related risk factors (LRFs) is unclear. Therefore, we studied the influence of partners and the level of partner participation on LRF modification in patients after an acute coronary syndrome (ACS) or revascularization. Methods The RESPONSE-2 trial was a community-based lifestyle intervention trial comparing nurse-coordinated referral to a comprehensive set of lifestyle interventions (physical activity, weight reduction and/or smoking cessation) to usual care. In the current analysis, we investigated the association of having a partner on the improvement in >1 LRF without deterioration in other LRFs at 12 months follow-up. Secondary, the influence of the level of partner participation (participating partner, non-participating partner and no partner) in the intervention group was studied. Results In total 824 individuals (411 intervention, 413 control) were included in the analysis. The presence of a partner was comparable in both groups (intervention 74%, control 69%). In the intervention group, 48% of partners participated in the lifestyle interventions. Overall, having a partner was positively associated with improvement in LRFs (odds ratio (OR) 2.57 (95% confidence interval (CI) 1.57–4.21), p<0.001). In the intervention group, having a participating partner was also positively associated with improvement in LRFs compared to patients without a partner (OR 2.45 95% CI 1.25–4.79, p=0.009). When analysed the influence of partners on LRFs separately, patients with a participating partner were most successful in reducing weight compared to patients without a partner (OR 2.71 95% CI 1.16–6.36, p=0.02). Conclusion Having a partner is associated with improvement on LRFs in patients after ACS or revascularization. Moreover, patients with participating partners in the lifestyle programs were most successful in LRF modification. Involvement of partners in lifestyle interventions should be considered in routine practice. Funding Acknowledgement Type of funding source: Public grant(s) – National budget only. Background: Physical activity is of particular interest due to its potential for improving quality of life and reducing health care costs. The contribution of this paper is to analyse the correlates of physical activity (PA) among individuals aged 50-70 years old. We differentiate between physical activity during leisure time (LTPA) and total physical activity (Total PA) and besides we offer potential policy advice to increase PA. Methods: We use a cross-sectional survey from a sample of Spanish individuals between 50 and 70 years of age. We analyse the correlates of LTPA and Total PA by estimating ordered probit models including socio-demographic characteristics, health and emotional wellbeing and social support. Results: The covariates explain in different ways LTPA and Total PA levels. In particular, the accomplishment of a minimum of LTPA is positively related to partner participation in LTPA (p<0.01), a good life satisfaction (p<0.01), being male (p<0.01) and secondary education completion (p<0.02); and it is negatively related to working status (p<0.01). On the contrary, very high levels of total PA are positively associated with partner participation (p<0.01), self-perceived health (p<0.01), women (p<0.01) and working status (p<0.05).  Conclusions: It is important to increase male energy expenditure in other life domains besides leisure time. Regarding women, social support seems necessary to reach minimum levels of LTPA. Finally, the socialisation effect that we have obtained as well as the positive impact of the extrinsic social support variables might suggest the necessity to take into account couple and family variables.

Multi-actor studies are particularly suited for partner and family research, as they capture relationships beyond the conventional restraint of the household. Previous research on partner participation in the German Family Panel indicates higher participation of cohabiting and married partners compared to those living apart together. The present study evaluates whether this finding is due to unobserved relationship quality aspects associated with relationship status, differentially affecting the likelihood of partner response, or rather to field procedures favoring the participation of coresidential partnerships. Fixed effects models find a positive effect of moving in together on partner response, indicating that part of the relationship status effect found in previous research can, in fact, be attributed to coresidence. Analyzing the response process in detail reveals that the effect of moving in together goes back to main respondents’ consent to the partner survey, whereas no significant effect can be found on partner participation.
