Numerical Treatment for the Second Law Analysis in Hydromagnetic Peristaltic Nanomaterial Rheology: Endoscopy Applications

In current study, analysis is presented for peristaltic motion of applied magnetic field and entropy generation within couple stress (Cu/H2O) nanofluid through an endoscope. An endoscope contains two coaxial cylindrical tubes in which the internal tube is nonflexible while the external tube has sinusoidal wave passing through the boundary. Influences of mixed convection along with applied magnetic field are encountered as well. Formulated governing model is fabricated introducing long wavelength and creeping Stokesian flow approximation which are then analyzed numerically by utilizing Adams Bashforth method. For a physical insight, results are demonstrated to examine the behaviors of flow profiles and entropy generation number for emerging flow parameters with the help of graphs, bar-charts and tables.

Coenzyme Q10 Efficacy Test for Human Skin Equivalents Using a Pumpless Skin-On-A-Chip System

A human skin equivalent (HSE) composed of the epidermis and dermis is cultured using a pumpless skin-on-a-chip system to supply cultures the desired flow rate using gravity flow without a pump or an external tube connection. Coenzyme Q10 efficacy is tested by adjusting its concentration, as it is known to have anti-aging and antioxidant effects in culture solutions. The relationship between the contraction rate of a full-thickness human skin equivalent and secreted transforming growth factor (TGF) β-1 is analyzed via enzyme-linked immunosorbent assay (ELISA). Following hematoxylin and eosin (H&E) staining, an image of the skin equivalent is analyzed to measure the epidermal layer’s thickness. The cell density and differentiation of the dermis layer are investigated. Gene and protein expression in the dermal and epidermal layers are quantitatively analyzed using quantitative real time polymerase chain reaction (qPCR) and immunohistochemical staining. As the coenzyme Q10 treatment concentration increased, the number of cells per unit area and the thickness of the epidermal layer increased, the expression level of filaggrin increased, and the contraction rate of full-thickness HSE was proportional to the amount of TGF β-1 secreted.

Segment III bypass: An Option to Palliate Proximal Biliary Obstruction

Introduction: In the era of minimally invasive therapy, endoscopic or interventional radiological techniques have become preferred technique for palliating advanced malignant hilar obstruction due to carcinoma gallbladder or cholangiocarcionmas. However, in selected group of these locally advanced unresectable disease and fit patients, surgical segment III bypass could still be a viable option for palliation. Methods: I retrospectively reviewed the medical records of all the patients of segment III bypass performed over last six years (Oct 2012- May 2018) and analyzed the indication, perioperative morbidity, symptom relief and survival. Results: Total 17 patients underwent palliative biliary bypass over the specified period. Among them 8 patients underwent segment three III biliary bypass for proximal biliary obstruction. Except 1 patient, all segment three bypass were offered to malignant proximal biliary obstruction. One patient developed postoperative sepsis and had mortality on day 7. Symptom relief in the form of relief of pruritus, improved sleep pattern and improved appetite was seen in 5 patients. In remaining 2 patients, the bilirubin didn’t drop to normal level and the symptom reliefs were partial. However, the patient had good satisfaction over not having an external tube attached to their body. Despite the patent anastomosis, failure in dropping bilirubin level significantly was considered due to segmental obstructions. One patient survived for 18months, 1 had follow up to 13 months. Three patients were lost to follow up after 3 months. Two patients had six weeks follow up and were symptomatically doing better. Conclusion: Surgical segment III bypass is still a good option for palliating malignant hilar obstruction in selected group of patients.

Baerveldt tube shunt implantation through a long scleral tunnel

Purpose: To develop a new technique for the placement of a Baerveldt glaucoma implant tube through a long scleral tunnel. Materials and methods: Patients with refractory glaucoma undergoing the maximum tolerated medical therapy were recruited. We created a scleral tunnel with a 24-gauge catheter needle and used the external tube for guidance when introducing a Baerveldt glaucoma implant tube. Main outcome measures included intraocular pressure, supplemental medical therapy score, intraoperative complications, and postoperative complications. Results: Nine eyes of six patients were included in the study. The mean preoperative intraocular pressure (score) was 41.0 mmHg (4.78). Mean postoperative intraocular pressures were 18.3 mmHg (0.22), 18.8 mmHg (0.13), and 16.9 mmHg (0.50) at postoperative 1, 3, and 6 months, respectively. Minor vitreous hemorrhages were observed in three cases. Postoperative hypotony (<5 mmHg) was observed only in the first case at postoperative day 3. In one case, a stent suture could not be placed because the patient was restive intraoperatively, which resulted in an expulsive hemorrhage at 3 months postoperatively. The tube could have penetrated into the anterior chamber in another case. Tube exposure and corneal erosions were not observed in all cases. Conclusion: We developed a new technique to place a Baerveldt glaucoma implant tube through a long scleral tunnel. The outcomes were comparable to other reports of Baerveldt glaucoma implant surgery, although the number of cases was limited in this study. A long scleral tunnel can substitute for a preserved scleral patch and self-scleral flap to avoid tube-related complications.

Numerical Studies on Convergent Tube-in-Tube Heat Exchanger

HHeat exchangers have been used since the start of industrialisation, and since then have been modified in a plethora of complex and intricate ways to suit the needs of the corresponding application, but in this paper a simple modification on the existing design has been investigated numerically, which has proven to be effective. A standard tube-in-tube heat exchanger has been modified such that the external tube tapers (converges) to a smaller diameter. The inner tube carries the cold working fluid which absorbs the heat energy carried by the fluid in the outer tube. A numerical simulation has been carried out, taking into account the mass flow rate, fluid temperatures, heat transfer etc., which are governed by fundamental heat transfer equations. The results of the simulation are used to analyse the effect of this modification (convergent tube) on the efficiency of the heat exchanger, which has been found to increase significantly along with the rate of heat transfer.

Gas-Liquid Two-Phase Flow Detection Techniques Based on Internal and External Tube Differential Pressure Flowmeter

The gas-liquid two-phase flow exists widely in nature and in our daily life, to realize the phase flow does not separate online measurement has become an important subject in the study. Through CFD simulation experiment, the optimal structure of inner and outer tube differential pressure flowmeter prototype. Through the analysis of the experimental data, comparison of the classical theoretical models found high Chishlom prediction model error is minimum. Moisture the modified model, the relative error of measurement is better than in the range of experiment 5%.
