Therapeutic communication skills in palliative care nursing education: an exploratory study using a role-playing tool

This study aim to explore the experiences of nursing final-year students on the care of the person in a palliative situation and their family using the role-playing technique. Qualitative, descriptive research developed in October 2020 with 30 third-year nursing students from a palliative care nursing course, in Portugal. Individual debriefing reflections and thematic analysis of the information were conducted. Two main themes emerged: 1) facilitators; and 2) barriers of role-playing in a simulated palliative care context. We concluded that type of simulation allows students to deal with complex palliative care situations from an individual and realistic perspective.

Current and Future Systemic Therapies in Biliary Tract Cancer

<b><i>Background:</i></b> Despite an increasing incidence, biliary tumors are still considered a rare tumor entity. Due to an often long clinically inapparent course and a lack of early detection strategies, surgical resection is often not possible at the time of diagnosis. Since 2010, chemotherapy with gemcitabine and cisplatin is considered the standard of care in the palliative situation. Only recently, first studies have been published or initiated that expand the treatment options in the first line and, for the first time, also suggest valid systemic approaches in the second line. <b><i>Summary:</i></b> Molecularly targeted therapies in selected patient subgroups are rapidly changing the field. In addition to IDH1 mutations and FGFR2 fusions in patients with intrahepatic tumors, the therapeutic relevance of rare but targetable alterations such as HER2/neu amplification, NTRK fusions, or BRAF mutations should be considered in patients with biliary tract cancers. <b><i>Key Message:</i></b> The current study landscape clearly shows that precision medicine will play an important role in the therapy of biliary malignancies and underlines the importance of early tumor genetic diagnostics. In this article we provide an overview of systemic therapy concepts in the adjuvant and palliative setting.

Spirituelle Bedürfnisse schwer kranker Menschen mit Fluchterfahrung. Herausforderungen für die Palliativversorgung

ZusammenfassungFlüchtlinge und Menschen mit Migrationshintergrund, die im deutschen Gesundheitssystem palliativ versorgt werden, befürchten Ausgrenzung. Ergebnisse aus einem qualitativen Forschungsprojekt zeigen, dass diese Patientinnen und Patienten ihre Flucht und die palliative Situation als doppelte Vulnerabilität erleben. Sie fragen sich in diesem Zusammenhang, was am Lebensende wichtig ist. Seltener werden ‚klassisch‘ spirituelle Antworten etwa aus der jeweiligen Religion, der man angehört, gegeben, sondern zumeist stehen die professionelle Unterstützung bei der Erfüllung von Grundbedürfnissen und die Wahrnehmung der Ängste etwa durch die behandelnden Ärzte für die Patientinnen und Patienten als hilfreiche Ressourcen im Vordergrund. Die Palliativversorger betonen die Probleme, die bei der Unterstützung auftreten, wenn das palliative Angebot von Patientinnen und Patienten nicht wahrgenommen wird. Zu den Ressourcen, die für die Patientinnen und Patienten in der Verarbeitung der Situation wichtig sind, gehört ein Vertrauen in die Palliativversorgung. In diesem Zusammenhang sollte von Seiten des medizinischen und pflegerischen Personals die Akzeptanz der Diversität der Patienten-Biografien wichtig sein. Spiritualität, die zu wenig aus der Perspektive der Patientinnen und Patienten betrachtet wird, kann auch zu einer Barriere im Austausch von Patient/-in und Palliativversorgern werden.

3D Conformal Total Liver Radiotherapy for Locally Advanced Hepatocarcinoma: A Case Report

Hepatocarcinoma is the third localization of cancer in Senegal. More than 90 per cent of these cases in Senegal are painfull locally advanced stages. Radiotherapy is helpful in palliative situation. Newer radiation techniques, such as stereotactic body radiation therapy (SBRT) help reducing the radiation to nearby healthy tissues. This technique is not available in our institution. We report a case of 3D conformal total liver radiotherapy for locally advanced hepatocarcinoma.

Updated S3 Guidelines – Diagnosis and Treatment of Colorectal Carcinoma: Relevance for Radiological Diagnosis and Intervention

The updated German S3 guidelines “Colorectal Carcinoma“ were created as part of the oncology program of the Association of the Scientific Medical Societies (AWMF), German Cancer Society and the German Cancer Aid under the auspices of the German Society for Digestive and Metabolic Disorders (DGVS) and they replace the previous guidelines from 2013. The main changes in the updated guidelines include the latest recommendations regarding endoscopy and adjuvant/neoadjuvant therapies as well as a complete restructuring of the section regarding therapeutic approach in metastases and in the palliative situation. The present manuscript discusses the importance of the current recommendations for radiological diagnosis and treatment and is intended to enhance the quality of patient information and patient care by widespread distribution. „Das heißt, ich muss sterben?!” Training kommunikativer Fertig keiten in der Onkologie und Palliativmedizin

ZusammenfassungDas Überbringen schlechter Nachrichten wird von Ärzten oft als Herausforderung empfunden. Hinweise zum kompetenten Umgang mit solchen Gesprächssituationen sind daher zunehmend in die ärztliche Aus- und Weiterbildung implementiert worden, die entsprechenden Fertigkeiten nehmen im Nationalen Kompetenzbasierten Lernzielkatalog Medizin (NKLM) einen wichtigen Stellenwert ein. Das in diesem Beitrag vorgestellte Praxisbeispiel ermöglicht es Medizinstudierenden, das überbringen schlechter Nachrichten am übergang zu einem palliativen Behandlungsansatz im Rollenspiel mit einer Simulationspatientin zu üben. Aufgabe der Studierenden in der Arztrolle ist es, den Befund (metastasierter Brustkrebs, palliative Situation) und - sofern möglich - die Behandlungsoptionen zu besprechen. Am Universitätsklinikum Würzburg wird das Szenario im Querschnittsbereich „Interdisziplinäre Onkologie” im 7. Semester verwendet. Die besonderen Herausforderungen dieser Fallvignette sind die emotionalen Reaktionen der Patientin und ihre Situation als junge Mutter, die das Aufwachsen ihres Kindes mit hoher Wahrscheinlichkeit nicht mehr erleben wird. Die Evaluationsergebnisse der Veranstaltung sprechen für einen hohen Realitätsgehalt der Rollenspiele und eine breite Akzeptanz bei den Studierenden.
