PATHOMORPHOLOGICAL CHANGES AND BACTERIAL PATHOGENS ASSOCIATED WITH SWINE PNEUMONIAS IN SOUTHWEST NIGERIA

Pneumonia is a major economic threat to swine industry worldwide, however, there is still dearth of information on the pathology and associated pathogens in Nigeria, and these were therefore investigated. Lungs from 408 slaughtered pigs were randomly collected from abattoirs in Abeokuta, Ibadan and Lagos. The lung samples were cultured for bacterial pathogens using standard techniques, while formalin-fixed tissues were processed for histopathological examination. Grossly, the most consistent lesion was bronchopneumonia (35.3%). The main histopathological findings were lymphoid hyperplasia of bronchus-associated lymphoid tissue (BALT) (88.2%), suppurative bronchopneumonia (63.6%), suppurative bronchitis and bronchiolitis with concurrent epithelial hyperplasia (57.1%), as well as thickened alveolar septa due to cellular infiltration consisting predominantly of neutrophils (54.1%). Ten different species of bacteria were isolated from the lung samples in which two or more pathogens were isolated from each sample (82.7%). Pasteurella multocida was the most frequently isolated bacterium (54.8%). Among the bacteria isolated, there were significant (P < 0.05) differences in the frequencies of isolation of β-haemolytic Streptococci, P. multocida, Haemophilus species and Escherichia coli between the pneumonic lungs and apparently normal lungs. The results of bacterial culture, gross and histopathological changes recorded in this study are consistent with bacterial pneumonia possibly caused by most of the bacteria identified in the present study.

Atypical presentation of right-sided native valve infective endocarditis

A previously healthy 55-year-old man presented to hospital with 10 days of progressive dyspnea with fever, night sweats, and a productive cough and no history of recreational drug use or occupational or animal exposures. His wife had developed similar symptoms 2 weeks earlier but had since recovered. Physical exam revealed a new systolic murmur best heard at the left lower sternal border. Transesophageal echocardiogram demonstrated severe tricuspid regurgitation with a small vegetation. Blood cultures were positive for non-typeable Haemophilus influenzae. This case illustrates the necessity of both timely and proficient diagnosis of H. influenzae infection and the unique challenges associated with detecting H. influenzae–related pathology. Clinicians should be aware of the variable presentations of Haemophilus infection, including respiratory infection, neurological infection, and infective endocarditis. Given the fastidious nature of H. influenzae and variability between subtype pathogenicity, microbiology laboratories require tools to culture and differentiate Haemophilus species.

Microbiology, diseases, and epidemiology of Kingella kingae in young children: A short review

Kingella kingae is a Gram-negative coccobacilli and it is a member of the HACEK (Haemophilus species, Aggregatibacte actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens, and K. kingae). HACEK organisms are typically oropharyngeal commensals and have long been recognized as a cause of infective endocarditis in children and adults. K. kingae in difficult to be recovered from cultured pharyngeal samples due to its slow growth and the high presence of resident bacterial flora, however, the organism can be better detected using PCR tests. Based on our search in PubMed and other sources, we couldn't discover any study about K. kingae originated from any Arab country. Therefore, we wrote this review to draw the attention of our physicians and clinical microbiologists on the importance of this neglected group of organisms in clinical medicine. However, this review article aims to cover the most important diseases of K. kingae in the pediatric population.

Recent Trends in Bacteriology of Adult Patients with Chronic Rhinosinusitis

This study aimed to identify trends in bacteria isolated from Korean adults with chronic rhinosinusitis (CRS). Enrolled were CRS patients who underwent sinus bacterial culture during endoscopic sinus surgery between 2007–2008, 2011–2012, and 2017–2018 (n = 510). Patients’ clinical characteristics, bacterial culture results, and antibiotic resistance were reviewed. The bacteria isolation rate was 76.3% (73.9% for CRS with nasal polyps and 82.8% for CRS without nasal polyps; p = 0.038). In total, 650 strains were isolated, the most common was Coagulase Negative Staphylococci (CNS) (28.0%), followed by Streptococcus species (12.2%), Propionibacterium species (8.0%), Corynebacterium species (7.5%), Staphylococcus aureus (6.2%), Haemophilus species (5.7%), Klebsiella species (5.1%), and Pseudomonas aeruginosa (4.2%). Furthermore, an analysis of the bacterial trends in the three groups showed significant increases over time for the isolation of CNS (p = 0.006), Klebsiella (p = 0.002), and P. aeruginosa (p = 0.007) and extended-spectrum beta-lactamase (ESBL) producing Klebsiella (p < 0.001) and Enterobacter (p = 0.007) species in terms of antibiotics resistance. This study demonstrates that the frequency of CNS, Klebsiella, and P. aeruginosa in CRS patients and the ESBL-producing Klebsiella and Enterobacter species has significantly increased in CRS patients over the last decade.
