Background Tuberculosis (TB) is a major health problem worldwide. Even in highly prevalent countries, primary gastroduodenal tuberculosis is a rare manifestation of extrapulmonary tuberculosis. In recent years, as the incidence of tuberculosis has increased year by year, the occur of gastroduodenal tuberculosis has also increased. Endoscopy is an important tool for diagnosing gastroduodenal tuberculosis. The performance of gastroduodenal tuberculosis under endoscopy is often non-specific, which may imitate other benign or malignant gastroduodenal diseases. Diagnosis of gastroduodenal tuberculosis relies on a combination of endoscopy and guided biopsy. Case presentation Here, we report a rare and interesting case of gastroduodenal tuberculosis with acute pancreatitis. The case initially mimicked gastroduodenal ulcers in morphology and appeared in a middle-aged person with normal immunity but with prolonged fever and abdominal pain. The disease was diagnosed through endoscopy and guided biopsy, and it responded well to antituberculosis drugs. Conclusions Clinicians must remember that even in the absence of immunodeficiency, as in this case, tuberculosis can affect any part of the gastrointestinal tract.

Non-clostridial gas gangrene of a diabetic foot with COVID-19 infection

Gas gangrene is a manifestation of soft tissue infection by clostridial group of bacteria. Non-clostridial gas gangrene is uncommon and seen rarely in diabetics. In the era of COVID-19 pandemic where COVID-19 itself induces hyperglycemia non clostridial gas gangrene may arise due to altered immunity. We present a case of non clostridial gas gangrene of lower limb in a middle aged person who was not a known diabetic. A brief case report with review of literature is presented.

Consumer Behavior towards Regional Eco-Labels in Slovakia

Slovakia, as part of the European Union, participates in all forms of regional development and sustainability. A transparent form of regional development and sustainability is regional labeling, which has a 10-year tradition. Our regions represent excellent potential for the development of domestic products. Acceptance of essential requirements for regional products (domestic raw materials, manual work share, respect for the environment) opens up opportunities for the labor market, promotes tourism, and increases citizens’ purchase ability, none of which are clearly demonstrable in Slovak regions. Residents of individual regions have specific approaches in relation to regional brands depending on the region studied. For the purpose of the survey, four regions of southern Slovakia (with common environmental morphology and different industrial development) were selected. Indicators of age, gender, education, monthly income, and location relative to regional product preferences were studied. The results obtained were processed by cluster analysis (as a way of segmenting consumers). An average conscious purchaser of regional products is a local productive middle-aged person with a secondary or tertiary education, either male or female, and from a more economically advanced region. The results show regional branding as a mobilizing function for connecting inhabitants and the subsequent joint presentation of regional activities.

On Metalepsis

Even before I heard about this celebration of Elaine Freedgood's work and mentorship, I was already thinking about how I'd come to know her and about the numerous ways she has shaped my writing and my writing life. This reflection was prompted, inadvertently, by Elaine herself. A little while back, Elaine wrote me an email with the always-thoughtful subject line, “How are you, my friend?” Except, Elaine had forgotten the comma after “you,” so it read instead, “How are you my friend?” This message made me laugh, as did her subsequent response about leaving out that crucial comma. Elaine came to be an unusually generous mentor through the most usual of circumstances. I was first put in touch with her when she came to Berkeley to give a talk called “The Secret Life of Diegesis,” five years ago. I have a vivid memory of the emails we'd exchanged about booking flights and reserving hotel rooms and all that—usually these would be somewhat tedious issues to sort through, but Elaine's typically dry humor enlivened even the driest of topics. When asked if she wanted to “be a trooper” and take a red-eye over to the West Coast, she responded, “Thanks so much. I would be a trooper if necessary but of course would rather be a pathetic middle-aged person (i.e., myself).”

Double Aortic Arch – A Case Study

Double aortic arch (DAA) is one of the 2 most common forms of vascular ring, a class of congenital anomalies of the aortic arch system in which the trachea and esophagus are completely encircled by connected segments of the aortic arch and its branches. The aim of this study is to describe a case of DAA in a middle aged person. A 40 year old male came to outdoor patient department with cough and breathlessness since childhood which was diagnosed earlier as bronchial asthma, cold allergy, and dust allergy. The final diagnosis DAA was made after CT angiogram. THE PROFILE OF PATIENTS WITH SCHIZOPHRENIA WHO ATTEMPTED SUICIDE

Introduction: Suicide is a complex phenomenon, and deliberate act of taking one’s own life, associated with numerous and various biological, social and psychological factors. It is an important psychopathological entity and one of the most demanding clinical problems, which psychiatrists face in their everyday practice, especial in schizophrenic patients. According to literature data, the number of suicides of schizophrenic patients has been increasing in recent decadesObjectives: The aim of this study was to determinate the profile of patients with schizophrenia who attempted suicide in the Psychiatric hospital “Skopje”-Skopje, R.Macedonia.Methods: The study included 114 patients with schizophrenia, 47.4% (n=54) of them attempted suicide, 52.6 %( 60) was group consists of patients with schizophrenia who is not registered for suicide attempt, nor the existence of suicide idea. Psychiatric diagnosis of schizophrenia was based on ICD-10 criteria.Results: Based on the investigation of patients with schizophrenia who attempted suicide, we obtained a typical profile as follows: middle-aged person(31.4 average years), single (61,1% were not married, 44.4% live alone), (79.5%) unemployed, repeatedly hospitalized and history of suicide attempts, attempting suicide using a violent method (poisoning -55.5%, drowing-7.4%, hanging-9.2%, slashing their wrists-16.6%, jumped from height-5.5% etc.) after more than tree weeks of hospital treatment and out of hospital.Â
