The effects of electronic navigation devices on marine accident occurrences

In recent years, maritime-related organizations and companies have moved to a risk-based approach. To determine the risks, it is necessary to understand comprehensively why accidents occur and how it develops. The most effective measures need to be identified to implement the accident prevention measures successfully. According to the results of scientific studies conducted in the past, 80% of human factors risks were effective in marine accidents. Nowadays, maritime technologies are the most effective method for reducing the risks of human factors. However, the use of electronic navigation devices has not eliminated accidents. In this study, the accident reports for collision and grounding due to the electronic navigation devices' risk was evaluated using Human Factors Analysis and Classification System (HFACS) method. As a result of the study, more than half of the visible (active) causes of accidents have been identified as operating failure factors in electronic navigation equipment. Recommendations to prevent the occurrence of accident factors have been made.

Implementing A Human Factors Approach To Rca2: Analysis Of Resultant Safety Recommendations

Root Cause Analysis and Action (RCA2) guidelines offer fundamental improvements to traditional RCA. Yet, these guidelines lack robust methods to support a human factors analysis of patient harm events and the development of systems-level interventions. We previously described how human factors tools can be integrated into RCA2 to create a robust process called HFACS-RCA2. Prior analyses of qualitative data associated with an 18-month implementation project at a large academic health center indicated that HFACS-RCA2 fosters a more comprehensive human factors analysis of serious patient harm events and the identification of broader system interventions. The present study builds on this prior research by presenting the analysis of actual recommendations extracted from RCA reports. Results corroborate qualitative stakeholder findings that HFACS-RCA2 produced recommendations that were stronger and included more substantive changes compared to former RCA methods.

Human Factors Analysis of a Waiver: A Case Study

Case law has questioned the enforceability of waivers due to deficiencies in two categories: whether the defendant took reasonable steps to bring notice of the waiver and its contents to the attention of the plaintiff, and whether the waiver contained key design flaws. This report presents a case study on an accident at a recreational sports facility. An analysis was conducted on two versions of the facility’s waiver from a human factors perspective. The analysis reveals the application of forensic human factors concepts to waiver design and the influence that improperly designed waivers can have on sports and recreational injury lawsuits.
