Analysis of Consumer Exposure Cases for Alcohol-Based Disinfectant and Hand Sanitizer Use against Coronavirus Disease 2019 (COVID-19)

The development and distribution of vaccines and treatments as well as the use of disinfectants and hand sanitizers to cope with coronavirus disease 2019 (COVID-19) infection has increased rapidly. As the use of disinfectants and hand sanitizers increased, the number of unintended exposures to these substances also increased. A total of 8016 cases of toxic exposure to disinfectants and hand sanitizers were reported to the American Association of Poison Control Centers (AAPCC) from 1 January 2017 to 30 May 2021. The cases have been characterized by substance, sex, patient age, exposure reason and site, treatments received, and outcomes. The number of exposures correlates closely to the rise of COVID-19 cases, rising significantly in March 2020. About half of the total cases involved children less than 10 years old and 97% of those exposures per year were unintentional. In addition, the most common exposure site was the patient’s own residence. Over-exposure to disinfectants and hand sanitizers can cause symptoms such as burning and irritation of the eyes, nose, and throat, coughing, chest tightness, headache, choking, and, in severe cases, death.

The tenacity of trust in petrochemical communities: Reckoning with risk on the Fawley Waterside (1997–2019)

Around the world, people living close to polluting industries have different perceptions of the risks of toxic exposure, ranging from anger to acceptance to denial. We draw attention to a case with relatively high levels of social trust, but also relatively high levels of risk perception: the communities living adjacent to the Fawley (UK) oil refinery and petrochemical complex, a site that has been operated by Esso since the early 1950s. Our findings are based on a novel comparative analysis of two qualitative studies of local risk perceptions in Fawley conducted more than two decades apart in 1997 and 2019, incorporating focus group and individual interviews with residents, alongside documentary analysis and stakeholder interviews. Perceptions of risk and trust in the local polluting industry have fluctuated over the years, with unease never far from the surface as industrial employment has slowly contracted. Yet overall, the picture in 2019 was not too dissimilar from that in 1997: while community–industry relations were strained amidst periodic risk incidents and a sense of decline, a cautious sense of trust in the polluting enterprise had endured, based on a delicate balance of heritage, risk, and recognition. We draw attention to the residents’ careful reckoning with risks over time and the tenacity of social trust as an act of negotiation that took risk into account but also included other important factors such as recognition and reciprocity. Local risk perceptions in Fawley are closely bound up with the residents’ shared industrial heritage and enduring perceptions of Esso as a ‘good neighbour’. Our longitudinal analysis allowed us to reflect on changes over time in Fawley, providing greater temporal depth to the risk perception literature.

Development and Application of Multidimensional Lipid Libraries to Investigate Lipidomic Dysregulation Related to Smoke Inhalation Injury Severity

Lipids play many biological roles including membrane formation, protection, insulation, energy storage, and cell division. These functions have brought great interest to lipidomic studies for understanding their dysregulation in toxic exposure, inflammation, and diseases. However, lipids have shown to be analytically challenging due to their highly isomeric nature and vast concentration ranges in biological matrices. Therefore, powerful multidimensional techniques such as those integrating liquid chromatography, ion mobility spectrometry, collision induced dissociation, and mass spectrometry (LC-IMS-CID-MS) have recently been implemented to separate lipid isomers as well as provide structural information and increased feature identification confidence. These multidimensional datasets are however extremely large and highly complex, resulting in challenges in data processing and annotation. Here, we have overcome these challenges by developing sample-specific multidimensional libraries using the freely available software Skyline. Specifically, the human plasma library developed for this work contains over 500 unique, experimentally validated lipids, which is combined with adapted Skyline functions for highly confident lipid annotations such as indexed retention time (iRT) for retention time prediction and IMS drift time filtering for increased sensitivity and selectivity. For broad comparison with other lipidomic studies, this human plasma database was initially used to annotate LC-IMS-CID-MS data from a NIST SRM 1950 extract, giving comparable results to previous studies. This workflow was then utilized to assess matched plasma and bronchoalveolar lavage fluid (BALF) samples from patients with varying degrees of smoke inhalation injury to identify potential lipid-based patient prognostic and diagnostic markers.

Dichlorodiphenyltrichloroethane and the Adrenal Gland: From Toxicity to Endocrine Disruption

Endocrine disruptors are exogenous compounds that pollute the environment and have effects similar to hormones when inside the body. One of the most widespread endocrine disruptors in the wild is the pesticide dichlorodiphenyltrichloroethane (DDT). Toxic doses of DDT are known to cause cell atrophy and degeneration in the adrenal zona fasciculata and zona reticularis. Daily exposure in a developing organism to supposedly non-toxic doses of DDT have been found to impair the morphogenesis of both the cortex and the medulla of the adrenal glands, as well as disturbing the secretion of hormones in cortical and chromaffin cells. Comparison of high and very low levels of DDT exposure revealed drastic differences in the morphological and functional changes in the adrenal cortex. Moreover, the three adrenocortical zones have different levels of sensitivity to the disruptive actions of DDT. The zona glomerulosa and zona reticularis demonstrate sensitivity to both high and very low levels of DDT in prenatal and postnatal periods. In contrast, the zona fasciculata is less damaged by low (supposedly non-toxic) exposure to DDT and its metabolites but is affected by toxic levels of exposure; thus, DDT exerts both toxic and disruptive effects on the adrenal glands, and sensitivity to these two types of action varies in adrenocortical zones. Disruptive low-dose exposure leads to more severe affection of the adrenal function.

Toxic Waste and Race in Twenty-First Century America

In 1987, the United Church of Christ Commission for Racial Justice released its groundbreaking study, Toxic Waste and Race in the United States. The report found race to be the most significant predictor of where hazardous waste facilities were located in the United States. We review this and other studies of environmental racism in an effort to explain the relationship between race and the proximity to hazardous waste facilities. More recent research provides some evidence that the effect is causal, where polluting industries follow the path of least resistance. To date, the published work using Census data ends in 2000, which neglects the period when economic and political changes may have worsened the relationship between race and toxic exposure. Thus, we replicate findings using data from 2010 to show that racial disparities remain persistent in 2010. We conclude with a call for further research on how race and siting have changed during the 2010s.

Airborne toxic exposure and pulmonary outcomes among petrochemical complex workers

Background Petrochemical workers are exposed to a variety of airborne toxic compounds which have been associated with increased risk for respiratory outcomes. However, long-term exposure to SO2, NO2, O3, H2S and NH3 in relation to spirometric parameters and self-reported respiratory problems is largely unknown. Methods Airborne concentration levels of SO2, NO2, O3, H2S and NH3 were collected from two fixed stations over a 3-year period in a petrochemical complex. We assessed spirometric parameters and respiratory symptoms in the petrochemical workers (n = 200) and in an unexposed group (n = 200). We calculated β-coefficients (β) and odds ratios (ORs) with 95% confidence intervals (CIs) before and after adjustment for covariates. Results The mean airborne pollution levels were 159 µg/m3 for SO2, 43 µg/m3 for NO2, 66 µg/m3 for O3, 6 µg/m3 for H2S, and 24 µg/m3 for NH3. We found a significant reduction in spirometric parameters among petrochemical workers compared to the unexposed: FEV1 (forced expiratory volume in 1s) (adjusted β -12; 95%CI -16, -7.64), FEV1/ FVC (forced vital capacity) (β -7.26; 95%CI -9.23, -5.28), and PEF (peak expiratory flow) (β -6.61; 95%CI -12, -0.76). Additionally, we observed higher adjusted risks for any respiratory symptom (OR 4.69; 95%CI 1.76, 12), mucus (OR 4.36; 95%CI 1.70, 11) and shortness of breath (OR 15; 4.95, 46) among petrochemical workers compared to the unexposed group. Conclusions Most measured airborne pollution levels were within the ambient recommendation levels. Still, long-term exposure to low level airborne pollutants, reduced FEV1, FEV1/FVC and PEF, and increased respiratory symptoms in Iranian petrochemical workers compared to unexposed controls.

The assessment of the time-lapse between toxic exposure and examination in the Toxicology Department in pediatric severe acute poisoning

The exposure of the pediatric population to xenobiotics is an important public health problem because children are particularly vulnerable to the unfavorable consequences of intoxications due to their age-related peculiarities, having limited physiological reserves, and poorly developed metabolic pathways. Numerous studies emphasize the importance of targeted therapeutic intervention and as soon as possible in patients with severe acute intoxications that associate signs of cardiogenic shock due to the severity of cases and the risk of adverse evolution. This paper presents a statistical analysis of the time-lapse impact between exposure to the toxicant and the toxicological examination on the patient's prognosis. Its purpose is to identify both qualitative and quantitative correlations between a series of parameters associated with unfavorable evolution and the time elapsed until the specific toxicological management is performed.
