Contextualização: Atualmente, há uma demanda significativa de encaminhamentos médicos aos serviços terapêuticos quanto às hipóteses diagnósticas de crianças com Transtorno do Déficit de Atenção com Hiperatividade (TDAH). Contudo, identificam-se outros vislumbres de compreensão e cuidado à infância. Processo de intervenção: Assim, este artigo busca descrever e analisar a prática da terapia ocupacional em um grupo de Movimento Vital Expressivo, com crianças, em um dispositivo de cuidado no interior de São Paulo. Síntese das considerações: A partir da descrição dos encontros realizados, à luz do Sistema Rio Aberto e do arcabouço teórico da Terapia Ocupacional, considera-se que práticas de cuidado que respeitam as formas de ser e fazer das crianças corroboram na construção de possibilidades de produção de vida.Palavras-chave: Terpia Ocupacional. However, other glimpses of understanding and care for children are identified. Introduction: Health treatment services in health resorts are a specific type of health care services in the Polish health protection system. In the context of performing the services, this specificity should be indicated in the rules for referring patients to treatment in health resorts. Referrals to treatment or rehabilitation in health resorts must be confirmed by the voivodeship branch office of the Fund, which is competent for the place of residence of the respective beneficiary. By confirming referrals, the voivodeship branch office of the National Health Fund specifies, inter alia, the type, place and date of the treatment. Therefore, it should be pointed out that the process in which the National Health Fund confirms referrals to treatment in health resorts is an important factor impacting the performance of treatment services in health resorts. Material: In order to inform about the performance of health care services in health resorts in 2019, the National Health Fund analysed the data by taking into account, inter alia, quantitative and value data (the value and number of registered medical referrals), medical data (the basis of the referrals, treatment profile, and scope of services), and statistical data (age and sex). Conclusions: From the analysis containing the above mentioned data, which is performed by the National Health Fund, results that the largest age group being treated in health resorts is the group aged from 56 to 65 years, which accounts for 41.03% of the total group of beneficiaries. On the other hand, the smallest groups is comprised of beneficiaries aged from 19 to 40 years, i.e. 1.39% of the total group of beneficiaries. This means that there is a significant disparity in the performance of treatment in health resorts between different age groups. Disparities may also be noticed in the performance of medical referrals to treatment in terms of sex, i.e. women account for 63.67% and men for just 36.33%.

Abstract Introduction In no other country magnetic resonance imaging (MRI) is as frequently used as in Germany. The studyʼs aim is to analyse a daily referral procedure for hip MRI in German healthcare and to estimate ineffective costs for the healthcare system. Material and Methods Over one year 203 consecutive MRIs of the hip joint were analysed retrospectively. Referrals were reviewed for their indications, e.g. prevalence of MRIs to detect intra-articular pathologies in the German population was estimated with data of three health insurances. Results No indication was noted on 21% of the referrals to MRI. On 66% of the referrals a reasonable indications could be identified. There were more uncertainties of the indications for arthrographies. Collecting data concerning the prevalence of MRI for intra-articular hip pathologies is difficult due to the lack of precise diagnosis and procedure coding. The expendable costs caused by MRI of the hip joint amount from 800,000 to 2.4 million € during a one year period. Discussion Medical referrals should be used thoroughly for communication between referrers and radiologists. Contribution of the letter of referral to health economics is underestimated. To improve estimation of prevalences in the diagnostics of intra-articular hip pathologies, precise diagnosis and procedure codings are needed.

BACKGROUND The referral process in acute care remains challenging in many areas including burn care. Mobile phone apps designed explicitly for medical referrals and consultations could streamline the referral process by using structured templates and integrating features specific to different specialties. However, as these apps are competing with commercial chat services, usability becomes a crucial factor for successful uptake. OBJECTIVE The aim of this study was to assess the usability of a mobile phone app for remote consultations and referrals of burn injuries. METHODS A total of 24 emergency doctors and 4 burns consultants were recruited for the study. A mixed-methods approach was used including a usability questionnaire and a think-aloud interview. Think-aloud sessions were video-recorded, and content analysis was undertaken with predefined codes relating to the following 3 themes: ease of use, usefulness of content, and technology-induced errors. RESULTS The users perceived the app to be easy to use and useful, but some problems were identified. Issues relating to usability were associated with navigation, such as scrolling and zooming. Users also had problems in understanding the meaning of some icons and terminologies. Sometimes, some users felt limited by predefined options, and they wanted to be able to freely express their clinical findings. CONCLUSIONS We found that users faced problems mainly with navigation when the app did not work in the same way as the other apps that were frequently used. Our study also resonates with previous findings that when using standardized templates, the systems should also allow the user to express their clinical findings in their own words.

Background: Although ginger is considered a harmless remedial substance for a wide range of medical complaints, according to Persian medicinal texts, its long-term or high-dose consumption is potentially harmful. Case Report: The case of a 43-year-old man, with a complaint of urinary stream interruption, dysuria, and flank pain, following a non-prescribed use of ginger was reported. The symptoms were reported to persist for four years, despite some medical referrals. Remarkably, the symptoms were attested to be shrinking eight weeks after ginger-intake cessation; besides, no further intervention was asserted. Conclusion: The history of herbal remedies use should be considered in patients with any unexplained urinary symptoms. [GMJ.2018;7:e1086]

The present cross-sectional study aimed to assess daytime sleepiness in Chinese adolescents using the Paediatric Daytime Sleepiness Scale (PDSS) and to identify associations between PDSS answers and craniofacial characteristics. A group of 265 Chinese adolescents aged 11-17 years self-completed the PDSS, and their extra- and intra-oral craniofacial characteristics were recorded. Among the participants, 59.7% (157) experienced one or more daytime sleepiness events. No significant associations were found between total PDSS scores and the craniofacial parameters, but when PDSS answers were assessed at the item level, several craniofacial characteristics were found to be positively associated with daytime sleepiness, such as hypertrophic tonsils (P = 0.05), a relatively large tongue (P < 0.01), a bilateral Class II molar relationship (P < 0.05) and increased overjet (P < 0.05). A short lower face (P < 0.01) and a convex profile (P < 0.01) were found to be negatively associated with daytime sleepiness. Daytime sleepiness is commonly reported among Chinese adolescents seeking orthodontic treatment and there are potential associations between the condition and craniofacial characteristics. An assessment of daytime sleepiness is recommended to orthodontists in young patients presenting with hypertrophic tonsils, relative large tongues and Class II tendency malocclusions, and appropriate medical referrals should also be considered.
