Molecular diversity survey on diarrheagenic Escherichia coli isolates among children with gastroenteritis in Fars, Iran

Aim: To differentiate Escherichia coli isolates from diarrheal pediatric patients in clinical laboratories. Materials & methods: Patients with watery diarrhea were selected for sampling and tested for Diarrheagenic E. coli (DEC) by API kit. DEC isolates were tested for phylotyping, pathotyping and presence of determined virulence-encoding genes by specific molecular methods. Results: About 50% of isolates were detected as DECs (>55 and >31% were categorized B2 and D phylotypes respectively). Enterotoxigenic E. coli was the most and Enteroinvasive E. coli was the lowest prevalent pathotypes. csg and fim genes were the most present virulence factors. Conclusion: Typing of E. coli isolates from stool specimens will help to determine the diversity of diarrheal pathogens and take proper decisions to reduce the health burden of diarrheal diseases.

First detection of diarrheagenic Escherichia coli pathotypes in the Mearim River Watershed, Maranhão State, Brazil

The Mearim River Watershed has multiple uses e.g. leisure, navigation, fishing and subsistence agriculture and constitutes the main source of supply for the populations of municipalities situated along its course. In addition to being a water supply source, the existence of the ‘pororoca’ (tidal bore) effect in a stretch of the lower course of the Mearim River attracts people from several Brazilian states and different countries, as it offers excellent conditions for surfing in fresh water. In this respect, given the importance of the watershed, this study was developed to report the detection of diarrheagenic Escherichia coli pathotypes in a stretch of the lower course of the Mearim River, located in the state of Maranhão, Brazil. Thirty water samples were collected from 10 sampling points. To quantify E. coli, the chromogenic enzymatic system was used and positive samples were isolated and biochemically identified. Pure cultures underwent DNA extraction by heating followed by polymerase chain reaction (PCR) characterization. At the time of the collections, an observation schedule was used to record information on the existence of rearing of livestock and domestic animals; businesses; residences; and fruit and vegetable farming on the riverbanks. The samples were analyzed for the mean populations of E. coli, which ranged from 444 to 2,585 MPN mL-1. Twenty bacterial isolates were identified and the diarrheal pathotypes ETEC, typical EPEC and atypical EPEC were detected. The detection of these pathotypes can represent an epidemiological risk and compromise several uses of this water resource, such as irrigation of fruits and vegetables consumed raw, fishing, animal watering and recreation. Structural investments in basic sanitation are essential to minimize environmental degradation resulting from anthropic activities and to act preventively in public health. In addition, the recovery of riparian forests along the watershed and the maintenance of vegetation in these areas are measures to reduce the transport of particles from the soil to the watercourses, improving the qualitative and quantitative characteristics of this water resource.

Antibody therapy against antibiotic-resistant diarrheagenic Escherichia coli: a systematic review

Over four billion episodes of diarrhea occur annually in developing countries with diarrheagenic Escherichia coli (DEC) outbreaks also being reported, until now bacterial diarrhea is conventionally addressed by the antibiotic treatment regimes. In recent decades, the emergence of antimicrobial-resistant strains has become a major obstacle in diarrheal treatment; hence, novel and ideal therapeutics are needed. Notably, 80% of DEC is resistant to first-class antibiotics. Among the existing strategies, passive immunization is considered as an alternative to combat drug-resistant bacteria. Antibodies specific to an antigen can be used for prophylactic and therapeutic purposes. In this review, we have systematically discussed the effect of passive immunotherapy to combat DEC and explored the types and advancements in antibodies used against antibiotic-resistant DEC.

Potential Zoonotic Pathovars of Diarrheagenic Escherichia coli Detected in Lambs for Human Consumption from Tierra del Fuego, Argentina

Diarrheagenic Escherichia coli (DEC) pathovars impact childhood health. The southern region of Argentina shows the highest incidence of hemolytic uremic syndrome (HUS) in children of the country. The big island of Tierra del Fuego (TDF) in Argentina registered an incidence of five cases/100,000 inhabitants of HUS in 2019. This work aimed to establish the prevalence of STEC, EPEC, and EAEC in lambs slaughtered in abattoirs from TDF as well as to characterize the phenotypes and the genotypes of the isolated pathogens. The prevalence was 26.6% for stx+, 5.7% for eae+, and 0.27% for aagR+/aaiC+. Twelve STEC isolates were obtained and belonged to the following serotypes: O70:HNT, O81:H21, O81:HNT, O102:H6, O128ab:H2, O174:H8, and O174:HNT. Their genotypic profiles were stx1c (2), stx1c/ehxA (3), stx2b/ehxA (1), stx1c/stx2b (2), and stx1c/stx2/ehxA (4). Six EPEC isolates were obtained and corresponded to five serotypes: O2:H40, O32:H8, O56:H6, O108:H21, and O177:H25. All the EPEC isolates were bfpA- and two were ehxA+. By XbaI-PFGE of 17 isolates, two clusters were identified. By antimicrobial susceptibility tests, 8/12 STEC and 5/6 EPEC were resistant to at least one antibiotic. This work provides new data to understand the ecology of DEC in TDF and confirms that ovine are an important carrier of these pathogens in the region.

Characterization of Diarrheagenic Escherichia Coli Strains Isolated from Slaughtered Sheep in México

Sheep represent one of the main reservoirs of diarrheagenic Escherichia coli; this microorganism is an etiological agent of food-borne diseases, therefore, this work aimed to identify and characterize the principal pathotypes of diarrheagenic E. coli obtained through rectal swabs and samples from sheep carcasses slaughtered in an abattoir at the central region of Mexico. The isolates were subjected to bacteriological identification, serotyping; phylogenetic classification; detection for virulence factors, and antimicrobial sensibility. A total of 90 E. coli isolates were obtained, diarrheagenic serotypes with health public relevance were found: O76:H19 (5), O146:H21 (3), O91:H10 (2), O6:NM (1), and O8:NM (1). According to pathotype, 47.7% of total isolates were Shiga toxin-producing E. coli, while 3.3% were enteropathogenic, 2.2% enterotoxigenic, and 1.1% enteroinvasive E. coli; the remaining isolates did not express the genes used to assign them to some pathotype. Regarding the Shiga toxin subtypes, 31/43 (72.09%) were cataloged as stx1c, 11/43 (25.5%), stx1a- stx1c and 1/43 (2.3%) stx1a- stx1d; while for stx2 it was possible identify stx2g 4/7(57.14%), stx2b 1/7 (14.7%) and stx2b-stx2g 2/7 (28.5%). Almost all pathotypes (91–100%) belonged to phylogroup B1. Furthermore, it was observed that the 90 isolates showed an antimicrobial resistance of 100% to nitrofurantoin, followed by ampicillin, tetracycline, and trimethoprim-sulfamethoxazole. These results highlight the importance of diarrheagenic E. coli as a potential risk for public health during the slaughtering process.
