PurposeProfessional program assessment is necessary in an accreditation process, in order to ensure educational quality and public accountability. One avenue of assessment is through an internship. The challenge is to determine how evidence from this indirect learning experience can aid in accreditation. The purpose of this paper is to describe the use of internship supervisor evaluation feedback within the accreditation process for a professional interior design degree program.Design/methodology/approachIn this study, internship assessment was provided by feedback from intern supervisors. Ten years of supervisor feedback were analyzed using descriptive statistics and a content analysis of supervisor comments.FindingsTwo hundred forty-seven internship supervisor evaluation documents were analyzed. Overall, supervisors positively evaluated the performance of the intern as Good to Excellent. A majority of supervisors (91%) provided comments that were positive yet vague, as most could not differentiate between the intern and the intern's performance.Practical implicationsThis study links experiential learning to its evidence that can be used in an accreditation process. The challenges for educators in developing an assessment tool useful for accreditation evidence and to be shared by multiple program degree stakeholders are also described.Originality/valueResearch on internships usually focuses on the student's viewpoint. This study is original in that it examines the use of internship supervisor's evidence in program accreditation.

Lockout/tag out (LOTO) refers to specific practices and procedures to stop the release of hazardous energy and turnoff machinery and equipment during service or maintenance activities. An effective LOTO system will ensure the workers are protected from the unexpected conditions during maintenance activities. This paper focus on incidents of LOTO system failures in an electricity power industry. LOTO system had implemented for many years in the company, but many incidents of LOTO system failure was still happened. The purpose of the research was to investigate the cases of incident caused by LOTO procedure failures. The research was conducted using descriptive analytical approach to analyse the cases of incident to find the root causes and develop the solution. Main data used in the research was document of incidents and was analysed by Systematic Cause Analysis Technique (SCAT). The result of investigation showed the root causes of LOTO system failure were caused by individual, job and management factors. Based on SCAT chart synthesis, the control actions were identified. The identified control movement were improve operational procedure of LOTO and proposed job description of supervisor. Evaluation of control action concluded that basically the suggestions were feasible to implement and some adjustments were needed due to implementation.

This reseach aims to explore and uncover synergy with the supervisory teacher performance in relation to improving the quality of learning in Junior High School in West Sulawesi. This reseach will be approached using a quantitative method with survey approach. This reseach used a questionnaire to collect data relating to the performance of supervisors and teachers of PAI to synchronise for find synergy with the performance of both.Synergy of The data obtained will be used to describe the characteristics of the synergy between populations (Teachers of PAI and Supervisors).    The Research shows that the proportion of the balance of the teachers and superintendents in Polman and Majene uneven where supervisors amounted to only 5 people while teachers of PAI 121 people.    Synergy of PAI’s teachers performance and superintendents in Polewali Mandar and Majene on profetional aspect, pedagogic aspects, aspects of leadership suprervisi academic supervisor, supervisor evaluation aspects in general are at the level of medium quality, except for aspects of research and the level of development are very low quality.   Some supporting factors and obstacles in the implementation of the basic tasks and functions of teachers and supervisors motivating factors, among others, the school environment that supports learning of PAI implemented in containers MGM communications that facilitate synergy between teachers and supervisors. Factor inhibitors time of teaching  The Islamic studies are in the limit, vast distances and remote result watchdog could not reach the schools.

Intertextuality is essential in academic writing but it is governed by quite complex conventions. Different intertextuality practices can be judged evidence of academic com-petence or incompetence, or condemned as plagiarism. Learners can break the conventions with criminal intent, or from ignorance, or because their language proficiency prevents them from doing anything else. This paper reports the results of interviews with academic staff in a British university and their postgraduate students about intertextuality practices. The interviewees ahowed a wide range of – sometimes contradictory – attitudes to quotation, referencing, and copying. Several pointed out the developmental advantages of various types of copying.
