Đặt vấn đề: Nhiễm vi nấm ngoài da (dermatophytosis) là một trong những bệnh phổ biến và khó điều trị. Vấn đề chẩn đoán tác nhân gây bệnh chưa được quan tâm triệt để, cộng với tình trạng kháng thuốc ngày càng tăng. Do đó cần thiết phải có nghiên cứu khảo sát các chủng vi nấm ngoài da hiện đang lưu hành trên các bệnh nhân đến khám tại bệnh viện Da Liễu, từ đó có cơ sở chẩn đoán và điều trị hiệu quả. Mục tiêu: Phân lập, định danh và tìm hiểu tỷ lệ của các chủng vi nấm ngoài da. Khảo sát độ nhạy cảm với các thuốc khám nấm hiện nay trên các bệnh nhân đến khám tại bệnh viện Da Liễu Thành phố Hồ Chí Minh. Phương pháp nghiên cứu: cắt ngang mô tả, đối tượng từ 339 mẫu bệnh phẩm da, tóc, móng nghi ngờ do vi nấm ngoài da đến khám ngoại trú tại Khoa Khám bệnh Bệnh viện Da Liễu Thành phố Hồ Chí Minh từ tháng 1 đến tháng 5 năm 2021 có chỉ định soi tươi tìm vi nấm của bác sĩ lâm sàng. Các bệnh phẩm được cấy vào môi trường Dermatophyte test medium (DTM) và Sabouraud dextrose Agar (SDA) để phân biệt và định danh. Các chủng vi nấm ngoài da được thực hiện kháng nấm bằng phương pháp đĩa khuếch tán để đánh giá hiệu lực gồm các chất kháng nấm: fluconazole, griseofulvin, itraconazole, ketoconazole. Kết quả: Tỷ lệ bệnh nhân được chẩn đoán nhiễm nấm ngoài da là 47,2%. Trên 107 mẫu bệnh phẩm nuôi cấy phân lập được vi nấm ngoài da, Trichophyton rubrum chiếm tỉ lệ cao nhất là 63,55%, kế đến là Trichophyton mentagrophytes với tỉ lệ 28,04%, Microsporum gypseum chiếm tỉ lệ 4,67%, và cuối cùng là Microsporum canis có tỉ lệ thấp nhất là 3,74%. Tất cả vi nấm ngoài da đều nhạy với thuốc kháng nấm itraconazole (100%); trong khi đó, mức độ nhạy cảm với griseofulvin là 98%. Đối với thuốc kháng nấm ketoconazole, mức độ nhạy với thuốc đạt 52,9%, và có 30,4% mẫu vi nấm ngoài da kháng với ketoconazole. Kết luận: Tỉ lệ nhiễm nấm da do vi nấm ngoài da của bệnh nhân còn cao; trong đó, loài Trichophyton rubrum là loài thường gặp nhất. Kết quả kháng nấm đồ cho thấy tình trạng đề kháng ngày càng tăng của vi nấm, có thể giảm hiệu quả điều trị.

INFEKSI OPORTUNISTIK TINEA CAPITIS PADA PENDERITA PEDICULOSIS CAPITIS

Pediculosis capitis merupakan penyakit infeksi pada kulit kepala manusia yang disebabkan oleh infestasi ektoparasit Pediculus humanus capitis yang dapat menular di kepala tanpa disadari karena kontak erat di lingkungan yang sama seperti asrama pesantren. Di kepala penderita pediculosis ditemukan banyak lesi pada kulit, rambut saling melekat, bintik-bintik hitam atau coklat pada pangkal rambut, radang pada kulit kepala serta eksudat nanah yang berasal dari luka gigitan Pediculus humanus capitis yang meradang. Infeksi sekunder berupa tinea capitis dapat menyerang penderita pediculosis. Infeksi ini disebabkan dermatofita genus Microsforum sp dan Trichophyton sp. Penelitian ini bertujuan untuk mengetahui jenis jamur penyebab tinea capitis pada kulit kepala santriwati pesantren di Makassar. Jenis penelitian yang digunakan adalah penelitian deskriptif dengan mengkultur spesimen pada media Sabouraud Dextrose Agar (SDA) dan dilanjutkan dengan identifikasi jamur. Hasil penelitian dari 10 sampel menunjukkan bahwa 1 sampel terinfeksi Microsporum audouinii.

Screening of aflatoxin-producing fungi in maize and groundnuts from three regions in Tanzania

This study screened aflatoxin-producing fungi in maize and groundnuts from Dodoma, Singida and Iringa regions in Tanzania, and assessed the level of peoples’ awareness on aflatoxins health effects. One hundred and twenty samples (20 each crop) were collected and inoculated on Sabouraud dextrose agar and one hundred and eleven samples were observed to have fungal growths that were identified to belong to four genera, namely Aspergillus, Rhizopus, Mucor and Dermatophyte with occurrence frequencies of 75, 2.5, 5 and 10%, respectively. Among the four screened genera of fungi, Aspergillus was observed to be the major aflatoxin-producing fungi. Five species of genus Aspergillus, namely A. flavus, A. parasiticus, A. niger, A. fumigatus, and A. terreus were isolated with frequencies of 39.1, 10, 7.5, 15.8 and 2.5%, respectively. Macromorphology and micromorphology of isolated Aspergillus spp were also observed using a light microscope. Furthermore, it was noted that the prevalence of fungi and aflatoxins contamination is more in groundnuts than in maize. In terms of awareness in individuals concerning aflatoxins, only 34% respondents in Dodoma, 29.9% in Singida and 24% in Iringa were aware of aflatoxins. Therefore, the creation of awareness and sensitization on aflatoxins health effects to the people is an important part of intervention ways to forestall and control aflatoxins in Tanzania. PENGARUH EKSTRAK LIDAH BUAYA (ALOE VERA L.) TERHADAP PERTUMBUHAN CANDIDA ALBICANS PADA BASIS GIGI TIRUAN RESIN AKRILIK HEAT CURED

Candida albicans dapat berpenetrasi pada resin akrilik dan berkembang biak pada gigi tiruan sehingga dapat menginfeksi jaringan lunak yang menyebabkan denture stomatitis. Lidah buaya (Aloe vera L.) memiliki sifat anti jamur yang menurut penelitian efektif untuk menghambat pertumbuhan Candida albicans. Penelitian ini bertujuan untuk menjelaskan pengaruh lidah buaya (Aloe vera L.) sebagai pembersih gigi tiruan terhadap jumlah Candida albicans pada basis gigi tiruan resin akrilik heat cured. Subjek penelitian diinkubasi dalam suspensi Candida albicans selama 24 jam pada suhu 37°C. Subjek penelitian berupa lempeng akrilik dibagi menjadi 3 kelompok yaitu kelompok yang direndam dalam ekstrak lidah buaya (Aloe vera L.) konsentrasi 75 %, 100 % dan akuades sebagai kelompok kontrol. Setelah diberi perlakuan, spesimen tersebut dimasukkan ke dalam tabung reaksi yang berisi NaCl 0,9 % 10 ml dan digetarkan dengan vortex selama 30 detik, kemudian 0,1 ml dari larutan tersebut dibenihkan ke dalam Sabouraud Dextrose Agar. Analisis data menggunakan uji Kruskal-Wallis dan Mann-Whitney. Hasil penelitian ini menunjukkan bahwa ekstrak lidah buaya (Aloe vera L.) konsentrasi 75 % dan 100 % dapat menghambat pertumbuhan Candida albicans. Berdasarkan pernyataan tersebut, maka terdapat pengaruh ekstrak lidah buaya (Aloe vera L.) sebagai pembersih gigi tiruan terhadap pengurangan jumlah Candida albicans pada basis gigi tiruan resin akrilik heat cured.

Evaluation of Dermatophyte Test Medium and Sabouraud Dextrose Agar for Isolation of Dermatophyte Species

Background and Objective: Dermatophyte infections require laboratory diagnosis before treatment is started. Although direct microscopy is routinely performed but culture of dermatophytes is the gold standard. However, it takes about 4 weeks for species identification on primary media. Our aim was to compare dermatophyte test medium (DTM) as a screening medium for the isolation of dermatophytes in comparison with sabouraud dextrose agar (SDA). Methods: It was a comparative study carried out at the Department of Microbiology of Post Graduate Medical Institute, Lahore over a period of nine months. Samples were collected from one hundred patients with clinically suspected dermatophytoses after taking informed written consent. The samples were examined microscopically and then inoculated on two types of culture media, one Sabouraud dextrose agar (SDA) with added chloramphenicol, gentacin and cycloheximide and other dermatophyte test medium (DTM) with added chlortetracycline, gentacin and cyclohexamide. Results: Fungal growth was observed in fifty-six samples on culture. Out of the fifty-six positive on cultures, nineteen were that of dermatophytes. Out of n = 100 patients, ten were positive on SDA while n = 14 dermatophyte species were able to grow on DTM. A significantly higher positivity (P ³ 0.05) for isolating dermatophytes was observed by DTM as compared to SDA. DTM was able to isolate (71%) of the dermatophytes in first 10 days. Isolation rate of dermatophyte species was higher (73.68%) on DTM as compared to SDA which was 52.6%. Conclusion: Authors recommend the use of dermatophyte test medium for the primary isolation and identification of dermatophyte species to be more effective and time saving.

Microbiological profile of urban mangrove sediments exposed to effluents from the pharmaceutical industry

Este estudo teve como objetivo avaliar o impacto da liberação de efluentes da indústria farmacêutica, por um longo período, na microbiota de um mangue em São Luís, MA. Veja pontos equidistantes que foram amostrados aleatoriamente e o pH foi analisado através da solução de KCl. Além disso, o perfil microbiológico dos sedimentos da área foi estudado através do isolamento pela técnica de diluição em série usando Muller Hinton (MH), Bushnell Hass (BH) mais antibiótico, Sabouraud Dextrose Agar (SAB) e Eosin Methylene Blue (EMB). Após o isolamento e a contagem de placas, o número maior de colônias no meio MH (30000 UFC / mL), seguido pelo meio BH (3000 UF / mL), meio SAB (2500 UFC / mL) e EMB (400 UFC) / mL ) O pH dos seis núcleos estava na faixa de pH neutro, variando de 7,10 a 7,40.

Effect of ethanol extract from Karuk leaf (Piper sarmentosum Roxb.) on the growth of Malassezia furfur in vitro

In Indonesia, there are numerous therapeutic plants found. Some of the plants used in herbal medicine are Karuk leaf (Piper sarmentosum Roxb.) belong to the Piperaceae family. Karuk leaf has chemical contents such as saponins, polyphenols, flavonoids, and essential oils and many tests are carried out on several bacteria, but testing of fungi is rarely studied. This study aims to determine the ethanol extract activities from karuk leaf in inhibiting the growth of the Malassezia furfur fungus and to determine its minimum inhibition by using the Kirby-Bauer method. The study was conducted by an experimental method of the M. furfur fungus using the Kirby-Bauer method. The ethanol extract from karuk leaf was made in various concentrations and tested on 0.5 McFarland fungus by diffusion on Sabouraud Dextrose Agar (SDA). The results of this analysis showed that the ethanol extract of Karuk leaf could inhibit the M. furfur fungus at a concentration of 30% by 5.3 mm, 40% by 6.6 mm, 50% by 7.6 mm, 60% by 11.3 mm, 70% by 12.5 mm, 80% by 15.6 mm, 90% by 17.4 mm, and 100% by 19.5 mm. Based on the results of the study and the data analysis, it can be concluded that several concentrations of ethanol extract of Karuk leaf affect the growth of M. furfur in vitro.

Uji Aktivitas Antifungi Ekstrak Etanol Kulit Nanas (Ananas comosus (L.) Merr) Terhadap Pertumbuhan Pityrosporum ovale dan Candida albicans Penyebab Ketombe

Ketombe adalah kondisi kulit kepala yang biasanya ditandai dengan gatal dan pengelupasan pada kulit kepala, hal ini dapat disebabkan oleh jamur Pityrosporum ovale dan Candida albicans. Kulit Nanas (Ananas comosus (L.) Merr) merupakan suatu bahan alam yang mengandung senyawa yang berkhasiat sebagai antijamur seperti flavonoid, alkaloid, tannin dan enzim bromelain. Selain itu, kandungan enzim bromelain yang terdapat pada kulit nanas (Ananas comosus (L.) Merr) juga berkhasiat sebagai antiinflamasi, antikanker dan antibakteri. Penelitian ini bertujuan untuk mengetahui aktivitas antifungi ekstrak etanol kulit nanas (Ananas comosus (L.) Merr) terhadap pertumbuhan Pityrosporum ovale dan Candida albicans penyebab ketombe. Uji aktivitas antifungi dilakukan dengan metode difusi cakram menggunakan kertas cakram terhadap lima konsentrasi yaitu 5%, 10%, 15%, kontrol positif ketokonazol 2% dan kontrol negatif DMSO 10%, ditanam pada media Sabouraud Dextrose Agar (SDA) dan media Potato Dextrose Agar (PDA) yang sebelumnya telah ditanamkan biakan jamur uji dengan menggunakan metode swab kemudian diinkubasi pada temperatur 37oC selama 3-5 x 24 jam. Hasil penelitian menunjukkan bahwa dari tiga konsentrasi yaitu 5%, 10% dan 15% memiliki rata-rata diameter zona hambat berturut-turut terhadap Pityrosporum ovale sebesar 7,77 mm, 10,33 mm, 11,99 mm dan kontrol positif ketokonazol 2% sebesar 23 mm. Dimana, pada setiap konsentrasi terdapat perbedaan bermakna (p<0,05). Sedangkan terhadap Candida albicans sebesar 7,99 mm, 10,14 mm, 11,55 mm dan kontrol positif ketokonazol 2% sebesar 23 mm. Dimana, pada setiap konsentrasi terdapat perbedaan bermakna (p<0,05). Berdasarkan hasil penelitian ini, dapat disimpulkan bahwa ekstrak etanol kulit nanas (Ananas comosus (L.) Merr) pada konsentrasi 5%, 10% dan 15% memiliki aktivitas antifungi terhadap pertumbuhan Pityrosporum ovale dan Candida albicans penyebab ketombe.
