Effect of Die Spacer Thickness on the Microshear Bond Strength of CAD/CAM Lithium Disilicate Veneers

Aim. The aim of this study was to compare the microshear bond strength of ceramic veneers with digital die spacer settings at 20, 40, and 100 µm. Materials and Methods. Eighteen milled lithium disilicate microdiscs (IPS e.max CAD, Ivoclar Vivadent) were divided into three groups (n = 6) according to their digital die spacer settings: group A = 20 µm, group B = 40 µm, and group C = 100 µm. Six randomly selected sound maxillary premolars received three microdiscs each. Each microdisc was 1 mm in diameter and 1 mm in height. The buccal surfaces of the premolars were prepared with a 0.5 mm depth in enamel. After cementation, the specimens were thermocycled for 2,500 cycles between 5 and 55°C. Microshear bond strength testing was performed using a universal testing machine until bonding failure. Failure modes were evaluated using a stereomicroscope. Statistical analyses included one-way ANOVA, Tukey’s post hoc test, and chi-square test with a 5% alpha error and 80% study power. Results. The mean microshear bond strength values were calculated in MPa for group A = 31.91 ± 12.41, group B = 29.58 ± 5.03, and group C = 13.85 ± 4.12. One-way ANOVA ( p ≤ 0.05 ) showed a statistically significant difference in microshear bond strength among the three groups. Tukey’s post hoc test showed significant differences between groups A and C ( p = 0.004 ) and between groups B and C ( p = 0.011 ). The failure modes were presented as cohesive, adhesive, and mixed failures. Chi-square test indicated that the failure mode distribution was not significantly different among the three groups ( p = 0.970 ). Conclusion. Higher digital die spacer settings decrease the microshear bond strength of CAD/CAM lithium disilicate veneers.

Comparison of reconstruction of cement space in resin copings fabricated with the use of a 3D printer in single- and three-unit restorations

Marginal and internal gaps are very important factors in fixed prosthetic restorations. The marginal fit is affected by the cementing process of the restoration. One of the techniques used to create the required space between the prepared tooth and the crown is to use a die spacer. The aim of the current study was to assess the accuracy of reconstruction of the cement space in the resin copings of extracoronal restorations fabricated with the use of the 3D printing technique and compare it in single- and three-unit restorations. After taking impressions from the left maxillary first premolar and first molar teeth of a jaw model, a total of 20 models were prepared with the use of a 3D printer for single-unit and three-unit groups (a total of 120 samples) and cemented on their relevant dies. The samples were sectioned buccolingually. The images of the internal gaps were prepared under a light microscope and evaluated. The values in different groups were compared in relation to marginal and internal fit and analyzed with ANOVA (mixed models). Statistical significance was set at P<0.05. Comparison of the accuracy of reconstruction of the cement space in resin copings fabricated with the use of a 3D printer showed that in the single-unit premolar tooth with a presumptive cement space of 45 µm the cement space was 30.45±20.01 µm (P>0.001) at the margin and 36.01±13.52 µm (P>0.001) in the internal area; in the reconstruction of the three-unit molar tooth with a presumptive cement space of 25 µm, the cement space at the margin was 105.97±33.7 µm (P>0.964), with 120.09±63.15 µm (P>0.005) in the internal area. It was concluded from the results of the present study that the accuracy of reconstruction of the cement space in resin copings fabricated with the use of a 3D printer in samples with a larger presumptive cement space was higher than that in samples with a smaller presumptive cement space. The accuracy of the reconstruction of the cement space in resin copings fabricated using a 3D printer in single-unit restorations was higher than that in three-unit restorations.

Resektionsarthroplastik nach Lundborg vs. Pyrocarbon-Spacer (Pyrocardan®) bei fortgeschrittener Rhizarthrose – Eine Zwei-Center-Studie

Zusammenfassung Hintergrund Ziel dieser Studie an zwei Zentren war es, die Ergebnisse nach konventioneller Resektionsarthroplastik nach Lundborg mit der Implantation des Pyrocardan®-Spacers zu vergleichen. Patienten und Methoden Bei symptomatischer Rhizarthrose im Stadium III / IV nach Eaton und Littler wurde bei 12 Patienten eine Resektionsarthroplastik nach Lundborg durchgeführt (Zentrum 1) und bei 8 Patienten ein Pyrocardan®-Spacers implantiert (Zentrum 2). Beiden Gruppen waren bezüglich des Durchschnittsalters, des präoperativen Schmerzniveaus, des radiologischen Schweregrads der Arthrose nach Eaton-Littler und der Anamnesedauer bis zur Operation vergleichbar. Die Patientendaten wurden retrospektiv erhoben und eine Nachuntersuchung nach mehr als 18 Monaten postoperativ durchgeführt, bei der der DASH-Score, das Intervall bis zur Beschwerdefreiheit, das Schmerzniveau, die Griffkraft, die Pinchkraft und die Zufriedenheit mit der Behandlung erhoben wurden. Ergebnisse Beide Gruppen verzeichnen einen ähnlichen Nachbeobachtungszeitraum von 23,6 Monaten (Resektions-Gruppe) vs. 26,1Monaten (Spacer-Gruppe). Die Schnitt-Naht-Zeit betrug für die Resektions-Gruppe 31 min und für die Spacer-Gruppe 29 min. Der mittlere DASH betrug in der Resektions-Gruppe 21,9 und in der Spacer-Gruppe 18,3. Die VAS-Schmerzintensität betrug zum Nachuntersuchungszeitraum für die Spacer-Gruppe 1,5 und für die Resektions-Gruppe 1,0. Das zeitliche Intervall bis zur Beschwerdefreiheit war mit durchschnittlich 3,7 Monaten in der Spacer-Gruppe signifikant kürzer als in der Resektions-Gruppe 5,7. Die Griffkraft und Pinchkraft waren zwischen beiden Gruppen nicht signifikant unterschiedlich. Die Zufriedenheit mit der Behandlung wurde in der Resektions-Gruppe mit durchschnittlich 9,3angegeben, in der Spacer-Gruppe mit 7,4. Schlussfolgerung Im Beobachtungsintervall von jeweils über 1,5 Jahren führten beide Techniken zu einer zufriedenstellenden Gebrauchsfähigkeit und deutlich reduzierten Beschwerdesymptomatik der operierten Hand. Die Implantation des Pyrocardan®-Spacers scheint im Vergleich zur Resektionsarthroplastik leichte Vorteile bezüglich eines kürzeren Zeitraums bis zur kompletten Ausheilung zu haben. Auf der anderen Seite stehen die Materialkosten des Spacers von mehreren Hundert Euro, die bei einer konventionellen Resektionsarthroplastik nicht anfallen.
