Characterization of Transverse Aortic Constriction in Mice Based on the Specific Recruitment of Leukocytes to the Hypertrophic Myocardium and the Aorta Ascendens

Transverse aortic constriction (TAC) is a model that mimics pressure overload-induced left ventricular (LV) hypertrophy in mice. Alterations in immune cell functionality can promote cardiac and vascular remodeling. In the present study, we characterized the time course in innate immune cell dynamics in response to TAC in the different tissues of mice. It was determined whether TAC induces a characteristic leukocyte-driven immune response in the myocardium, aorta ascendens and descendens, spleen, blood, and draining lymph nodes supported by cytokine-driven chemotaxis in mice at 3, 6, and 21 days following surgery. We used complex flow cytometry staining combinations to characterize the various innate immune cell subsets and a multiplex array to determine cytokine concentrations in the serum. The results of the current study indicated that leukocytes accumulate in the myocardium and aorta ascendens in response to TAC. The leukocyte dynamics in the myocardium were dominated by the Ly6Clow macrophages with an early accumulation, whereas the response in the aorta ascendens was characterized by a long-lasting proinflammatory phenotype driven by Ly6Chigh macrophages, neutrophils, and activated DCs. In contrast to the high-pressure environment of the aorta ascendens, the tissue of the aorta descendens did not react to TAC with any leukocyte increase. The levels of proinflammatory cytokines in the blood were elevated in response to TAC, indicating a systemic reaction. Moreover, our findings strongly suggest that cardiac macrophages could origin from splenic pools and reach the site of the inflammation via the blood. Based on the current findings, it can be concluded that the high-pressure conditions in the aorta ascendens cause a characteristic immune response, dominated by the accumulation of leukocytes and the activation of DCs that varies in comparison to the immune cell dynamics in the myocardium and the aorta descendens.

Aorta ascendens und Arcus aortae – endovaskuläre Therapie heute und in der Zukunft

ZusammenfassungPathologien im Bereich des Aortenbogens können isoliert auftreten, weit häufiger jedoch sind auch benachbarte Abschnitte der thorakalen Aorta – die Aorta ascendens oder Aorta descendens – mit betroffen. Die ersten operativen Eingriffe zur Behandlung des Aortenbogens wurden vor knapp 6 Jahrzehnten vorgenommen. Trotz zahlreicher Verbesserungen und Innovationen während des 20. und Anfang des 21. Jahrhunderts bleiben diese Eingriffe mit einer relevanten operativen Mortalität und neurologischen Komplikationsrate vergesellschaftet. Endovaskuläre Techniken sowie moderne Hybridverfahren erweitern das therapeutische Spektrum im Bereich des Aortenbogens zunehmend, auch wenn das offene operative Vorgehen aktuell noch der Goldstandard ist. Die endovaskuläre Behandlung eines Aortenaneurysmas wurde erstmals im Bereich der infrarenalen Aorta abdominalis in den frühen 1990er-Jahren durchgeführt. Es dauerte nicht lange, bis die ersten endovaskulären Therapieversuche zur Behandlung des Aortenbogens unternommen wurden. Schon 1996 berichteten Inoue et al. über den Einsatz der ersten gebranchten Endoprothese zur Behandlung von Aneurysmen im Aortenbogen. Fortwährende Verbesserungen und Verfeinerungen der Implantationstechniken und auch des implantierten Materials führten dazu, dass heutzutage im Bereich der deszendierenden thorakalen und abdominellen Aorta die endovaskuläre Therapie eine immer bedeutsamere Option im Vergleich zu den offenen chirurgischen Verfahren darstellt und diese teils als Goldstandard abgelöst hat. Diese Übersichtsarbeit soll einen Überblick über Voraussetzungen, Ergebnisse, aber auch Grenzen der endovaskulären Chirurgie des Aortenbogens geben.

Krónikus aortadissectio talaján kialakult thoracoabdominalis aneurysma endovascularis kezelése fenesztrált sztentgrafttal

Összefoglaló. Az aortadissectio krónikus stádiumában kialakuló thoracoabdominalis tágulatok megoldása multidiszciplináris megközelítést, nagy felkészültséget és fejlett technológiát igényel. A jellemzően többlépcsős műtétsorozat mortalitása és morbiditása az endovascularis technológia fejlődésével csökkent, de még mindig jelentős. A fenesztrált endovascularis aortaműtét a thoracoabdominalis nyitott műtét alternatívája, mely kisebb mortalitással és morbiditással, rövidebb kórházi tartózkodással jár. Aortadissectio esetén történő alkalmazása az aorta lumenében lévő membrán miatt kihívást jelent. Esetbemutatásunkban egy 56 éves nőbeteget demonstrálunk, aki tíz évvel korábban A-típusú dissectio miatt aorta ascendens rekonstrukción esett át. A követés során csaknem a teljes aorta tágulata alakult ki, melynek megoldása három lépésben történt. Az első lépésben a disszekált aortaív nyitott műtétjét végeztük ’frozen elephant trunk’ technikával, majd az aorta descendens tágulatának endovascularis kezelése történt sztentgraft-implantációval. A műtétsorozat záró lépése egy fenesztrált endovascularis aortaműtét volt, mely egyben ezen technikának az aortadissectio esetében történt első hazai alkalmazását jelenti. Thoracoabdominal aortic aneurysms developing in the chronic phase of an aortic dissection require multidisciplinary approach, experienced operators and advanced technology. The mortality and morbidity rate of these multistage operations were reduced with the latest technical achievements in endovascular repair, but they are still significant. Fenestrated endovascular aortic repair, an alternative of thoracoabdominal open repair, is associated with less mortality and morbidity, shorter hospital stay. Using fenestrated devices in aortic dissection is usually technically demanding due to the dissection membrane. We report the case of a 56-year-old woman, who underwent ascending aortic repair due to type A aortic dissection. During the follow-up, a large thoracoabdominal aneurysm developed involving also the arch. We performed a three-stage operation starting with the open repair of the aortic arch using a ‘frozen elephant trunk’ device followed by a thoracic endovascular aortic repair of the descending aorta. The final stage was a fenestrated endovascular aortic repair, which is the first use of this technique in aortic dissection in Hungary. Orv Hetil. 2021; 162(31): 1260–1264.

P2785Persons with dilated aorta ascendens - who are they?

Background Dilation of the ascending aorta (AscA) may result in life-threatening events. However, it often remains asymptomatic and undiagnosed until the catastrophic complication occurs. The prevalence of dilated AscA in the general population is largely unknown. It is also unclear whether traditional cardiovascular risk factors are associated with dilated AscA. Purpose The aim was to study the prevalence of dilated AscA and its determinants with focus on cardiovascular risk factors in a middle-aged population. Methods Five thousand subjects from the Swedish CardioPulmonary BioImage Study (SCAPIS), aged 50–65 years, underwent transthoracic echocardiography (echo) and coronary CT angiography. Subjects with dilated AscA (diameter ≥40 mm) were identified. Two age- and gender-matched controls with normal AscA diameter were selected per case. Presence of carotid artery plaques was determined by duplex ultrasound. Results Seventy one cases (16 women, 55 men) and 142 controls were included. In cases, echo and CT measurements were 42 (41–43) and 42 (41–45) mm, respectively. Body surface area-adjusted AscA diameters were higher in female than in male cases, 22,7 (21,9–23,6) vs 20,2 (19,0–20,8) mm/m2 (p<0,001), see Figure. Bicuspid aortic valves and mild aortic regurditation were more common in cases than in controls (12% vs 0% and 30% vs 11%). The use of antihypertensive drugs was higher among cases, 47% vs 32%, p=0.035. There were no differences in body size, smoking status, diabetes, history of cardiovascular disease or presence of carotid plaques between cases and controls. Conclusions The prevalence of dilated AscA was 1.4% in a Swedish middle-aged population. Dilated AscA was associated with male sex, aortic valve pathology and antihypertensive medication. Whether dilated AscA is associated with impaired regulation of blood pressure and vascular function deserves further investigation. Also, data raise the intriguing question that a 40 mm cut-off value may not be appropriate for the diagnosis in women.

Erkrankungen der Aorta: von der Aortenklappe bis zur Aortenbifurkation

Was ist neu? Stand der Dinge Sowohl im klinischen Bereich als auch in der Forschung etabliert sich zunehmend das Konzept, die Aorta als Gesamtorgan zu betrachten, das sich von der Aortenklappe bis zur Bifurkation erstreckt. Aortenklappenstenose Aktuell laufen erste randomisierte Studien zur Ausweitung der Indikation zum interventionellen Aortenklappenersatz (TAVI, TAVR) hin zu einem Patientenkollektiv mit niedrigem OP-Risiko. Des Weiteren hat die interventionelle Versorgung der „Low flow, low gradient“-Aortenklappenstenose eine Aufwertung im Empfehlungsgrad erhalten und die interventionelle Valve-in-valve-Prozedur Eingang in den klinischen Alltag bei der individualisierten Patientenversorgung gefunden. Aortenklappeninsuffizienz Kathetergestützt implantierte Klappenprothesen, die dediziert zur Behandlung der isolierten Aortenklappeninsuffizienz entwickelt werden, befinden sich für dieses Krankheitsbild in der klinischen Prüfung. Erkrankungen der Aorta ascendens und descendens Während das Screening von Aortenaneurysmen zur ambulanten Routine wird, sind Studien zum risikostratifizierten Follow-up bei zunehmend endovaskulär versorgten Patienten notwendig, um eine bedarfsgerechte Nachsorge zu ermöglichen.
