Stroke impairs the control of isometric forces and muscle activations in the ipsilesional arm

Stroke often impairs the control of the contralesional arm, thus most survivors rely on the ipsilesional arm to perform daily living activities that require an efficient control of movements and forces. Whereas the ipsilesional arm is often called ‘unaffected’ or ‘unimpaired’, several studies suggested that during dynamic tasks its kinematics and joint torques are altered. Is stroke also affecting the ability of the ipsilesional arm to produce isometric force, as when pushing or pulling a handle? Here, we address this question by analyzing behavioral performance and muscles’ activity when subjects applied an isometric force of 10 N in eight coplanar directions. We found that stroke affected the ability to apply well-controlled isometric forces with the ipsilesional arm, although to a minor extent compared to the contralesional arm. The spinal maps, the analysis of single muscle activities and the organization of muscle synergies highlighted that this effect was mainly associated with abnormal activity of proximal muscles with respect to matched controls, especially when pushing or pulling in lateral directions.

Bimanual coordination associated with left- and right-hand dominance: testing the limb assignment and limb dominance hypothesis

In an experiment conducted by Kennedy et al. (Exp Brain Res 233:181–195, 2016), dominant right-handed individuals were required to produce a rhythm of isometric forces in a 2:1 or 1:2 bimanual coordination pattern. In the 2:1 pattern, the left limb performed the faster rhythm, while in the 1:2 pattern, the right limb produced the faster pattern. In the 1:2 pattern, interference occurred in the limb which had to produce the slower rhythm of forces. However, in the 2:1 condition, interference occurred in both limbs. The conclusion was that interference was not only influenced by movement frequency, but also influenced by limb dominance. The present experiment was designed to replicate these findings in dynamic bimanual 1:2 and 2:1 tasks where performers had to move one wrist faster than the other, and to determine the influence of limb dominance. Dominant left-handed (N = 10; LQ = − 89.81) and dominant right-handed (N = 14; LQ = 91.25) participants were required to perform a 2:1 and a 1:2 coordination pattern using Lissajous feedback. The harmonicity value was calculated to quantify the interference in the trial-time series. The analysis demonstrated that regardless of limb dominance, harmonicity was always lower in the slower moving limb than in the faster moving limb. The present results indicated that for dominant left- and dominant right-handers the faster moving limb influenced the slower moving limb. This is in accordance with the assumption that movement frequency has a higher impact on limb control in bimanual 2:1 and 1:2 coordination tasks than handedness.

Force enhancement after stretch of isolated myofibrils is increased by sarcomere length non-uniformities

When a muscle is stretched during a contraction, the resulting steady-state force is higher than the isometric force produced at a comparable sarcomere length. This phenomenon, also referred to as residual force enhancement, cannot be readily explained by the force-sarcomere length relation. One of the most accepted mechanisms for the residual force enhancement is the development of sarcomere length non-uniformities after an active stretch. The aim of this study was to directly investigate the effect of non-uniformities on the force-producing capabilities of isolated myofibrils after they are actively stretched. We evaluated the effect of depleting a single A-band on sarcomere length non-uniformity and residual force enhancement. We observed that sarcomere length non-uniformity was effectively increased following A-band depletion. Furthermore, isometric forces decreased, while the percent residual force enhancement increased compared to intact myofibrils (5% vs. 20%). We conclude that sarcomere length non-uniformities are partially responsible for the enhanced force production after stretch.

Validity and Reliability of Isometric-Bench for Knee Isometric Assessment

There is a strong need for a new, probably cheaper, smaller, and more portable isometric dynamometer. With this aim, we investigated the concurrent validity and reliability of a low-cost portable dynamometer to measure the isometric strength of the lower limb. Seventeen young participants (age 16.47 ± 0.51 years) were randomly assessed on three different days for knee flexion and extension isometric forces with two different devices: a commonly used isokinetic dynamometer (ISOC) and a portable isometric dynamometer prototype (ISOM). No significant differences were observed between the ISOC and the ISOM (all comparisons p > 0.05). Test–retest comparison showed the ISOM to have high reliability (ICC 0.879–0.990). This study showed that measurements with the ISOM could be performed without systematic bias and with high reliability. The ISOM is a device that is able to assess knee isometric strength with excellent concurrent validity and reliability.

Methodology to Customize Maximal Isometric Forces for Hill-Type Muscle Models

One approach to increasing the confidence of muscle force estimation via musculoskeletal models is to minimize the root mean square error (RMSE) between joint torques estimated from electromyographic-driven musculoskeletal models and those computed using inverse dynamics. We propose a method that reduces RMSE by selecting subsets of combinations of maximal voluntary isometric contraction (MVIC) trials that minimize RMSE. Twelve participants performed 3 elbow MVIC in flexion and in extension. An upper-limb electromyographic-driven musculoskeletal model was created to optimize maximum muscle stress and estimate the maximal isometric force of the biceps brachii, brachialis, brachioradialis, and triceps brachii. Maximal isometric forces were computed from all possible combinations of flexion-extension trials. The combinations producing the smallest RMSE significantly reduced the normalized RMSE to 7.4% compared with the combination containing all trials (9.0%). Maximal isometric forces ranged between 114–806 N, 64–409 N, 236–1511 N, and 556–3434 N for the brachii, brachialis, brachioradialis, and triceps brachii, respectively. These large variations suggest that customization is required to reduce the difference between models and actual participants’ maximal isometric force. While the smallest previously reported RMSE was 10.3%, the proposed method reduced the RMSE to 7.4%, which may increase the confidence of muscle force estimation.

MUSCLE FORCE MAGNITUDES IN THE HUMAN LEG FOR ISOMETRIC EXERCISES WITH VARIOUS RESULTANT FORCE DIRECTIONS AND JOINT ANGLES

Using a gradient-based numerical optimization routine, the force magnitudes required of 10 major sagittal plane leg muscles to create a constant magnitude isometric resultant force against a fixed surface at the toe directed anteriorly, posteriorly, superiorly, and inferiorly were quantitatively predicted for three sets of joint angles: a straight leg configuration, with the knee flexed, and with both the hip and knee flexed. Comparisons over the conditions studied for each individual system muscle found that the maximum variation occurred in knee and hip extensor forces (up to two orders of magnitude). Comparisons within the set of active muscles for each studied condition identified dominant muscles and muscle functions. All anteriorly-directed and posteriorly-directed resultants required a small number of muscles with common functions (mainly knee or hip extensors) and large force magnitudes (O(1000[Formula: see text]N)). In contrast, a large number of muscles, with wide-ranging synergistic and antagonistic functions, acting across multiple joints with relatively small magnitudes (O(100[Formula: see text]N)) were needed to create the superiorly-directed resultant with flexed hip and knee. With good correlation to experimentally measured trends in the interrelationships between leg joint angles and isometric forces, the systematic muscle force prediction and analysis presented in this work can be used to guide the design of targeted muscle strengthening exercises and study of muscle-specific injury.

The strength of the corticospinal coherence depends on the predictability of modulated isometric forces

Isometric compensation of predictably frequency-modulated low forces is associated with corticomuscular coherence (CMC) in beta and low gamma range. It remains unclear how the CMC is influenced by unpredictably modulated forces, which create a mismatch between expected and actual sensory feedback. We recorded electroencephalography from the contralateral hand motor area, electromyography (EMG), and the motor performance of 16 subjects during a visuomotor task in which they had to isometrically compensate target forces at 8% of the maximum voluntary contraction with their right index finger. The modulated forces were presented with predictable or unpredictable frequencies. We calculated the CMC, the cortical motor alpha-, beta-, and gamma-range spectral powers (SP), and the task-related desynchronization (TRD), as well as the EMG SP and the performance. We found that in the unpredictable condition the CMC was significantly lower and associated with lower cortical motor SP, stronger TRD, higher EMG SP, and worse performance. The findings suggest that due to the mismatch between predicted and actual sensory feedback leading to higher computational load and less stationary motor state, the unpredictable modulation of the force leads to a decrease in corticospinal synchrony, an increase in cortical and muscle activation, and a worse performance.
