Ethambutol (EMB) is one of the first-line drugs in the treatment of tuberculosis. EMB induces ocular toxicity which is a rare side-effect that is either dose-related or duration-related effect and is often reversible on therapy discontinuation. The toxicity has been reported at 4 to 12 months of the initiation of the therapy. We reported a rare case of EMB-induced optic neuritis after 1 month of initiation of the therapy. After the withdrawal of the drug, there was complete recovery of the patient. This case description would improve the understanding of the clinical presentation as well as the treatment of ethambutol optic neuritis, especially in recourse-limited settings. It would also enrich the knowledge regarding the prevention strategies and timely recognition of the side effect. Keywords: Ethambutol, Optic neuritis, Isoniazid.

Intra-articular steroid injections are widely prescribed for management of osteoarthritis when oral medication or physiotherapy treatment fails to relieve patient’s pain. Septic arthritis is an uncommon side effect of steroid injection. Common causal micro-organisms are bacterial with Staphylococcus aureus being the most frequent. Fungal septic arthritis is rare and does not usually present beyond the neonatal period. We present a rare case of septic arthritis secondary to fungal infection.

Psychosis is a mental disorder in which the thoughts, affective response, ability to recognise reality and ability to communicate and relate to others are significantly impaired to interfere grossly with the capacity to deal with the reality. Psychosis can also occur as a side effect of some type of drugs and may be misdiagnosed as psychiatric illness. We report a rare case of 30 years old female admitted to Nepal Medical College with hallucinations and delusions following ofloxacin use, by extensive / detailed history taking.

Abstract Introduction: Immunomodulators like Nivolumab and Ipilimumab are important regimens in many malignancies. However, their use is linked with the rare side effect of pituitary dysfunction that if screened and treated can prevent misdiagnosis. Case Description: A 56-year-old male with a history of renal cell carcinoma status post bilateral partial nephrectomy with metastasis to the lung on Nivolumab/Ipilimumab presented with dizziness. He endorsed difficulty with balance, nausea, decreased appetite, feeling dehydrated and recent blood pressure readings in the 90s. Vitals on presentation were stable. An MRI, from 2 weeks ago, showed a slightly enlarged sella but no metastatic disease. The next day, he became hypotensive with minimal response to fluids and was started on Midodrine. However, considering his persistent hypotension, his cortisol level was checked and found to be profoundly low (<1). Literature review of his immunomodulator revealed the possibility of central adrenal insufficiency. In addition, he was noted to have a low TSH (0.02) with normal FT4 (1.08). Subsequently, his ACTH level was also found to be low (<1.5) which further elucidated a central cause for his adrenal insufficiency. Therefore, he was ultimately treated with PO hydrocortisone with plans to taper off in the next few weeks. Conclusion: This case demonstrates a rare yet significant side effect of Nivolumab/Ipilimumab therapy. Timely diagnosis and therapy can alleviate symptoms due to associated hormonal deficiencies. Moving forward it will be interesting to see if starting prophylactic steroids or routine screening will allow us to diagnose pituitary dysfunction due to Nivolumab/Ipilimumab earlier.

Neomercazole is one of the most commonly used drugs to treat hyperthyroidism. Common side effects include rash, hair loss and agranulocytosis. Hepatotoxicity is a common side effect of Propylthiouracil, the other major antithyroid drug, but it has been rarely described as a side effect of neomercazole. Here, a patient presents with hepatitis 6 months after starting neomercazole therapy for graves disease. Diagnosis is based on excluding other causes of hepatitis, and treatment involves removing the offending drug. This is followed by normalization of liver function. Rechallenging should not be done as it can lead to recurrence of symptoms.

Imiquimod cream is an immunomodulatory agent that has been approved by the US Food and Drug Administration for use in the treatment of anogenital warts (condylomata acuminata) due to its local immune effects in activating Toll-like receptors 7 and 8 on antigen-presenting cells, resulting in reduction of the viral load of human papillomavirus with subsequent wart regression. After its application, some side effects are commonly reported, including erythema, edema, scaling, erosion, and ulceration. While pigmentary changes, including vitiligo-like depigmentation, have been mentioned as a possible side effect, they have rarely been reported in the literature. Alterations in pigmentation occur because imiquimod application causes human melanocyte apoptosis and autodestruction resulting in loss of melanocytes. Herein, we report the rare case of a 34-year-old healthy male who developed vitiligo-like depigmentation following imiquimod application for his genital warts. This case report aims to increase physicians’ awareness of this possible side effect that could be irreversible and difficult for the patient to accept.

Electron microscopy and energy dispersive x-ray microanalysis (EDXA) are found to be very useful tools for identification of etiologic agents in pneumoconiosis or interstitial pulmonary disorders. Pulmonary interstitial fibrosis and granulomatosis are frequently associated with occupational and environmental pollution. Numerous reports of pneumoconiosis in various occupations such as coal and gold miners are presented in the literature. However, there is no known documented case of pulmonary changes in workers in the sandpaper industry. This study reports a rare case of pulmonary granulomatosis containing deposits from abrasives of sandpaper diagnosed by using EDXA.
