Abstract.Objective: To implement an Occupational Therapy treatment in elderly people with dementia and frailtyMethod: A longitudinal study of a sample of 23 users, 14 women and 9 men aged between 71 and 92 is performed, the average age 84.66 years, users of a residence, presenting frailty and dementia. Carried out an initial assessment and final tests with Short Physical Performance Battery (SPPB) and Dementia Apathy Interview and Rating (DAIR), participants attend occupational therapy sessions.Statistical analysis: it was performed nonparametric Wilcoxon, Spearman correlation and Mann Whitney U test.Results: We observed that the levels of fragility do not improve, there is no relationship of age with frailty, the level of apathy does not decrease with the intervention, there is no difference in the score of apathy or fragility by sex.Conclusions: the results indicate a maintenance of vital functions on both scales, which can be considered beneficial when dealing with degenerative processes.Keywords: fragility, apathy, dementia of Alzheimer’s disease type, psychomotor activities.Resumen.Objetivo: Implementar un tratamiento de Terapia Ocupacional en personas mayores con demencia y fragilidadMétodo: Se realiza un estudio longitudinal sobre una muestra de 23 usuarios, 14 mujeres y 9 hombres con edades comprendidas entre 71 y 92, siendo la edad media 84.66 años, usuarios de una residencia, que presentan fragilidad y demencia. Se lleva a cabo una evaluación inicial y otra final con las pruebas Batería Corta del Desempeño Físico (SPPB) y Dementia Apathy Interview and Rating (DAIR), los participantes asisten a sesiones de Terapia Ocupacional.Análisis estadístico: se llevan a cabo pruebas no paramétricas de Wilcoxon, Correlación de Spearman y U de Mann Whitney.Resultados: se observa que los niveles de fragilidad no mejoran, que no hay relación de la edad con la fragilidad, el nivel de apatía no disminuye con la intervención, no existen diferencias en la puntuación de apatía ni fragilidad según el sexo.Conclusiones. los resultados indican un mantenimiento de las funciones vitales en ambas escalas, lo que se puede considerar beneficioso al tratarse de procesos degenerativos.Palabras clave: fragilidad, apatía, demencia tipo Alzhéimer, actividades psicomotrices.

In piloting an occupational therapy service in Hampshire Social Services' homes for elderly people, it became evident that the occupational therapy philosophy of enabling independence was perceived by the staff of the homes to be contrary to their role as carers. An approach was required which could demonstrate that the concept of care was not incompatible with that of independence and the CARE© approach was developed. This uses a mnemonic device to show that there are four factors that combine to result in the act of caring: Communicating, Assisting, Rehabilitating and Empathising. When all four factors are present, independence is promoted by enabling the elderly person to function to his/her maximum potential. The CARE approach is currently being used in Hampshire Social Services' homes for elderly people in training programmes for staff. The approach is appropriate to anyone who is involved in caring for others. It is used as a framework that assists the carer to make the transition to the role of an enabler.

O objetivo deste artigo é apresentar as vivências da Terapia Ocupacional na enfermaria de clínica médica do Instituto de Medicina Integral Professor Fernando Figueira, como práticas de estágio supervisionado, no ano de 2019. A experiência evidencia um perfil de adultos e idosos acometidos por múltiplas patologias, com perdas em seu nível de independência funcional e comprometimento das Atividades Básicas de Vida Diária após a hospitalização. As intervenções foram voltadas principalmente para o resgate da funcionalidade e demonstraram efetividade ao longo dos atendimentos. Portanto, reforça-se a importância do terapeuta ocupacional no hospital, para a recuperação da saúde e autonomia dos indivíduos. Abstract: The objective of this article is to present the Occupational Therapy experiences in the medical clinic ward of the Instituto de Medicina Integral Professor Fernando Figueira, as supervised internship practices, in 2019. The experience shows a profile of adults and elderly people with multiple pathologies, with losses in their level of functional independence and impairment of the Basic Activities of Daily Living after hospitalization. The interventions were mainly aimed at the rescue of functionality and demonstrated effectiveness throughout the care. Therefore, the importance of the occupational therapist in the hospital is reinforced for the recovery of health and autonomy of individuals.Key words: Internship; Functionality; Hospital; Occupational therapy. Resumen: El objetivo de este trabajo es presentar las experiencias de terapia ocupacional en el pabellón de la clínica médica del Instituto de Medicina Integral Professor Fernando Figueira, como prácticas supervisadas, en 2019. La experiencia muestra un perfil de adultos y ancianos con patologías múltiples, con pérdidas en su nivel de independencia funcional y deterioro de las Actividades Básicas de la Vida Diaria después de la hospitalización. Las intervenciones se dirigieron principalmente al rescate de la funcionalidad y la efectividad demostrada en toda la atención. Por lo tanto, la importancia del terapeuta ocupacional en el hospital se refuerza para la recuperación de la salud y la autonomía de las personas.Palabras clave: Funcionalidad; Hospital; Prácticas; Terapia ocupacional.

Occupational Therapy Departments are concerned with improving the quality of life for disabled people of varying ages and the increasing number of frail elderly people. Increasing emphasis is being placed upon continuing life in the community and this calls for good communications between O.T. Departments and social services although there is no administrative link between them. For an O.T. Department to fulfill its role effectively there is a need for good and reliable data for planning and evaluation. This survey examines the problem in some detail.
