BackgroundUp to 40% of patients with suspected hip fracture report inadequate or no pre-hospital pain management. Morphine may raise risk of complications and may be avoided by paramedics. Fascia Iliaca Compartment Block (FICB) is used in Emergency Department and orthopaedic wards. The RAPID trial tested feasibility of paramedics administering FICB to patients with suspected hip fracture.ObjectiveTo explore patients’ views and experience of care received for suspected hip fracture and in particular FICB before ambulance transport to hospital.MethodWe interviewed six patients and the daughter of a patient who received FICB to manage their suspected hip fracture. Interviews, by telephone or face-to-face, were audio-recorded with consent. We conducted thematic analysis of transcripts. Two researchers, one paramedic and one lay member were in the analysis teamResultsRespondents’ memory of prehospital care was dominated by their experience of extreme pain. While they recalled events before falling, they only had partial memory of care prior to hip surgery. Although they recalled paramedics’ arrival, which they reported was up to six hours after their injury, respondents said they remembered little else. Just one recalled consenting to receiving FICB and could describe the process. Other respondents said they were in too much pain to comprehend what occurred or respond coherently. They explained their priority was to receive pain management and they expected the paramedics to treat them safely and effectively. Respondents appeared to be a stoical and trusting group who accepted the treatment they were offered. They did recall high quality of care and praised paramedics for their reassuring and calm manner.ConclusionsHip fracture patients’ overwhelming memory of injury and treatment was of pain and their priority was to receive pain relief. The quality of care, reassurance and administration of pain management was more important to patients than the mechanism of delivering the intervention.

Objective: to know the nursing professors’ perception ahead prehospital care to multiple victims. Method: this is about a qualitative study, from phenomenological approach. The sample was composed of five nurses from University professor José do Rosário Vellano  - Unifenas, which has experience and who work or have worked in prehospital care from August 2006 to September 2008, directed by the following question: what is your perception before the assistance pre-hospital to multiple victims? Results: from the analysis of the data, obtained through testimony, it was possible determine five categories: 1) events with multiple victims: living with the stress all the time; 2) the criterion for assessing the scene and victims: putting into practice the training; 3) training and professional recognition: important aspects in the quality of care; 4) work in teams: fundamental aspect in response to multiple victims; 5) experiencing traumatic scenes: how to deal with death. Conclusion: the results show that serve multiple victims is a difficult, stressful, delicate, requiring knowledge a lot and practice skill on the part of professional, and an emotional balance and great human sensitivity. Decriptors: nursing; prehospital; multiple victims.RESUMOObjetivo: conhecer a percepção dos enfermeiros diante do atendimento pré-hospitalar a múltiplas vítimas. Método: trata-se de um estudo qualitativo com enfoque fenomenológico. A amostra constitui-se de cinco enfermeiros docentes da Universidade José do Rosário Vellano - Unifenas, experientes e que atuam ou já atuaram em atendimento pré-hospitalar, entre agosto de 2006 a setembro de 2008, direcionada pela seguinte questão norteadora: Qual sua percepção diante o atendimento pré-hospitalar a múltiplas vítimas? Resultados: a partir da análise dos dados, obtidos por meio de depoimentos, foi possível a elaboração de cinco categorias: 1) ocorrências com múltiplas vítimas: convivendo com o estresse a todo momento; 2) o critério de avaliação da cena e das vítimas: colocando em prática o ABCDE; 3) treinamento e valorização profissional: aspectos importantes na qualidade do atendimento; 4) trabalho em equipe: aspecto fundamental no atendimento a múltiplas vítimas; 5) vivenciando cenas traumáticas: como lidar com a morte. Conclusão: os resultados revelam que atender múltiplas vítimas é uma situação difícil, estressante, delicada, que exige muito conhecimento e habilidade prática por parte do profissional, além de um equilíbrio emocional e muita sensibilidade humana. Método: este es un estudio cualitativo de enfoque fenomenológico, la muestra del estudio consistió de cinco enfermeras de la Universidad el profesor José del Rosario Vellano - Unifenas, que tiene experiencia y que trabajan o han trabajado en Atención prehospitalaria en el período comprendido entre agosto de 2006 a septiembre de 2008, dirigida por la siguiente pregunta: ¿Cuál es su percepción antes de que la asistencia prehospitalar a múltiples víctimas?  Resultados: Del análisis de los datos, conseguido por medio de deposiciones, la elaboración de cinco categorías: 1) ocurrencias con las víctimas múltiples: coexisting lo el estresse todo del momento; 2) el criterio de la evaluación de la escena y de las víctimas: colocando en práctico el ABCDE; 3) entrenamiento y valuación profesional: aspectos importantes en la calidad de la atención; 4) trabajo en equipo: aspecto básico en la atención las víctimas múltiples; 5) el vivir escenas profundamente traumáticas: en cuanto a reparto con la muerte. Conclusión: los resultados divulgan eso para llevar cuidado de víctimas múltiples que es una situación difícil, del estressante, delicado, que exige mucho conocimiento y capacidad práctica de parte del profesional, más allá de un equilibrio emocional y de mucho humano de la sensibilidad. IntroductionHistorically, ambulance services were established to provide rapid transport of patients to hospital. Contemporary prehospital care involves provision of sophisticated ‘mobile healthcare’ to patients across the lifespan presenting with a range of injuries or illnesses of varying acuity. Because of its young age, the paramedicine profession has until recently experienced a lack of research capacity which has led to paucity of a discipline-specific, scientific evidence-base. Therefore, the performance and quality of ambulance services has traditionally been measured using simple, evidence-poor indicators forming a deficient reflection of the true quality of care and providing little direction for quality improvement efforts. This paper reports the study protocol for the development and testing of quality indicators (QIs) for the Australian prehospital care setting.Methods and analysisThis project has three phases. In the first phase, preliminary work in the form of a scoping review was conducted which provided an initial list of QIs. In the subsequent phase, these QIs will be developed by aggregating them and by performing related rapid reviews. The summarised evidence will be used to support an expert consensus process aimed at optimising the clarity and evaluating the validity of proposed QIs. Finally, in the third phase those QIs deemed valid will be tested for acceptability, feasibility and reliability using mixed research methods. Evidence-based indicators can facilitate meaningful measurement of the quality of care provided. This forms the first step to identify unwarranted variation and direction for improvement work. This project will develop and test quality indicators for the Australian prehospital care setting.Ethics and disseminationThis project has been approved by the University of Adelaide Human Research Ethics Committee. Findings will be disseminated by publications in peer-reviewed journals, presentations at appropriate scientific conferences, as well as posts on social media and on the project’s website.
