The COVID-19 pandemic, which was first seen in Wuhan, China, caused by the virus named SARS-CoV-2, causes serious, economic and social problems with the deaths of nearly 1.5 million people all over the world. Although initially diagnosed with COVID-19, only mild/moderate cold/flu-like symptoms were seen in the vast majority of cases, COVID-19 results in severe respiratory failure in individuals with advanced age and chronic disease. Because the infection is very contagious, it makes the prognosis of the patients worse by switching to other patients. It plays an important key role in the healthcare team in breaking the nurses’ chain of infection and minimizing disease and treatment-related complications. Therefore, nurses are expected to take precautions to minimize the transmission of COVID-19 and at the same time meet their care needs with a holistic perspective. This situation is only possible with a qualified and holistic nursing care. It is a necessity for patients who have undergone amputation to receive qualified care both physically and mentally in terms of developing a favorable prognosis. Systematized data collection models, internationally accepted nursing diagnoses and interventions should be used in order to provide optimal and standardized nursing care in patient groups where comorbid conditions such as COVID-19 develop with amputation. In this case report, the nursing care of a transtibially amputated COVID-19 patient is presented using the Functional Health Patterns Model (FSO) and the North American Association of Nursing Diagnostics (NANDA) nursing diagnoses. The case in the study included nursing diagnoses that require the care of physiological problems such as hyperthermia, less than necessary nutrition, risk of fluid electrolyte imbalance, disruption in gas exchange, as well as nursing diagnoses aiming to care for emotional problems such as anxiety and body image deterioration. Thus, it was aimed to meet physiological, emotional and social care needs as a requirement of holistic care in nursing philosophy. Positive care outcomes were determined as a result of the holistic nursing care for the patient. Keywords: diabetic foot, amputation, COVID-19, nursing care, functional health patterns

ABSTRACT One of the nursing processes is determining nursing diagnoses. The determination of nursing diagnoses is based on the theory used, one of which is Carpenito's theory, which is based on patient responses as a guide in determining nursing diagnoses. This theory is not in line with the current situation. So this theory cannot answer the needs of patients because patients who are hospitalized cannot always respond or complain. Based on these problems, the authors provide instructions for determining nursing diagnoses based on the Abdella Faya theory, which is based on body physiological disorders. The service targets are nurses in the Bangil Hospital treatment room. The problem is resolved in two stages of activity, namely preparation and implementation. Preparation is carried out by conducting research in 2017 to see conditions in the field regarding nurses' procedures for determining nursing diagnoses. Implementation is carried out by the lecture method, namely the technique of determining nursing diagnoses and case exercises as a form of nursing diagnosis determination workshop activities. The results of the training activities show the level of success with an indication of the suitability of the material with the needs of nurses in determining nursing diagnoses, a positive response from the participants and the majority (90% of participants have understood the concept and how to determine nursing diagnoses.   Key words: determination, nursing diagnosis, Patient Centered, Bangil  general Hospital

Objective: to apply the nursing process fundament on the Theory of Orem’ Self-care, through the conduct of a report of clinical case, with a patient submitted to bariatric surgery. Methods: this is about a descriptive study, from qualitative approach, report of clinical case type, performed in a patient with morbid obesity, included in Bariatric Surgery Group of a teaching hospital in João Pessoa-PB city. For data collection was used a script adapted to Theory of Orem’ Self-care, which led to the identification of deficits of self-care and therefore to nursing diagnoses. It was then developed the plan of nursing care, with the determination of goals, objectives, method of assistance, type of system and nursing interventions. This study has been approved by the Research Ethics Committee of the Hospital of the Federal University of Paraiba (054/07). Results: from the identification of nursing diagnoses was established and implemented the plan of nursing care with the aim of restoring the patient to prevent postoperative complications, promote recovery and prepare you for the self-care. Conclusion: it is considered that the application of the nursing process, based on Theory of Orem’ Self-care, enabled the provision of assistance and qualified individual, encouraging the patient to participate actively in their treatment, but also to increase their responsibility in the outcome of care.
