Acute Nontraumatic Hemoabdomen in the Dog: A Retrospective Analysis of 39 Cases (1987–2001)

The medical records of 39 dogs with acute nontraumatic hemoabdomen were identified and reviewed. Anemia and hypoalbuminemia were identified in 36/37 (97%) and 25/33 (76%) dogs, respectively. Coagulopathies were identified in 26/31 (84%) dogs. When a definitive diagnosis was obtained, malignant neoplasia was diagnosed most frequently and occurred in 24/30 (80%) dogs. Hemangiosarcoma accounted for 21/30 (70%) diagnoses. Sixteen dogs underwent exploratory laparotomy, of which seven (44%) survived the perioperative period. Of the dogs that did not undergo surgery, 9/23 (39%) survived to be discharged from the hospital.

The complete medical records of 60 dogs with hemoabdomen were reviewed. All dogs underwent exploratory laparotomy and had a diagnosis established by a veterinary pathologist. Final diagnoses were hemangiosarcoma in 38 (63.3%) dogs, splenic hematoma in 16 (26.6%) dogs, splenic torsion in three (5%) dogs, hepatocellular carcinoma in two (3.3%) dogs, and carcinomatosis in one (1.6%) dog. Twenty-four (63.1%) dogs with hemangiosarcoma, 14 (87.5%) dogs with splenic hematoma, three (100%) dogs with splenic torsion, and two (100%) dogs with hepatocellular carcinoma were discharged from the hospital. Dogs with hemangiosarcoma were given a poor prognosis, while dogs with splenic hematoma, splenic torsion, and hepatocellular carcinoma were given a fair to good prognosis.

Concordance analysis using Cohen's Kappa index (􏰀􏰀) has been widely used in clinical research to determine the degree of agreement between two measurements made by the same observer (intra-observer agreement) or between the measurements of two observers on the same variable (inter- observer agreement). In this work, we used 􏰀􏰀 to determine the degree of inter-observer agreement between the reason for consultation and the final diagnosis issued by an interdisciplinary team of The Mexican Institute of Hearing and Language (IMAL). The institute receives around ten pediatric patients per month; the reasons for consulting these patients are diverse. In this research, 53 pediatric patients completed their studies in the IMAL and received a final diagnostic. A research assistant manually collected the data from the medical records of the IMAL Medical Direction. For example, 1) reason for consultation, 2) studies carried out, 3) recommendations, and 4) the final diagnosis. The global concordance between consultation and the definitive diagnosis was 􏰀􏰀 = 0.44 (moderate concordance). Although a Health Professional suggested the reason for consultation, it only covers a clinical aspect. Therefore, we think that the moderate concordance obtained is since an interdisciplinary medical group issues the final diagnostic, achieving a complete diagnosis; this allows the adequate medical care and rehabilitation of the pediatric patient.

Positive fluid balance (FB) during the perioperative period may increase the incidence of postoperative complications, which may lead to longer hospitalization and higher hospital costs. However, a definitive association between positive FB and hospital costs has not yet been established. This retrospective observational study examined the association between perioperative FB and hospital costs of patients who underwent major surgical procedures. Medical records of patients who underwent major surgery (surgery time >2 h, estimated blood loss >500 mL) from January 2010 to December 2017 were analyzed to determine the associations between calculated FB (%, total input fluid—output fluid in liter/weight (kg) at admission) and total hospital cost ($). The analysis included medical data of 7010 patients. Multivariable linear regression analyses showed that a 1% increase in FB in postoperative day (POD) 0 (24 h), 0–1 (48 h), 0–2 (72 h), and 0–3 (96 h) significantly increased the total cost by $967.8 (95% confidence interval [CI]: 803.4–1132.1), $688.8 (95% CI: 566.3–811.2), $591 (95% CI: 485.7–696.4), and $434.2 (95% CI: 349.4–519.1), respectively (all p < 0.001). Perioperative cumulative FB was positively associated with hospital costs of patients who underwent major surgery.

Gossypibomas are a rare cause of surgical morbidity and mortality. When unrecognised in the perioperative period, they can present later with a myriad of abdominal complications. We present an unusual case of gossypiboma that was discovered as a cause of secondary infertility, misdiagnosed as a complex adnexal mass. After a definitive diagnosis was made, the removal of gossypiboma restored fertility in the patient successfully.

The aim: To perform a retrospective analysis of leptospirosis morbidity in Ivano-Frankivsk region, to give the etiological characteristics of leptospirosis according to clinicallaboratory parameters, to determine the peculiarities of the course, prognostic features. Materials and methods: Data of the State Enterprise “Ivano-Frankivsk Regional Laboratory Center of the Ministry of Health of Ukraine”, reports and abstracts of medical records of the Regional Clinical Infectious Diseases Hospital for 2009-2018 were used. Clinical observation of patients, analysis of general-clinical, biochemical, and serological indices were carried out. Results: The leptospirosis morbidity in Ivano-Frankivsk region during 2009-2018 was higher than in general in Ukraine. Activation of foci of leptospirosis L. pomona and grippotyphosa in 2009-2013, decrease of L. grippotyphosa and increase of L. pomona foci and “new” L. autumnalis, australis, bataviae, cynopteri in 2014-2018 were noted. The emergence of leptospirosis L. autumnalis, which had a high virulence and caused a severe course, was noted. Conclusions: The leptospirosis morbidity in the Ivano-Frankivsk region over the past 10 years has exceeded the incidence in Ukraine and varied within 0.62-2.2 per 100,000. During the last 5 years, the serological structure has changed: L. pomona (27.7%) prevailed, L. grippotyphosa decreased (2.1%), L. bataviae and cynopteri, autumnalis increased (by 8.5%). Seasonality is shifted in the autumn-winter period (November-February) (L. grippotyphosa, australis, pomona, canicola). According to forecasts, leptospirosis induced by L. autumnalis was most adverse. Leptospiroses of serogroups L. australis, bataviae, cynopteri had a mild course, there were fewer complications.

Benign intra-abdominal cystic masses in children are rare and they have diverse etiopathogenesis, clinical presentation. The present study highlights the experience in the management of benign intra-abdominal cysts pertaining to the diverse etiologies associated with these lesions. The medical records of our hospital between November 2016 to November 2019 were retrospectively reviewed. Patients with cystic abdominal masses were studied with respect to less different clinical presentations, localization of masses, diagnostic tests, surgical aapproaches, histopathological examinations and outcome. Out of the 55 cases, most common lesion was a choledochal cyst. Miscellaneous diagnosis includes an omental cyst, urachal cyst and a pedunculated bile duct cyst. All the cystic lesions of the abdomen need to be considered as close differentials in clinical practice due to the common presentations and similar symptoms produced by these lesions. All the lesions were managed by exploratory laparotomy except two ovarian cysts which were managed with laparoscopic approach.

Purpose. To identify the clinical course of the «risk-forms» of peripheral vitreochorioretinal dystrophy (PVCRD) depending on sex, age, type and degree of refraction in children, as well as to establish the characteristics of the diagnosis and treatment of PVCRD in childhood. Methods. A retrospective analysis of medical records of pediatric patients who underwent peripheral prophylactic laser retinal coagulation (PPLRC) regarding the «risk-forms» of PVHRD at the ophthalmology department of MAUZ Children’s Clinical Hospital No. 1 from 2017 to 2019 was performed. 241 childr (293 eyes) aged 7 to 17 years (mean age 14.1 ± 2.4 years) were operated. Results. PVCRD in children is asymptomatic and it is detected at the age of 7 to 17 years, regardless of gender, the peak of occurrence is 10–14 years. The most common type of PVCRD is «lattice» dystrophy. PVCRD is predominant in low degree myopia. Quite a lot of cases of PVCRD detected in emmetropic refraction, as well as in hyperopic refraction and combined astigmatism. Conclusions. A thorough examination of the periphery of the fundus is necessary in all children, regardless of age and refraction. The parameters of laser coagulation in children differ from those in adults. Given the characteristics of childhood, the success of the operation depends on the correct preoperative preparation, including psychological preparation, contact of the surgeon with the child during the operation. ABSTRACTA retrospective analysis of 145 medical records from our teaching hospital laboratory showed an overall specificity of greater than 97% for the IgA immunosorbent agglutination assay (ISAGA A) performed on the sera of babies to diagnose congenital toxoplasmosis (CT). These actualized data emphasize the ability of this test to confirm a diagnosis of congenital toxoplasmosis.
