Purpose: The present study aims to evaluate, ex vivo, the accuracy of electronic apex locators Root ZX II and Romiapex-15, for working length (WL) determination in permanent teeth. Materials and Methods: Fourteen single-rooted teeth (incisors and canines), with their apices fully formed were used. The dental crowns were removed. The anatomic length of the tooth (real measurement) was visually determined through the insertion of a size 10 K-file until the tip of the instrument could be observed in the apical foramen under a microscope (20X). Teeth were fixed in a model of resin and adapted into alginate soaked with saline solution, which was used as an  electrical conductor. Using a K-file, root canals were measured electronically using both devices. The results obtained for each apex locator were compared to the real measurements. The accuracy between the devices was statistically analyzed using the Bland-Altman graph, Intraclass Correlation Coefficient (ICC), and Student’s t-test. Results: The mean difference between the measurements using the Root ZX II was 0.277mm greater than the real measurement, while the measurements from the Romiapex-15 were 0.308mm higher on average. The comparison between Root ZX II and Romiapex-15 had no significant difference (p= 0.868). Conclusion: It was concluded that Root ZX II and Romiapex-15 had similar accuracy. Romiapex-15 could be an option for WL determination in permanent teeth.

The purpose of this study was to assess the presence of apical debris remaining in the apical third of flattened root canals of vital and nonvital teeth after biomechanical preparation with Ni-Ti rotary instruments. Fresh-extracted human mandibular incisors were used in this study. The teeth had clinical indication for extraction and were submitted to cold pulp vitality testing and radiographic examination. Eighteen teeth were selected and randomly assigned to two groups (n=9), according to the clinical diagnosis, i.e., pulp vitality or pulp necrosis. The canals were instrumented with the ProTaper NiTi rotary system in the following sequence: S1 - up to the middle third; SX - at the cervical third; S2- up to the apical third; and S1, F1, F2, F3 - at the working length. The canals were irrigated with 1% sodium hypochlorite, dried and were submitted to the histological processing. Sections from the apical third were analyzed by an optical microscope (X40) that was coupled to a computer where the images were captured and analyzed using specific softwares. A grid was placed over these images to assess the total canal area and the areas with debris. Mann-Whitney U-test showed no statistically significant difference (p>0.05) between the teeth with pulp vitality (6.49 ± 3.39) and those with pulp necrosis (5.95 ± 2.22). It may be concluded that the clinical condition of pulp tissue did not interfere with the amount of debris remaining in the apical third of flattened root canals prepared with Ni-Ti rotary instruments.

The periapical radiograph is used to determine the working length in endodontic treatment. However, this radiograph does not define the position of the major apical foramen. Objective: This ex vivo laboratorial study aims to determine the prevalence of the major apical foramen mismatching the root apex in human permanent teeth, because when present, this mismatch makes the radiographic method inaccurate for determining the working length. Material and methods: This study used 400 human permanent teeth: 100 incisors, 100 canines, 100 premolars, and 100 molars, totalizing 634 canals (Ethics Committee, Federal University of Ceará #263/10). The access and negotiation of the root canals were performed with K-type files #10 and #15 until the tip of the instrument were visible at the root foramen through 5-fold magnifying glass. Periapical radiographs were performed on bucco-lingual incidence for single-rooted teeth; the incidence angle was changed for multi-rooted teeth. The films were processed and then analyzed by three calibrated examiners (Kappa 80%). Descriptive statistics and chi-square test were used for analyzing of the results. Results: The prevalence of major apical foramens mismatching the root apex was 19.72% of the canals studied. The incisors showed the lowest prevalence (9%, p<0.05) while the highest prevalence was seen in canine teeth (28%). Conclusion: According to the methodology applied, the prevalence of major apical foramens mismatching the root apex foramens make necessary an adjunct method for determining the working length, because it alone was not capable of determining the foramen position.

Abstract Background Radiographic analysis of tooth morphology is mandatory for accurate calibration of the degree of canal curvature angle and radiographic working length to its real dimensions in case difficulty assessment protocols. This study aimed to determine the impact of the degree of root canal curvature angle on maintaining the real working length and the original canal axis of prepared root canals using a reciprocating rotary instrumentation technique. Methods Radiographic image analysis was performed on 60 extracted single-rooted human premolar teeth with a moderate canal curvature (10°–25°) and severe canal curvature (26°–70°). Working length and longitudinal canal axis were determined using cone-beam computed tomography (CBCT) and digital periapical radiography. The real canal length was determined by subtracting 0.5 mm from the actual canal length. Root canals were prepared using the WaveOne Gold reciprocating file (Dentsply Maillefer, Ballaigues, Switzerland). Results There was no significant relation of the degree of canal curvature angle to the accuracy of radiographic working length estimated on CBCT and digital periapical radiographic techniques (P > 0.05). Postinstrumentation changes in the original canal axis between moderate and severe canal curvature angles, assessed on CBCT and periapical digital radiographic images were statistically non-significant (P > 0.05). Conclusions A standardized digital periapical radiographic method performed similarly to the CBCT technique near to its true working length. No significant interaction exists between the diagnostic working length estimation, postoperative root canal axis modification, and the degree of canal curvature angle, using reciprocating rotary instrumentation technique.

Abstract Background: Ophthalmic formulations of chloramphenicol have poor bioavailability of chloramphenicol in the ocular cavity. Aim: The present study aimed at exploring the impact of different oil mixtures in the form of emulsion on the permeability of chloramphenicol after ocular application. Materials and methods: Selection of oil mixture and ratio of the components was made by an equilibrium solubility method. An emulsifier was chosen according to its emulsification properties. A constrained simplex centroid design was used for the assessment of the emulsion development. Emulsions were evaluated for physicochemical properties; zone of inhibition, in-vitro diffusion and ex-vivo local accumulation of chloramphenicol. Validation of the design using check-point batch and reduced polynomial equations were also developed. Optimization of the emulsion was developed by software Design® expert 6.0.8. Assessment of the osmolarity, ocular irritation, sterility testing and isotonicity of optimized batch were also made. Results: Parker Neem®, olive and peppermint oils were selected as an oil phase in the ratio 63.64:20.2:16.16. PEG-400 was selected as an emulsifier according to a pseudo-ternary phase diagram. Constrained simplex-centroid design was applied in the range of 25-39% water, 55-69% PEG-400, 5-19% optimized oil mixture, and 1% chloramphenicol. Unpaired Student’s t-test showed for in-vitro and ex-vivo studies that there was a significant difference between the optimized batch of emulsion and Chloramphenicol eye caps (a commercial product) according to both were equally safe. Conclusion: The optimized batch of an emulsion of chloramphenicol was found to be as safe as and more effective than Chloramphenicol eye caps.

Abstract Objectives: This study was undertaken to evaluate the efficiency of two rotary nickel titanium instruments and hand instrumentation in removing gutta-percha and sealer from root canals. Study design: – 63 extracted human single rooted premolars were instrumented with K-files and filled using lateral compaction of gutta-percha (GP) and three different sealers. The teeth were randomly divided into three experimental groups of 21 specimens each. Removal of gutta-percha was performed with R-Endo retreatment files, Mtwo retreatment files and Hedstrom files. Time to reach working length and to eliminate filling material was also recorded. The specimens were sectioned for evaluation of the area of the remaining gutta-percha/sealer under stereomicroscope at 6X magnification. Photographs were taken for further analysis using computer image analysis program. The results were statistically analyzed using Analysis of variance (ANOVA) and Tukey’s honestly significant difference test. Results: The R-Endo retreatment system resulted in a smaller percentage of canal area covered by residual GP/sealer than in other groups, a significant difference was found between R-Endo and Mtwo group and between Hedstrom groups (P < 0.001). The Mean operating time was minimum with R-Endo group while it was found to be maximum with hand files. Conclusion: It was concluded that all test techniques left gutta-percha/sealer remnants within the root canal. The R-Endo retreatment files and Mtwo retreatment files system proved to be an efficient method for removing gutta-percha and sealer from single rooted premolars.

This study evaluated the accuracy of the Root ZX (J. Morita, Tokyo, Japan) electronic apex locator in determining the working length when palatal maxillary molar roots are in a relationship with the sinus. Seventeen human maxillary molars with vital pulp were scheduled for an extraction and implant placement as part of a periodontal treatment plan. The access cavity was prepared, and a #10 K file (Dentsply Maillefer, Ballaigues, Switzerland) was inserted into the palatal root using the Root ZX apex locator in order to determine the electronic working length (EWL); then, the teeth were extracted. To determine the real working length (RWL), a #10 K file was introduced into the root canal until its tip touched a glass plate. EWL and RWL were compared. Images reconstructed with CBCT (cone beam computerized tomography) revealed that eight palatal roots were related to the maxillary sinus, whereas nine were not. The results showed a significant difference between the EWL and the RWL of the palatal roots related to the sinus (p < 0.001). No significant difference was observed in measurements of roots not in contact with the sinus (p > 0.05). Within the study limitations, the reliability of Root ZX was influenced by the relationship of the roots with the maxillary sinus.

Abstract The aim of this study was to evaluate the integrity of the coronal seal of Temp-Bond and compare it to Cavit and IRM after post space preparation using S. faecalis as a microbial tracer. In addition, the affect of two methods of gutta percha removal on the apical seal of root canal fillings was also evaluated. Forty extracted human single rooted teeth were prepared chemomechanically and obturated with gutta percha and AH26 sealer cement using the lateral cold condensation technique to a standardized working length of 15 mm. About 10 mm of the coronal gutta-percha was removed with either Peeso-reamer or a hot plugger. The roots were divided into three experimental groups of 10 roots and a control group. Each experimental group was subdivided equally into two groups of 15 each according to the method of post space preparation. Cavit, IRM, and Temp-Bond were used to seal the access opening. Each root was fixed in a cuvette containing Tryptic Soya Broth which, covered 2 mm of the root apex. Bacterial suspension was introduced through pipette. Fresh bacterial suspension was added every week, and the system was monitored daily for the growth of microorganisms for a period of one month. The results showed there was no significant difference in terms of coronal leakage between the three coronal materials used (P=0.478), but the methods of gutta-percha removal did have an impact on the apical leakage (P=0.047). The mean value showed the Peeso-reamer provided less leakage compared to using a hot plugger during the 30-day experimental time period. It was concluded the temporary type of coronal seal of endodontically treated teeth will not prevent coronal leakage if left for a long period of time. In addition, permanent cementation of the post with the coronal restoration should be carried out as soon as possible to prevent recontamination of the root canal. The aim of this study was to evaluate the apical microleakage of Biodentine and MTA orthograde apical plugs and to compare the effect of thickness of these biomaterials on their sealing ability. A total of eighty maxillary anterior teeth were used. The apices were removed by cutting with a diamond disc (Jota, Germany) 2 mm from the apical root end in an attempt to standardize the working length of all specimens to 15 ± 1 mm. Both materials were placed in 1–4 mm thickness as apical plugs root canal. Root canal leakage was evaluated by the fluid filtration technique. One-way ANOVA was used in order to determine normality of dispersal distribution of parameters; thereafter, results were analyzed by Kolmogorov-Smirnov test. Overall, between microleakage values of MTA and Biodentine regardless of apical plug thickness, no difference was observed. In terms of plug thickness, a statistically significant difference was observed between the subgroups of MTA and Biodentine (p<0.05). The apical sealing ability of Biodentine was comparable to MTA at any apical plug thickness.

The present paper evaluated the accuracy of two different methods for transferring working length (WL) between manual endodontic instruments and nickel–titanium (NiTi) shaping files. Thirty root canals of extracted permanent teeth were used. Root canals were divided according to canal length (CL) and canal curvature (CC). The reference cusp and the root end were flattened to provide reproducible and accurate measurements. During shaping, the WL measurements were obtained with manual k-files (KF) and transferred to WaveOne (W1) NiTi reciprocating files using the traditional method with the endodontic ruler (method I) and an alternative clinical procedure based on the comparison of the instruments side by side from tip to shank (method II). For each file and each tested method, two measures were taken by two examiners using Rhino (ver. 4.0, McNeel, Seattle, WA, USA) software for a total of 360 (30 × 3 × 2 × 2) measures. Analysis of variance was performed by taking the difference in length (Delta WL, DWL) between files used for the same canal. The difference between methods I and II for WL transfer was found to be statistically significant (df = 1; F = 71.52; p < 0.001). The DWL absolute values obtained with method II were found to be closer to 0 mm (i.e., same length as corresponding KF) than those obtained with method I. Both CL (df = 2; F = 1.27; p = 0.300) and CC (df = 1; F = 2.22; p = 0.149) did not significantly influence WL measurements. With respect to WL transfer, method II seemed to better preserve the correct WL transfer between instruments during the clinical endodontic procedures.
