Physiotherapy practices for patients undergoing coronary artery bypass grafting: a cross- sectional study

Abstract Objective: To evaluate physiotherapy practices in the rehabilitation of patients undergoing coronary artery bypass grafting. Methods: The cross-sectional study was conducted from October 2016 to January 2017 in cardiac care units of two private and two government hospitals in Karachi, and comprised physiotherapists dealing with patients undergoing coronary artery bypass grafting. Data was collected using a modified and validated version of ‘tucker q’ questionnaire. Data was analysed using SPSS 21. Results: Of the 101 physiotherapists, 60(59.4%) were females and 41(40.6%) were males. The overall work experience was 4.89+2.08 years. Of the total, 60(59.4%) physiotherapists used air suctioning in preoperative physiotherapy practices. Relaxation techniques were used by 86(85.10%) professionals and postural drainage by 85(84.20%), while breathing exercises were used by 65(64.4%) subjects in postoperative physiotherapy practices. Conclusion: Physiotherapists were found to be more or less keeping abreast with advances made regarding managing patients undergoing cardiac artery bypass grafting. ObjectiveThe study aims to explore the association between serum proprotein convertase subtilisin/kexin type 9 (PCSK9) level and saphenous vein grafts disease (SVGD) after coronary artery bypass grafting (CABG).DesignA cross-sectional study.SettingA secondary hospital in Tianjin City, China.ParticipantsA total of 231 participants were included in the study. Inclusion criteria were as follows: age ≥18 years, previous CABG surgery at least 12 months ago, at least one SVG for bypass during CABG, abnormal non-invasive test results or recurrent stable angina pectoris by coronary angiography indications, and willing to participate and sign informed consent. Participants with any of the following were excluded from the study: congenital valvular disease, decompensated heart failure, anaemia defined as a haemoglobin level of <12 g/dL in women or <13 g/dL in men, malignant neoplasms, renal failure, severe hepatic disease, thyroid disease, acute or chronic inflammatory disease and chronic obstructive lung disease.Primary outcome measureSVGD was defined as at least one SVG with significant stenosis (≥50%). Circulating PCSK9 levels were measured using commercial ELISA kits according to the manufacturer’s instructions.ResultsThe mean PCSK9 level in the SVGD group was significantly higher than that in the patent group (275.2±38.6 vs 249.3±37.7, p<0.01). The multivariate logistic regression model revealed a significant association between serum PCSK9 and SVGD (OR 2.08, 95% CI 1.46–2.95) per 1 SD increase in serum PCSK9.ConclusionsThe present study is the first to identify an independent association between PCSK9 and late SVGD after adjustment for established cardiovascular risk factors. A multicentre prospective cohort study with large sample size should be conducted in the future to further research this relationship.

Background: Muscle atrophy and prolonged inactivity are associated with an increased sensation of fatigue and reduced functional capacity in the postoperative period in patients undergoing coronary artery bypass grafting. Cardiac rehabilitation after hospital discharge is highly recommended and contributes to improvement in functional capacity and quality of life. However, few studies have evaluated the effectiveness of early mobilization protocols during hospitalization on the patterns of physical activity and functional capacity after coronary artery bypass grafting. Objective: To investigate the effectiveness of an early mobilization program on the functional capacity of patients undergoing coronary artery bypass grafting in the short and long term. Methods: This is a prospective, randomized, controlled, single-blind trial protocol that will evaluate 66 consecutive patients undergoing coronary artery bypass grafting. Patients will be randomized into two training groups: the control group (N = 33), which will perform breathing exercises and the intervention group (N = 33), which will perform breathing exercises and aerobic exercises. The groups will receive treatment from first to the seventh postoperative day, twice daily. In the preoperative period, the following outcomes will be assessed: physical activity level (Baecke Questionnaire), Functional Independence Measure, and functional capacity (6-min walking test). Functional capacity will be reassessed after the 7th and 60th postoperative day. Pulmonary complications and length of hospital stay will also be evaluated. Statistical analysis will be calculated using linear mixed models and will be based on intention-to-treat. The level of significance will be set at α = 5%.

Acute Kidney Injury (AKI) adalah sindrom kompleks yang ditandai dengan penurunan fungsi ginjal. Pada penelitian ini, rasio neutrofil-limfosit (RNL) yang tinggi dikaitkan dengan perkembangan AKI setelah Coronary Artery Bypass Grafting On-Pump (CABG on-pump). Penggunaan RNL sebagai parameter yang tersedia dengan mudah untuk mendeteksi pasien yang berisiko terkena AKI. Tujuan penelitian untuk mengetahui perbedaan nilai RNL pasca operasi CABG on-pump terhadap kejadian AKI dan Non-AKI. Penelitian ini merupakan penelitian analitik dengan pendekatan cross sectional. Semua data klinis preoperative dan postoperative diambil sebagai data sekunder dari rekam medik pasien yang menjalani CABG di Rumah Sakit Umum Daerah Abdul Wahab Sjahranie (RSUD AWS) Samarinda periode Januari 2017-Desember 2020. Penelitian menggunakan uji statistik dengan Mann-Whitney dan Uji Regresi. Hasil penelitian ini ditemukan 41,1% dari 56 sampel mengalami AKI. Hasil didapatkan bahwa terdapat perbedaan nilai RNL pasca operasi pada sampel AKI dan Non-AKI dengan nilai p=0,008. Peningkatan RNL ≥ 10 kali risiko sebesar 6,352 kali lipat.
