BACKGROUND: Hyperglycemia is common in acute stroke patients. Hyperglycemia can induce the production of reactive oxygen species, causing increased activity of matrix metalloproteinase-9 (MMP-9). AIM: This study aimed to determine an association between the increased levels of MMP-9 and the incidence of hyperglycemia in acute ischemic stroke patients. METHODS: This is a case-control study. Acute ischemic stroke patients admitted to the Stroke Unit of a reference hospital in Yogyakarta, Indonesia was divided into the hyperglycemic and non-hyperglycemic group. Demographic and clinical characteristics of each subject were recorded, and blood levels of MMP-9 were measured. Seventy-one patients were recruited, 40 subjects in the hyperglycemic group and 31 subjects in the non-hyperglycemic group. RESULTS: The median levels of blood MMP-9 level in the hyperglycemic and non-hyperglycemic group were 974.37 and 748.48 ng/mL, respectively, and the difference was statistically not significant (95% CI, 191.24-2849.53; p = 0.07). When the calculated cut-off point of 600.99 ng/mL was used, the proportion of patients with higher MMP-9 levels was significantly more in the hyperglycemic group compared with the ones in the non-hyperglycemic group (82.5% and 54.8%, respectively; OR = 3.88; p = 0.011). CONCLUSION: We concluded that the proportion of patients with MMP-9 level >600.99 ng/mL was significantly higher in acute ischemic stroke patients with hyperglycemia.ng/mL was significantly higher in acute ischemic stroke patients with hyperglycemia.

Introduction: Saliva is the type of liquid which contains enzyme, hormone, antibody, constituent microbe, and cytokines. Matrix metalloproteinase-9 (MMP-9) is one kind of proteolysis cellular enzyme in saliva that has a role in inflammation. Saliva is very easy to take and noninvasive treatment, proved efficient for early diagnosis. The purpose of this research was to study the difference MMP-9 level in saliva patient with temporomandibular joint disorders between myofascial pain and disc displacement. Methods: Cross sectional comparative study. The research was carried out at the Prosthodontics Clinic of Arifin Achmad Pekanbaru Hospital and at Biomedical Laboratory of Faculty of Medicine, Universitas Andalas Padang, Indonesia. The saliva samples were taken from 37 myofascial pain patients and 37 disc displacement patients. MMP-9 levels were examined by the Elisa and the data result was analyzed using the t-test. Result: The average value of MMP-9 level of disc displacement was 650.98±384.94 pg/mL and myofascial pain was 168.70±41.24 pg/mL. There was a significant difference on MMP-9 level in saliva between myofascial pain and disc displacement (p<0.05). Conclusion: The MMP-9 level in saliva of patients with disc displacement of temporomandibular joint disorders was higher than patients with myofascial pain.

One important reason why diabetic ulcer fails to heal is an increase of collagen degradation by matrix metalloproteinase-9 (MMP-9). Vibration therapy is one of complementary therapies which can reduce inflammation and improve reepithelialization. However, up to present, there is no study that investigate the effect of vibration on reduction of MMP-9 in diabetic ulcer. Therefore, the effect of vibration on reduction of collagen degradation in diabetic ulcer is still unknown. If vibration can reduce collagen degradation in diabetic ulcer, nurse can recommend patients to apply vibration not only during inflammation and proliferation phase, but also during maturation phase since MMP-9 also contributes to collagen degradation during maturation phase.The purpose of this study was to investigate the effect of vibration on reduction of MMP-9 in diabetic ulcer. Diabetic rats were divided into two groups, vibration and control groups. The intensity of polymorphonuclears cells (PMNs) and fibroblast, collagen appearance, and the presence of positive cells for MMP-9 were compared. Fibroblast, PMNs and collagen appearance were stained with H and E staining, and MMP-9 was stained by immunohistochemistry method. The difference in the intensity of fibroblast and PMNs was analyzed by Mann-Whitney U test. The result showed that the intensity of PMNs was significantly lower, and the intensity of fibroblast was higher in vibration group than in control. Collagen alignment in the vibration group was more organized in a more regular fashion than in control group. The intensity of positive cells for MMP-9 was lower in the vibration group than in control group. This study showed that vibration could reduce MMP-9, therefore it could be concluded that vibration could reduce collagen degradation of diabetic ulcer. This study bring implication in the clinical setting that vibration therapy should also be applied during maturation phase since MMP-9 is also released in the maturation phase.
