Objective To investigate peripheral blood lymphocyte subpopulations, particularly helper T (Th) cells and cytokine expression before and after treatment in polymyositis (PM) and dermatomyositis (DM). Patients and Methods Ten patients with PM and 15 with DM were enrolled in this study. As a control we used 11 age-matched healthy subjects. We analyzed peripheral blood lymphocytes using flow cytometry. Serum cytokines were determined by enzyme-linked immunosorbent assay. Results Th2/Th1 and Th2/Th17 ratios were significantly higher in both PM and DM than in controls and this Th2-predominancy is marked in DM associated with interstitial pneumonia. IL-1β, IL-6 and TGF-β were significantly higher in both PM and DM and IL-4 showed a significant increase in DM when compared to controls. There was no significant correlation between either any lymphocyte subpopulations or serum cytokines and clinical severity markers such as the manual muscle strength test, serum creatine kinase, and the total CT score. Th2/Th17 in both PM and DM, as well as Th2/Th1 in the latter, significantly decreased after clinical remission compared with before treatment. Conclusions Th2-predominancy as shown by the increase in Th2/Th1 and Th2/Th17 ratios may suggest active disease in PM/DM but does not reflect clinical severity.

Abstract BackgroundCSF-1 was found to be accumulated in the lesions and peritoneal fluid of endometriosis patients, and CSF-1 induced THP-1-derived macrophages to polarize toward a suppressive phenotype. Researchers found that macrophages were the predominant cells in the peritoneal fluid (PF) of endometriosis patients, and the primary consensus is that the immune status in the PF of endometriosis patients exhibits a depressed state. Does the cytokine CSF-1 induce monocytes to differentiate into macrophages with a DC-SIGN+ suppressive phenotype in endometriosis?MethodsThe level of CSF-1 in control endometrium (N=11), eutopic endometrium (N=17), and ectopic (N=39) endometrium of endometriosis patients was evaluated by real-time polymerase chain reaction and in the PF of control (N=25) and endometriosis (N=35) patients by enzyme-linked immunosorbent assay. CSF-1 was examined by a MILLIPLEX MAP Mouse Cytokine/Chemokine Magnetic Bead Panel in an in vivo study. DC-SIGN+ suppressive macrophages were detected by immunohistochemical staining of tissues and flow cytometric analysis of the PF of control (N=25) and endometriosis (N=35) patients. The phenotypes and biological activities of the resulting macrophages derived from THP-1 cells induced by CSF-1 were compared by an in vitro coculture system with peripheral blood lymphocytes from normal subjects.Results In this study, we found the proportion of DC-SIGN+ suppressive macrophages was larger in the abdominal immune microenvironment of endometriosis patients. CSF-1 was primarily secreted from the ectopic lesions and peritoneum of mice with endometriosis. And, CSF-1 induced the polarization of macrophages toward a DC-SIGN+ suppressive phenotype; this effect was abolished by the addition of anti-CSF-1R. CSF-1 induced DC-SIGN+ macrophages, leading to a depressed status of peripheral blood lymphocytes, including a high percentage of Treg cells and a low percentage of CD8+ T cells. Similarly, blockade with anti-CSF-1R abrogated this biological effect. This is the first study on the predominant role of DC-SIGN+ suppressive macrophages in the depressed immune status of endometriosis patients.Conclusions This is the first study on the predominant role of DC-SIGN+ suppressive macrophages in the depressed immune status of endometriosis patients. Further study of the mechanism and biological activities of CSF-1-induced DC-SIGN+ suppressive macrophages will enhance our understanding of the physiology of endometriosis and indicate new directions for further study.

✓ The mitogenic responsiveness of peripheral blood lymphocytes obtained from Fischer 344 rats inoculated with avian sarcoma virus was studied. In addition, quantitative alterations in lymphocyte subpopulations were determined in these animals. Only peripheral blood lymphocytes from rats bearing astrocytomas had significantly diminished responses to concanavalin A when compared to control responses. The percentage of lymphocyte subpopulations detected in either the peripheral blood or spleen of tumor-bearing rats did not differ from values obtained with control rats. However, rats bearing astrocytomas had a marked decrease in the absolute number of the various lymphocyte subpopulations as a result of lymphopenia. Neither the sarcoma-bearing rats nor the virus-inoculated rats that did not develop tumors exhibited this lymphopenia. In addition, sera from rats bearing astrocytomas diminished the concanavalin A reactivity of spleen cells obtained from normal rats. The results of this study establish the avian sarcoma virus-induced rat astrocytoma model as a useful immunological parallel for the human disease.

The cytokine profile in adenoidal lymphoid tissue was studied in 22 patients. Lymphocytes from adenoid tissues and peripheral blood were submitted for cytokine assays using an enzyme-linked immunosorbent assay kit for interferon-γ, interleukin (IL)–2, IL-4, IL-5, IL-6, and IL-10. Adenoidal lymphocytes appear to produce significantly less Th1 cytokines (IL-2, interferon gamma) compared to the patient's peripheral blood lymphocytes, whereas IL-4 and IL-5 (Th2 cytokines) appear to be synthesized to the same extent as, if not slightly more than, in the homologous peripheral blood lymphocytes. Because the relationship between Th1 and Th2 cytokines is extremely important in modulating the immune response, it is advisable to determine the role of the cytokine profiles of T-lymphocytes in the nasopharynx and its relationship to the development of inflammation of the eustachian tube and middle ear.
