ABSTRACT Objectives: To evaluate the effectiveness of NIC interventions “Teaching: Disease Process”, “Health Education” and “Cardiac Care” in the improvement of NOC results in individuals with Heart Failure (HF), and the Nursing Diagnosis (ND) “Ineffective Health Control”. Methods: Retrospective cohort, conducted at a Health Education in Nursing outpatient clinic. Fourteen patients with HF had follow-up for one year in six bimonthly consultations. The effectiveness of interventions was verified through the change of the result scores through the Friedman’s Non-Parametric Test and Analysis of Non-Parametric Variance for repeated measures. The level of significance was 5%. Results: The interventions were implemented in consultations in a variety of activities. There was a significant improvement in the result scores until the fourth visit (p<0.001). Conclusions: The interventions were effective in improving outcomes in patients with HF and Ineffective Health Control.

ABSTRACTObjective: to evaluate the epidemiological aspects of patients with Heart Failure (HF) and identify the risk factors in patients with HF. Method: this is about a descriptive study, quantitative, performed in a hospital from Fortaleza-CE. The population was composed of patients hospitalized with HF, the sample comprising 100 patients who met the criteria for inclusion. Data were collected in records and interview. Approved by the Ethics Committee n.º 514/2008. Results: prominent mention as sociodemographic characteristics are: male (63%), age> 60 years (43%), presence of hereditary factors (48%), married (67%), retired (42%) and family income < 1 minimum wage (70%). Modifiable risk factors identified: hypertension (61%), diabetes (22%), overweight (40%), hyperlipidemia (18%), sedentary (82%) smokers (11%) and consumers of alcohol (15%). Conclusion: as we conclude, knowing the profile of the population that carry the HF is favorable to develop strategies for health education to prevent cardiovascular complications in people with higher number of risk factors. Método: estudo descritivo, quantitativo, desenvolvido em hospital terciário de Fortaleza-CE. A população foi constituída por pacientes com IC hospitalizados, com amostra de 100 pacientes. Os dados foram coletados nos prontuários e em entrevista. Aprovado pelo comitê de ética sob parecer n.º 514/2008. Resultados: predominaram como características sociodemográficas: sexo masculino (63%); idade > 60 anos (43%); presença de fatores hereditários (48%); casados (67%); aposentados (42%) e com renda familiar < 1 salário mínimo (70%). Fatores de risco modificáveis detectados: hipertensão (61%); diabetes (22%); sobrepeso (40%); dislipidemia (18%); sedentários (82%); tabagistas (11%) e etilista (15%). Conclusão: conclui-se que conhecer o perfil da população portadora de IC é favorável para desenvolver estratégias de educação em saúde a fim de prevenir complicações cardiovasculares nas populações com fatores de risco. <p align="center"><strong><em>Abstrak  </em></strong></p><p><em>Kejadian gagal jantung dari tahun ketahun terus meningkat secara drastis dan<strong> </strong>menyebabkan masalah kesehatan utama di dunia. Berbagai keluhan yang dirasakan pasien gagal jantung baik secara fisik maupun psikologis. Salah satu masalah psikis yang sering ditemukan adalah ansietas. Tujuan penelitian ini untuk mengetahui efektifitas </em><em>terapi relaksasi nafas dalam dan hipnosis 5 jari</em><em> terhadap tingkat ansietas pasien gagal jantung. Penelitian ini menggunakan metode  </em><em>randomized controlled trial dengan d</em><em>esain paralel. Penelitian ini melibatkan sebanyak 50 responden pasien gagal jantung. Tingkat ansietas dinilai dengan menggunakan alat ukur DASS 21 yang dilakukan sebelum dan setelah diberikan terapi </em><em>relaksasi nafas dalam dan hipnosis 5 jari</em><em>. Hasil penelitian ini menunjukkan bahwa terdapat </em><em>perbedaan yang signifikan </em><em>rata – rata tingkat ansietas</em><em> sebelum dan setelah</em><em> pada kedua kelompok, baik pada kelompok relaksasi nafas dalam dan hipnosis 5 jari (p value 0.00) maupun pada kelompok pendidikan kesehatan dengan menggunakan booklet (p value 0.00)</em><em>.</em><em> Terdapat </em><em>perbedaan rata – rata </em><em>tingkat ansietas </em><em>setelah dilakukan </em><em>intervensi relaksasi nafas dalam dan hipnosis 5 jari</em><em> yang signifikan dibandingkan dengan rata</em><em> - rata</em><em> </em><em>tingkat ansietas</em><em> setelah </em><em>pendidikan kesehatan dengan menggunakan booklet (p value 0.039)</em><em>.</em><em> </em><em>Melalui hasil penelitian ini diharapkan dapat menjadi salah satu intervensi mandiri keperawatan yang dapat diterapkan pada pasien gagal jantung khususnya untuk mengatasi ansietas.</em></p><p> </p><p><em>Kata Kunci      : relaksasi nafas dalam; hipnosis 5 jari; heart failure</em></p><p><em> </em></p><p align="center"><strong><em>Abstract</em></strong></p><p><em> </em></p><p><em>The incidence of heart failure increase drastically every years and cause major health problems in the world.</em> <em>Various sign and simptom felt by heart failure patients both physically and psychologically.</em> <em>One psychological problem that is often found is anxiety.</em> <em>The purpose of this study was to determine the effectiveness of deep breath relaxation and 5 finger hypnosis therapy on the level of anxiety in patients with heart failure.</em> <em>This research method is a randomized controlled trial with a parallel design.</em> <em>This study involved 50 respondents of heart failure patients.</em> <em>The level of anxiety was assessed using a DASS 21 measuring instrument that was carried out before and after being given relaxation therapy for deep breathing and 5-finger hypnosis.</em> <em>The results of this study showed that there are significant differences in the average level of anxiety before and after in both groups, both in the group of 5 breaths and deep hypnosis relaxation groups (p value 0.00) and in the health education group using booklets (p value 0.00). There was a difference in the average level of anxiety after a significant relaxation of deep breathing and 5-finger hypnosis compared with the average anxiety level after health education using a booklet (p value 0.039).</em> <em>The results of this study are expected to be one of the nursing interventions that can be applied to patients with heart failure especially to resolve anxiety.</em></p><p><em> </em></p><p><em>Keywords         : relaxation of deep breathing; 5-finger hypnosis; heart failure</em></p>

The purpose of this systematic review is to identify the effect of individual nursing-led, patient sessions on adult care with heart failure (HF) in hospitals, outpatient clinics and at home or community. A systematic review of the intervention study. Using nurse terminology, education, heart failure, self-care in the PubMed database, ScienceDirect, Willey, ProQuest. The articles in question are related to educational interventions by individual nurses to patients with management of heart failure. The results of this review identified nursing-led education sessions to adults with heart failure proven to improve self-care abilities. These results suggest that the education of patients led by nurses to adult patients with heart failure improves self-care. Education led by nurses delivered in hospitals, outpatient and in-home clinics using diverse methods have an impact on improving self-care.

BACKGROUND: Effective education of patients is an integral component in reducing hospital readmissions for patients with heart failure and in improving patients' quality of life. OBJECTIVES: To describe the use of a card sort method for determining the learning needs of patients with heart failure and the ability of nurses and physicians to accurately predict patients' self-determined needs. METHODS: Thirty patients were asked to sort 12 cards with questions related to the content of discharge teaching for patients with heart failure. The nurse and the physician caring for each patient were instructed to sort the cards independently to characterize the learning needs of the patient. RESULTS: Patients most often selected the following as the most important educational topics: What is wrong with my heart? What is the future of my disease? and How will I know if my heart failure is getting worse? Physicians selected the identical top question as their patients did 17% of the time. Physicians matched any of their patients' top 3 choices 34% of the time. Questions selected as most important by patients were chosen by physicians as least important 13% of the time. Nurses chose the same top question as their patients did 23% of the time. Nurses matched any of their patients' top 3 choices 34% of the time. Nurses selected opposite choices from those of their patients 6% of the time. CONCLUSIONS: The card sort method may be an effective tool to ascertain the individual learning needs of patients with heart failure.
