Background: Outcomes of national policy change impact all levels of the organizational hierarchy. The medical education literature is sparse on how reflections from program directors (PDs) on past large-scale policy changes can inform future policy initiatives. To fill this gap, we conducted a national survey on PDs’ perceptions of, and reflections on, decision-making in medical education, accreditation procedures, and the CanMEDS framework implementation. Methods: The survey was distributed to former Canadian specialty medicine PDs (N = 684). Descriptive analysis was performed on quantitative data, thematic analysis was performed on qualitative comments, and comparisons between the quantitative and qualitative findings were performed to identify areas of convergence and/or divergence. Results: A total of 265 (38.7%) former PDs participated. Quantitative analysis revealed that 52.8% of respondents did not feel involved in decision-making regarding policy changes, 45.1% of respondents did not feel prepared to assess the CanMEDS Roles, and PDs were divided on the reasonableness of accreditation documentation. Qualitative analysis produced four themes: communication, resources, expectations of outcomes, and buy-in. Nine sub-themes were also identified. A high level of convergence was identified across the content, with only four areas of divergence identified. Conclusions: Our findings have the potential to inform future policy and/or accreditation changes. Without the lens of those charged with overseeing the implementation, policy evaluation and quality improvement will remain uninformed. PDs, therefore, bring unique insights into our understanding of national policy changes, and without the voices of these frontline implementers, the true success of policy change implementation will be hindered.

Bank Aceh Syariah has altered its policy on the installment pattern of murabahah financing as of December 2010. This changing was triggered by the decrease of margin in Bank Aceh Syariah. Therefore, this study aims at studying the mechanism of installment of murabahah financing before and after the policy changes. This article also intends to identify the risks that appears due to the decrease of margin in Bank Aceh Syariah and also analyze the view of Islamic economis towards the changes. Data for this study was gathered through intreview, observation and documentation study. The data was analyzed using descriptive analysis method. The findings show that there are two mechanisms of installment for murabaha financing at the bank. In first mechanism, bank only required its customers to pay the remaining installment plus the administrative cost, whereas in second mechanism after the policy change, bank provide discount to the customer. The risk encounter is the decrease of income for the bank itself. In Islamic economics perspective, the policy changes on murabahah financing is allowed with the condition no party is harmed and the agreed price on is the final price.

Abstract Background Healthcare providers need training to implement shared decision making (SDM). In Norway, we developed “Ready for SDM”, a comprehensive SDM curriculum tailored to various healthcare providers, settings, and competence levels, including a course targeting interprofessional healthcare teams. The overall aim was to evaluate a train-the-trainer (TTT) program for healthcare providers wanting to offer this course within their hospital trust. Methods Our observational descriptive design was informed by Kirkpatrick´s Model of Educational Outcomes. The South-Eastern Regional Health Authority invited healthcare providers from all health trusts in its jurisdiction to attend. The TTT consisted of a one-day basic course with lectures on SDM, exercises and group reflections followed by a two-day advanced course including an SDM observer training. Immediately after each of the two courses, reaction and learning (Kirkpatrick levels 1 and 2) were assessed using a self-administered questionnaire. After the advanced course, observer skills were operationalized as accuracy of the participants’ assessment of a consultation compared to an expert assessment. Within three months post-training, we measured number of trainings conducted and number of healthcare providers trained (Kirkpatrick level 3) using an online survey. Qualitative and quantitative descriptive analysis were performed. Results Twenty-one out of 24 (basic) and 19 out of 22 (advanced) healthcare providers in 9 health trusts consented to participate. The basic course was evaluated as highly acceptable, the advanced course as complex and challenging. Participants identified a need for more training in pedagogical skills and support for planning implementation of SDM-training. Participants achieved high knowledge scores and were positive about being an SDM trainer. Observer skills regarding patient involvement in decision-making were excellent (mean of weighted t = .80). After three months, 67% of TTT participants had conducted more than two trainings each and trained a total of 458 healthcare providers. Conclusion Findings suggest that the TTT is a feasible approach for supporting large-scale training in SDM. Our study informed us about how to improve the advanced course. Further research shall investigate the efficacy of the training in the context of a comprehensive multifaceted strategy for implementing SDM in clinical practice. A high level of team situational awareness is essential during complex, large-scale missions of autonomous mobile robots. When a situation appears that needs inter-agent interaction for cooperative decision-making, the basic understanding of the current conditions ought to be identical within the group. To achieve this requirement, all emergent information of acute importance must be promptly shared among team members. It is a non-trivial problem for large-sized and distributed robotic teams, especially under hard communication constraints. The problem considered in this paper is to nd an ecient emergency broadcasting strategy for search and survey operations of the robotic groups providing the fastest way for any agent to aware the remaining team in case of any unexpected changes. A number of simple ruled-based heuristics is proposed to treat the problem. The comparison between the suggested approaches is made regarding both the quality of the obtained solutions and the working speed.

AbstractWith the increasing number of overseas talent tasks in China, overseas talent and job fit are significant issues that aim to improve the utilization of this key human resource. Many studies based on fuzzy sets have been conducted on this topic. Among the many fuzzy set methods, intuitionistic fuzzy sets are usually utilized to express and handle the evaluation information. In recent years, various intuitionistic fuzzy decision-making methods have been rapidly developed and used to solve evaluation problems, but none of them can be used to solve the person-job fit problem with intuitionistic best-worst method (BWM) and TOPSIS methods considering large-scale group decision making (LSGDM) and evaluator social network relations (SNRs). Therefore, to solve problems of intuitionistic fuzzy information analysis and the LSGDM for high-level overseas talent and job fit, we construct a new hybrid two-sided matching method named I-BTM and an LSGDM method considering SNRs. On the one hand, to express the decision-making information more objectively and reasonably, we combine the BWM and TOPSIS in an intuitionistic environment. Additionally, we develop the LSGDM with optimized computer algorithms, where the evaluators’ attitudes are expressed by hesitant fuzzy language. Finally, we build a model of high-level overseas talent and job fit and establish a mutual criteria system that is applied to a case study to illustrate the efficiency and reasonableness of the model.

This paper concerns an analysis regarding the performance of X-ray equipment as well as the radiological safety in veterinary facilities. Data were collected from 380 X-ray veterinary facilities countrywide during the on-site regulatory inspections carried out by the Greek Atomic Energy Commission. The analysis of the results shows that the majority of the veterinary radiographic systems perform within the acceptable limits; moreover, the design and shielding of X-ray rooms as well as the applied procedures ensure a high level of radiological safety for the practitioners, operators and the members of the public. An issue that requires specific attention in the optimization process for the proper implementation of veterinary radiology practices in terms of radiological safety is the continuous training of the personnel. The above findings and the regulatory experience gained were valuable decision-making elements regarding the type of the regulatory control of veterinary radiology practices in the new radiation protection framework.

The author develops ideas about the origin of social inequality during the evolution of human societies and reflects on the possibilities of its overcoming. What makes human beings different from other primates is a high level of egalitarianism and altruism, which contributed to more successful adaptability of human collectives at early stages of the development of society. The transition to agriculture, coupled with substantially increasing population density, was marked by the emergence and institutionalisation of social inequality based on the inequality of tangible assets and symbolic wealth. Then, new institutions of warfare came into existence, and they were aimed at conquering and enslaving the neighbours engaged in productive labour. While exercising control over nature, people also established and strengthened their power over other people. Chiefdom as a new type of polity came into being. Elementary forms of power (political, economic and ideological) served as a basis for the formation of early states. The societies in those states were characterised by social inequality and cruelties, including slavery, mass violence and numerous victims. Nowadays, the old elementary forms of power that are inherent in personalistic chiefdom are still functioning along with modern institutions of public and private bureaucracy. This constitutes the key contradiction of our time, which is the juxtaposition of individual despotic power and public infrastructural one. However, society is evolving towards an ever more efficient combination of social initiatives with the sustainability and viability of large-scale organisations.
