Background. To identify the prevalence of allergic diseases, characteristics of the spectrum of allergens in athletes of the highest achievements. Materials and Methods. 417 athletes of the highest achievements were examined (members of the Russian Federation national teams of different sports). To assess the allergic status of the athletes allergological, pharmacological, nutritional histories were collected. Also we used skin prick tests with various groups of allergens, identification of allergen-specific IgE in serum (indicated) to confirm allergy. Results. Among the athletes of highest achievements a high prevalence of AD (24,5%) was revealed. In 20,3% latent sensitization was determined. The sensitization to the domestic and epidermal allergens was predominant (90,2%). In the spectrum of pollen allergens in 62,5% of athletes sensitization to Compositae and Chenopodiaceae pollen was dominated. In structure of AD AR occurred in 50%, hay fever — in 20,5%, urticaria — in 13,7%, drug allergy — in 10,8%, asthma — in 7,8%. Conclusion. Early detection and adequate therapy of AD in athletes will significantly improve the health level and athletic performance.

Currently, the problem of reactive arthritis (ReA) retains its importance due to the fairly high prevalence of the disease, primarily in Russia. Analysis of epidemiological data allows us to put forward a number of possible reasons explaining the different frequency of ReA in certain regions of the Russian Federation and in other countries. The lecture describes the clinical picture of the disease, as well as analyzes the significance of various laboratory techniques aimed at identifying the causative agent of ReA. The Russian diagnostic criteria for ReA are presented. The main approaches to the therapy of ReA are outlined with an emphasis on the use of antimicrobial drugs. The effectiveness and safety of drug immunocorrection (inducers of interferon, polyoxidonium, immunofan, etc.) in the treatment of urogenital chlamydia in patients with ReA have not been confirmed by data from randomized controlled trials.

Allergic rhinitis and bronchial asthma are widespread respiratory allergic diseases. In some territories of the Russian Federation, the dominant cause of pollinosis is ragweed. The aim of the study was to evaluate the clinical and immunological efficacy of ASIT with the allergen Ambrosia artemisiifolia in patients sensitized to Ambrosia trifida in the Samara region. Patients with proven sensitization to Ambrosia trifida was held immunotherapy with Ambrosia artemisiifolia allergoid preseason. After treatment, patients had a decrease in the severity of symptoms of allergic rhinitis according to VAS (p = 0.00001), a decrease in the need for medications (p = 0.0003), as well as the need for corticosteroids against the background of therapy from 34.6% to 0% (p = 0.00001). In 8% of cases, the result of treatment was good, in 69% satisfactory, in 23% unsatisfactory. In the control group, there were no changes in the severity of symptoms (p = 0.858). Also, in the control group, the need for medications remained unchanged and 14.3% of patients continued to use corticosteroids.After ASIT, there was a decrease in the level of IL-4 (p = 0.002), and a decrease in the ratio of IL-4/ IL-10 (p = 0.0063); at the same time, changes in the level of other cytokines (IL-10; IFNγ) were statistically insignificant (p > 0.05). Before treatment, the levels of IL-4/ IL-10 in both groups were comparable, and after treatment, the differences became statistically significant (p = 0.031). We did not get a statistically significant change in the level of IgG4 Amb a 1 or IgG4 Amb trifida. There was no correlation between the level of individual cytokines and the results of treatment. As a result of the conducted ASIT, positive clinical and immunological results were obtained. In most patients, the disease has acquired a controlled course. At the same time, the lack of excellent and low number of good results of ASIT is probably due to the intraspecific allergenic properties of ragweed.

HIV infection is a major threat to national security of the Russian Federation. Unfavorable epidemiological situation of HIV infection and drug addiction in Russia leads to a significant increase in the number of conscripts, recognized unfitfor military service on health grounds in connection with this disease. The quantitative and structural indices of detection of HIV infection in people at the stage of recruitment and during the period of their service in the Armed Forces of the Russian Federation in the dynamics have been analyzed. The most detection rate of HIV infection in soldiers in the regions of the Russian Federation with high prevalence of HIV = disease has been established. With aim to prevent the carrying and further spread of HIV infection in the Armed Forces of the Russian Federation the performance of the mandatory medical examination ofpersons qualifying for induction has been suggested.

The article presents a method for assessing the risk of an unfavorable epidemic situation for a parasitic disease using the example of enterobiasis in the constituent entities of the Russian Federation and a forecast for 2020. The method is based on a point assessment of internal risks developed on the basis of the document «Rapid assessment of events posing an immediate threat to public health. WHO-2012». All constituent entities of the Russian Federation were assessed and divided into three categories: high, medium and low risk of an unfavorable epidemic situation for enterobiasis. According to the results of the assessment, the «risk areas» in 2020 are Moscow, St. Petersburg, Sevastopol, the Republic of Tatarstan, Sverdlovsk Oblast, Khanty-Mansi Autonomous Okrug, Arkhangelsk Oblast, Kemerovo Oblast, Moscow Oblast, Omsk Oblast, Chelyabinsk Oblast. In the Republic of Sakha (Yakutia) and the Republic of Altai, a high prevalence of the population was revealed with low indicators of other criteria, which may indicate an insufficient epidemiological surveillance of enterobiasis, or about unaccounted or unknown risk factors for the disease.

Оральный аллергический синдром представляет собой IgEопосредованную аллергическую реакцию на продукты питания растительного происхождения у лиц с сенсибилизацией к различным пыльцевым аллергенам. Впервые описан клинический случай орального аллергического синдрома при употреблении в пищу лимона у женщины 25 лет. Кроме того, в анамнезе у больной зарегистрированы сезонный аллергический риноконъюнктивит, бронхиальная астма, контактный аллергический дерматит, проявления лекарственной аллергии в виде крапивницы, диареи и др. Установлена сенсибилизация ко многим группам аллергенов и наличие явлений аллергического среднего отита и вызванного аллергенами лабиринтита. Установлена целесообразность аллергенспецифической диагностики с использованием различных методов (лабораторных и прик, прикприктестов) для назначения индивидуальной диеты и гипоаллергенного режима.Oral allergic syndrome is an IgEmediated allergic reaction to foods of plant origin in persons with sensitization to various pollen allergens. A clinical case of oral allergy syndrome caused by consumption of lemon in a 25yearold woman with sensitization to many groups of allergens and presence of allergic otitis media and allergyinduced labyrinthitis is presented. The patient had a history of seasonal allergic rhinoconjunctivitis, bronchial asthma, contact allergic dermatitis, urticaria as a manifestation of drug allergy. In vivo (skin prick tests, prickprick tests) and in vitro allergen specific diagnostics allowed to work out the individual diet and a hypoallergenic regime in this case.

The relevance of this research was determined by the need for enhancement and increase in the efficiency of routine medical examinations of minors in the Russian Federation. The information and statistical materials of medical examination of the children’s population in 2014 in terms of the health of various age groups in a section of federal districts and subjects were studied. The following indicators were studied: coverage of the children’s population by routine medical examination; level and structure of the revealed general and primary morbidity; frequency of dispensary registration; need for additional consultations; and researches and treatment in an outpatient clinic, a day hospital, a hospital with round-the-clock services, and sanatorium organizations as well as coverage by these medical services. A high level of coverage of minors by routine medical examinations was registered. More than one-third of the healthy children’s population passed the medical examination. The revealed general and primary morbidity of the children’s population in more than one-third of subjects is above mean Russian levels. Diseases of the respiratory organs, bone, and muscular and nervous systems are the most common among children, whereas diseases of the bone and muscular system, eyes, and respiratory organs are common among teenagers. Despite the high prevalence of chronic diseases among the children’s population, recommendations for treatment and rehabilitation were given insufficiently during routine medical examinations. Regional characteristics of the medical examination results, which allowed for the identification of the most disadvantaged areas and defects in its organization and the lack of continuity between medical institutions providing this type of service, have been established.

Introduction. One of the reasons for the increase of pertussis in the Russian Federation on the background of the widespread coverage of prophylactic vaccinations of more than 95% is a large number of unreasonable medical recusals and parental refusals and, as a result, untimely prophylactic vaccinations against pertussis infection.Materials and methods. We analyzed 5101 emergency notifications for pertussis in children under 17 years of age in Moscow in 2012-2017 and 300 child development histories for children under 14 years of age in two clinics in Moscow. A сase-сontrol study was conducted to assess the relationship between pertussis and the presence of deviations in the immunization schedule.Results. It was found that the level of timely immunization against pertussis did not exceed 5%. Medical (20%) and parental refusals (45%) prevail among the reasons for delayed vaccination. In the structure of medical exemption, the main causes are perinatal encephalopathy, congenital heart defects, intestinal dysbiosis, allergic diseases, anemia and some chronic diseases.Discussion. The main principle for the formation of a full protective immunity from pertussis infection is the mandatory administration of a full primary course of vaccination against pertussis, with the completion of three vaccinations at the age of 10-12 months and mandatory control over the timely receipt of a booster dose no later than the child reaches two years of age. In addition, it is necessary to introduce additional booster doses of pertussis vaccine at the age of 6-7 years, 12-14 years, vaccination of adults every 10 years, as well as consider the introduction of vaccination of expectant mothers in each pregnancy at 27-36 weeks, vaccination for medical personnel and employees who provide child care services.

The epidemiology of premenstrual dysphoric disorders (PMDD) and the risk factors for PMDD in women of the Russian Federation are still not fully resolved. Objective: to assess the prevalence of PMDD among women in the Russian Federation and to identify risk factors for PMDD. Materials and methods. An anonymous questioning using the questionnaire was carried out by 1326 women of the reproductive period of different regions of the Russian Federation: Moscow (880), Ural (150), Rostov (146) and Novosibirsk (150) regions. Inclusion criteria were: age of 15-45 years, regular menstrual cycle of 25-35 days, lack of psychiatric diagnoses in history. The results. It was found that 15.6 % of Russian women meet the criteria for PMDD. The primary age of the first appearance of PMDD symptoms is 15-19 years. The statistically significant risks of developing PMDD are ages 15-24 years, education at a higher education institution, lack of regular sexual life, history of neurocirculatory dystonia, complication of heredity in neuropsychiatric diseases and premenstrual syndrome, painful periods and complications in childbirth or abortion. Conclusions: the high prevalence of PMDD (15.6 %) in young women of the reproductive period, the presence of psychosocial and biological risk factors for PMDD requires the development of a modified tool for the early diagnosis of PMDD for general practitioners, obstetrician gynecologists, and psychiatrists.

The aim of the study was the improvement and gain in the efficiency of preventive medical examinations of minors. There were studied the information and statistical materials of the dispanserization of the children population of the Russian Federation in 2014 concerning the health of various age groups in terms of Federal districts and subjects. There were analyzed indices of the coverage of the children’s population with preventive routine medical examination; level and structure of the revealed general and primary prevalence; frequency of the dispensary registration; neediness in additional consultations, examinations and treatment in the conditions of out-patient clinic, a day hospital, a hospital of the round-the-clock stay, the sanatorium organizations, as well as the coverage by these medical services. There was recorded the high level of the coverage of minors by routine medical examinations. The ration of healthy children's population was more than one third from all cases who passed medical examination. The revealed general and primary prevalence rate of the children's population in large part (more than one third) subjects has level above the Russian one. The main reasons of children’s morbidity are diseases of respiratory organs, bone and muscular and nervous systems while teenagers mostly have diseases of bone and muscular system, an eye, appendages of the eye and respiratory organs. The high prevalence of chronic diseases among the children’s population in preventive medical examinations failed to be followed by recommendations for treatment and rehabilitation in sufficient volume. Regional features of results of medical examination allowed to identify the most disadvantaged areas, to identify defects in its organization and the lack of continuity between medical institutions providing this type of services.
