scholarly journals Health for Performance: a necessary paradigm shift for youth athletes

Competitive and elite sport often challenges the balance between health and performance, especially when it involves youth athletes. As Lausanne was getting ready to host the 2020 winter Youth Olympic Games, we had the opportunity to reflect on these challenges and on what they mean for sports medicine practitioners. Elite sport pushes athletes to the limit, be it through their own intrinsic drive to achieve ever more, or through the pressures of the multiple stakeholders around sport.

The Youth Olympic Games were introduced in 2010 with the aim to bring young athletes not only an experience of competition on the world stage, but also to help them learn about the Olympic values, explore other cultures and develop the skills to become true ambassadors. The Lausanne 2020 Games provided an excellent opportunity to develop innovative concepts for health promotion and sports-related prevention. The enthusiasm across various sectors of society (political, academic and economy) empowered us to bring together multiple skills to materialize the IOC’s pledge. We developed communication concept around key sports-related issues targeting the competitors and population of our region. We set up pavilions hosting the various modules in Lausanne, at the Olympic Villages (Lausanne Vortex and St. Moritz), where the modules could be tested and videos were presented. Deep Impact is a virtual reality immersion into concussion, And You …? explores the perceptions of sports-related abuse through five excerpts of blockbuster movies. The Safety Clips for youth athletes introduce ten typical adolescent sports medicine topics in a youth-friendly format. Youth and elite (role model) athletes delivered the messages rather than professionals, whilst the content was true to current scientific knowledge. Three key elements emerged. 1. The power of interdisciplinarity and innovation to promote healthy sports. 2. Adolescence is the best time to sensitize athletes and their entourage. 3. The need for strong institutional and political support to develop health prevention in sport. We look forward to building on these positive experiences and bring prevention and health in sports forward.

Decision making in health care has become increasingly reliant on information technology, evidence-based processes, and performance measurement. It is therefore a time at which it is of critical importance to make data and analyses more relevant to decision makers. Those who support Bayesian approaches contend that their analyses provide more relevant information for decision making than do classical or “frequentist” methods, and that a paradigm shift to the former is long overdue. While formal Bayesian analyses may eventually play an important role in decision making, there are several obstacles to overcome if these methods are to gain acceptance in an environment dominated by frequentist approaches. Supporters of Bayesian statistics must find more accommodating approaches to making their case, especially in finding ways to make these methods more transparent and accessible. Moreover, they must better understand the decision-making environment they hope to influence. This paper discusses these issues and provides some suggestions for overcoming some of these barriers to greater acceptance.

This case study examines the personal experiences of an elite athlete, coach, and sport psychology consultant (SPC) during the athlete’s preparation and performance in a recent Olympic Games. The qualitative research details how the consultancy process was affected by the athlete’s late admission of the deteriorating relationship with his coach. The concepts of closeness, commitment, complementarity, and co-orientation provided a theoretical perspective to the SPC’s interpretation of athlete performance and the interpersonal conflict that developed between athlete and coach. The basic performance demand model provided an applied perspective. The SPC’s commentary adopts a reflexive discursive style that also focuses on the SPC’s role in the consultancy process and the effectiveness of the performance demand model materials. Five important recommendations arise from the case study, and these might inform other SPCs’ future athlete–coach consultancies and interventions.

Best value PIPS has been introduced in the Netherlands in the procurement of construction systems. The Water Board De Dommel utilized best value PIPS to select engineering consultant services. The test differed from other Dutch tests in that the PIPS process used the pre-award phase before the award phase. The best value selection of engineers also proposes that design firms can compete on value (price and performance). The engineering selection process was much closer to the original BVP/PIPS than the construction phases. The major lesson learned is the BVP/PIPS is a paradigm shift. The success of the test led to the recommendation of more professional services using BVP/PIPS.

Nutrition is an undeniable part of promoting health and performance among football (soccer) players. Nevertheless, nutritional strategies adopted in elite football can vary significantly depending on culture, habit and practical constraints and might not always be supported by scientific evidence. Therefore, a group of 28 Portuguese experts on sports nutrition, sports science and sports medicine sought to discuss current practices in the elite football landscape and review the existing evidence on nutritional strategies to be applied when supporting football players. Starting from understanding football’s physical and physiological demands, five different moments were identified: preparing to play, match-day, recovery after matches, between matches and during injury or rehabilitation periods. When applicable, specificities of nutritional support to young athletes and female players were also addressed. The result is a set of practical recommendations that gathered consensus among involved experts, highlighting carbohydrates periodisation, hydration and conscious use of dietary supplements.

Abstract Sanitation policy and development has undergone a paradigm shift away from supply-driven toward behavioral-based demand-driven approaches. This shift to increase sanitation demand requires multiple stakeholders with varying degrees of interest, knowledge, and capacity. Currently, the design of appropriate sanitation technology disconnects user preference integration from sanitation technology design, resulting in fewer sanitation technologies being adopted and used. This research examines how preferences for specific attributes of appropriate sanitation technologies and implementation arrangements influence their adoption and usage. Data collected included interviews of 1,002 sanitation users living in a peri-urban area of South Africa; the surveyed respondents were asked about their existing sanitation technology, their preferences for various sanitation technology design attributes, as well as their perspectives on current and preferred sanitation implementation arrangements. The data revealed that user acceptability of appropriate sanitation technology is influenced by the adoption classification of the users. Statistically significant motives and barriers to sanitation usage showed a differentiation between users who share private sanitation from those who use communal sanitation facilities. The user acceptability of appropriate sanitation systems is dependent on the technical design attributes of sanitation. The development of utility functions detailed the significance of seven technical design attributes and determined their respective priorities.

In this exploratory study, we first intended to identify the underlying structure of components affecting the sport participation of 1258 elite youth athletes from the USA through a principal component analysis of 23 items related to motives and barriers to participation and created for this study. A six-component solution was proposed, including college and professional aspirations and competence beliefs, coach and peer relationships, pressures from parents and coach, intrinsic and self-determined extrinsic motivation, external barriers, and non–self-determined extrinsic motivation. Noteworthy differences by age, gender, and race on those retained components were also reported. Lastly, results of hierarchical regression analyses indicated that 28% of the variance in sport enjoyment was explained by all retained components. Given the rapid growth of travel teams, this study provides a timely cross-sectional and multisport assessment of the state of elite youth sport in the USA. In consideration of optimistic outcomes observed in this study compared to alarming attrition trends commonly found in the elite youth sport environment, this study may serve as a reference as to the elements that influence sustained participation that is indispensable in today's youth sport landscape. Results also provided important theoretical contributions with respect to motivational processes underlying elite sport participation. Practical implications with regards to the influence of intrapersonal, interpersonal, and barrier elements on elite sport participation are also presented.

This article presents and analyses certain playing characteristics of men's and women's 3 × 3 basketball in relation to shooting at the basket. Our intention was to compare this young and developing basketball discipline with the more established and traditional 5v5 basketball. For this purpose, over 400 matches were analysed in the U18 age category during the men’s and women’s European Championship (5v5 Basketball) and the Youth Olympic Games (3 × 3 Basketball). The research confirms some differences in the efficiency and structure of shooting. It can generally be said that male and female 3 × 3 basketball players take more long-distance shots (behind the 6.75 m line) and take fewer free throws than their counterparts in 5v5 basketball. When shooting from behind the 6.75 m line, both male and female 3 × 3 basketball players are less efficient than their 5v5 basketball counterparts, and similar applies to free throws. The reasons for these differences may entail many factors, including differences in the game rules, the playing conditions on outdoor courts, different technical and tactical requirements and the characteristics (style) of playing as well as the quality and performance level of basketball players.
