Introduction: diaphragmatic dysfunction is a common cause of slow weaning in mechanically ventilated patients. Diaphragmatic dysfunction in ventilated patients can be global or regional. The aim of our study was to evaluate the motion of the entire diaphragm in patients who were ventilated for a protracted period in comparison with healthy controls by using Magnetic Resonance Imaging (MRI). Methods: Intensive care patients who had a prolonged ventilator wean and required tracheostomies were enrolled based on extensive exclusion criteria. MRI dynamic sequence and subtraction images were used to measure vertical displacement at five different points on each hemi-diaphragm during normal tidal breathing. Tidal displacement of each point on the right and left hemi-diaphragms of the patients were compared to the precise respective points on the right and left hemi-diaphragms of enrolled controls. Results: Eight intensive care patients and eight controls were enrolled. There were observed significant differences in the displacements of the left hemi-diaphragm between the two groups (median 6.4 mm [Interquartile range (IQR), 4.6–12.5]) vs. 11.6 mm [IQR, 9.5–14.5], p = 0.02). There were also observed significant differences in the displacements at five evaluated study points on the left hemi-diaphragms of the patients when compared to the precise respective points in controls, especially at the dome (median 6.7 mm [IQR, 5.0–11.4] vs. 13.5 mm [IQR 11.5-18], p value = 0.005) and the anterior zone of apposition (median 5.0 mm [IQR, 3.3–7.1] vs. 7.8mm [IQR, 7.1–10.5], p value = 0.01). The intensive care patients showed lower minimal and maximal values of displacement of right hemi-diaphragms compared to the controls, suggesting that the differences in the displacement of right hemi-diaphragm are possible; however, the differences in the mean values of displacement of right hemi-diaphragm between the intensive care patient group and the control group (median 9.8 mm [IQR (Interquartile range), 5.0–12.3] vs. 10.1 mm [IQR 8.3–18.5], p = 0.12) did not reach the level of significance. Conclusion: Although frequently global, diaphragm dysfunction in ventilated patients after prolonged ventilation can also be regional or focal when assessed by MRI dynamic sequence. The vertical displacement of both right and left hemi-diaphragms at various anatomical locations had different values in both controls, and patients. There were significant focal variations in the movement of diaphragm in patients with ventilator-induced diaphragmatic dysfunction.

Sepsis represents a serious medical problem accounting for numerous deaths of critically ill patients in intensive care units (ICUs). An early, sensitive, and specific diagnosis is considered a key element for improving the outcome of sepsis patients. In addition to classical laboratory markers, ICU scoring systems and serum miRNAs are discussed as potential sepsis biomarkers. In the present prospective observational study, the suitability of miRNAs in sepsis diagnosis was tested based on proper validated and normalized data (i.e., absolute quantification by means of Droplet Digital PCR (ddPCR)) in direct comparison to classical sepsis markers and ICU scores within the same patient cohort. Therefore, blood samples of septic intensive care patients (n = 12) taken at day of admission at ICU were compared to non-septic intensive care patients (n = 12) and a healthy control group (n = 12). Our analysis indicates that all tested biomarkers have only a moderate informative power and do not allow an unequivocal differentiation between septic and non-septic ICU patients. In conclusion, there is no standalone laboratory parameter that enables a reliable diagnosis of sepsis. miRNAs are not superior to classical parameters in this respect. It seems recommendable to measure multiple parameters and scores and to interpret them with regard to the clinical presentation.

Abstract Background: Diaphragmatic dysfunction is a common condition in intensive care units (ICU). Its presence correlates with prolonged weaning from mechanical ventilation and mortality. Diaphragmatic excursion (EXdi) and thickening fraction (TFdi) are the 2 main measures currently described in diaphragmatic ultrasound, but each has its limitations. Strain and strain rate are already used cardiac sonography and could be of interest in the assessment of diaphragmatic function in ICU. The aim of this work was to evaluate the feasibility of diaphragmatic strain and strain rate in ICU and to describe their distribution, reproducibility and agreement with existing parameters. Methods: All patients who underwent a T-tube weaning test were prospectively included. Ultrasound loops were recorded on each side of the patient during the last 30 minutes of the weaning test. Two operators measured strain, strain rate, EXdi, and TFdi blind to each other in post-treatment analysis. Results: Thirty patients were analyzed. The median values for strain and strain rate were -6.74% and -0.23.s-1 on the left side and -8.17% and -0.22.s-1 on the right side. Concerning strain and strain rate, intra-class coefficients showed systematically a very good reliability between operators. Conclusion: Diaphragmatic strain and strain rate measurements appeared feasible in an ICU environment and seemed reproducible and not strongly correlated with EXdi and TFdi. An improvement of the analysis software is needed to improve the ease of interpretation. The interest of these parameters in clinical practice should be explored in forthcoming studies.

AbstractBackgroundAspiration is a serious side effect of delayed gastric emptying and increased residual volume in intensive care patients. Aspiration can increase the risk of pneumonia and death. Currently, pharmaceutical methods are the most commonly used techniques for decreasing aspiration, but non-pharmaceutical methods may also be effective in reducing aspiration. One of these methods is an abdominal massage. The present study aimed to examine the effect of abdominal massage on aspiration incidence in intensive care patients with an endotracheal tube.MethodsThis study was a randomized controlled clinical trial. Seventy intensive care patients with an endotracheal tube were chosen by convenience sampling and allocated to an intervention or a control group by the minimization method. The intervention group was provided with a 15-min abdominal massage twice a day for 3 days, while the control group received only routine cares. The patients’ respiratory secretion was suctioned before and after the intervention, and immediately after suctioning, the incidence of the aspiration was assessed using special strips which are sensitive to respiratory secretion pH and glucose. SPSS 18 was used to analyze data. Descriptive statistics were used. T independent, Mann-Whitney U, χ2, and Fisher exact tests were used to compare two groups.ResultsThe incidence rates of the aspiration were 5.7% (confidence interval: 0.0–14.3%) and 20% (confidence interval: 8.6–34.3%) in the abdominal massage and the control groups, respectively. The incidence of aspiration differed between two groups, but it was not statistically significant (p=0.07).ConclusionAlthough the low rate of aspiration in the abdominal massage group did not differ significantly with that of the control group, it can be clinically important. It is suggested that further studies be conducted to confirm the effect of abdominal massage on the prevention of aspiration in intensive care patients with an endotracheal tube.

AbstrakKangaroo mother care (KMC) merupakan metode merawat bayi beratbadan lahir rendah (BBLR). Beberapa intervensi perawatan di neonatal intensive care unit seperti pijat bayi, KMC, dan mendengarkan musik bermanfaat untuk pertumbuhan bayi berupa respons fisiologis BBLR dan mengurangi lama rawat. Penelitian ini bertujuan untuk mengetahui manfaat musik keroncong terhadap respons BBLR selama KMC dan lama rawat. Rancangan penelitian adalah quasi eksperimental dengan pretest dan posttest dengan desain grup kontrol. Pada Juli - September 2014 populasi penelitian adalah ibu dan bayi BBLR yang melaksanakan KMC. Pengambilan sampel dengan purposive sampling sebanyak 60 bayi. Kriteria inklusi bayi BBLR yang ditetapkan adalah berat badan bayi 1.500 – 2.499 gram, tanpa memandang usia kehamilan, bayi mampu menghisap walaupun masih lemah, tidak mengalami kesulitan pernapasan. Kriteriaeksklusi adalah bayi dengan kelainan kongenital, gejala sepsis, dan bayi yang dilakukan foto terapi. Uji statistik menggunakan uji-t berpasangan, ujit independen dengan nilai p < 0,05 dan CI 95%. Setelah perlakuan hari ketiga, terjadi penurunan nadi pada bayi dengan BBLR 8,13 kali/menit (nilai p = 0,000), respirasi penurunannya 2,36 kali/menit (nilai p = 0,000). Rerata lama rawat bayi pada kelompok perlakuan adalah 8,57 hari, sedangkan kelompok kontrol adalah 11,87 hari (nilai p = 0,038). Suhu hasilnya tidak bermakna (nilai p > 0,05). Dapat disimpulkan bahwa musik keroncong berpengaruh terhadap penurunan nadi, respirasi selama KMC, dan lama rawat bayi.AbstractKangaroo Mother Care (KMC) is nursing care method for low birthweight(LBW) infants. Some care interventions in neonatal intensive care unit, such as infant massage, KMC and listening to music have advantage for infant growth in form of physiological responses and reduce LBW infant-nursing length. This study aimed to determine advantage of keroncong music toward LBW infant’s response during KMC and nursing length. The study design was quasi experimental using pretest and posttest using control group design. Population was mothers and LBW infants implementing KMC. Samples were 60 infants taken by purposive sampling. Inclusion criteria determined for LBW infants were having weight 1,500 – 2,499 gram, without considering pregnancy age, having ability to suckle though still weak, not suffering breathing distress. Meanwhile, exclusion criteria were infants with congenital disorder, sepsis symptoms and infants during therapy photo. Statistical test used paired t-test, independent t-test with p value < 0.05 and confidence interval (CI) 95%. After third day of treatment, LBW pulse decreased 8.13 times/minute (p value = 0.000), respiration decreased 2.36 times/minute (p value = 0.000). Nursing length mean on the treatment group was 8.57 days, while the control group was 11.87 days (p value = 0.038). Temperature result was insignificant (p value > 0.05). In conclusion, keroncong music influences on decrease of pulse, respiration during KMC and length of infant nursing.

The activities of children more involve the foot. One of the common problems in the foot is flexible flatfoot. A disturbance in the process of the formation of the arch foot could result in a deformation of the foot and increases the risk of an injury due to postural balance change. Normally, the arch of the foot formed the first five years for the age range of 2-6 years. The right choices of the intrinsic muscle exercises of the foot can prevent deformation and improve postural balance. This study aimed to discuss the effectiveness of foot strengthening exercise to improving postural balance and functional ability of foot on a flexible flatfoot 6-10 years old. This study was pre-experimental research with two groups of pre-post test design. The subjects of this research were 30 students that had been divided into two groups, case, and control. Each group consists of 15 students selected used purposive sampling method based on the criteria of inclusion that had been set. There was a significant difference after foot strengthening exercise between the case and control group, p-value = 0,000. The foot strengthening exercise could improve the postural balance and functional ability of the foot on a flexible flatfoot 6-10 years old.

Sensitive period is the right time to instill the basis of the importance of counting. In addition to counting skills that must be developed also pay attention to the application of learning used. However, in the learning process, teachers often forget the importance of playing methods. If too often with giving assignments the child will feel bored, therefore what is used is the method of playing. Play method is one method that can be used by teachers to teach the ability to count children in the beginning. Based on this background, the purpose of this study was to determine the effect of the use of playing methods on initial numeracy skills. This study used the Quasi Experiment method with the Nonequevalent Control Group Design research design. The results of the pre test showed that there were no significant differences in the experimental group and the control group with p-value> 0.05, which was 0.135> 0.05 which was tested by the two average test results. While the post-test results showed that the initial numeracy ability after the application of the playing method experienced a significant difference with the p-value <0.05, that is with the results of 0.0001 with the Mann-Whitney test. This method of playing can be used as a learning suggestion for educators to improve their ability to have an early start.

ABSTRACTBackground: Vulva hygine is necessary to prevent pathological vaginal discharge. The right strategy to prevent vaginal discharge is to provide Vulva hygine education. The study was conducted on April 1, 2020 for 1 week.Research methods: Using a quasi-experimental research method. This type of research is the pre and post test control group design with a sample of 65 respondents, using a questionnaire measuring instrument and using the Wilcoxon test for knowledge and attitudes and the T test for action.Results: statistical tests on knowledge and attitudes using the Wilcoxon test, from the results of these tests obtained results for knowledge and attitudes P value + 0.000 <α 0.05, and for action using the Paired sample T-test statistical test and the results obtained P value = 0.000 <α 0.05. This means that from the three domains above Ha is accepted.Conclusion: There is an effect of education on the knowledge, attitudes and actions of students at the risk of pathological vaginal discharge. BackgroundThe oral mucosa is an important defense barrier to penetration of microorganisms. Thus, changes in the oral epithelium might indicate risk for infection in intensive care patients receiving mechanical ventilation.ObjectiveTo evaluate the oral mucosa of intensive care patients who did or did not receive mechanical ventilation by using liquid-base exfoliative cytology.MethodsThe sample consisted of 3 groups: 27 patients admitted to intensive care during a 7- to 14-day period who received mechanical ventilation, 29 patients admitted during the same period who did not receive mechanical ventilation, and 27 healthy patients who had no lesions in the mouth. For all 3 groups, samples were collected from the buccal mucosa by using cytology brushes. Smears were applied to glass slides before Papanicolaou staining and were codified for blind analyses by a cytopathologist. Kruskal-Wallis and Dunn tests were used to analyze the results.ResultsPatients receiving mechanical ventilation had higher prevalence and intensity of karyomegaly, perinuclear halos, cell keratinization, deep cells, and leukocyte infiltrates than did patients in the other 2 groups (P &lt; .05). No significant differences were observed between the control group and the group who did not receive mechanical ventilation.ConclusionsLiquid-base exfoliative cytology can be used to detect preclinical alterations in the oral mucosa. Patients treated with mechanical ventilation are vulnerable to infections, and oral care may be valuable in their prognosis.

Background: Medroxyprogesterone and donepezil could be used as respiratory stimulants in ventilated patients. However, no randomized placebo-controlled trial is available to confirm this approach and compare these drugs. The aim of the current study was to evaluate the effects of donepezil or medroxyprogesterone compared to the placebo in improvement in respiratory status and weaning facilitation in critically ill adult patients receiving mechanical ventilation.Material and Methods: This randomized, triple-blind trial was conducted on 78 ventilated patients in intensive care units (ICU). Patients who were intubated due to pulmonary disorders were ruled out. Patients were randomized in a 1:1:1 ratio to receive 5 mg donepezil (n = 23) or 5 mg medroxyprogesterone (n = 26), or placebo (n = 24) twice a day until weaning (maximum 10 days). The primary endpoints were weaning duration, and duration of invasive mechanical ventilation. Secondary endpoints included rate of successful weaning, changes in arterial blood gas (ABG) parameters, GCS and sequential organ failure assessment (SOFA) score, hemoglobin (Hgb), ICU-mortality, and duration of ICU stay, were measured before and after the intervention and if successful weaning was recorded.Results: Of 78 studied patients who were randomized, 59 weaned successfully. 87% patients in donepezil and 88.5% patients in medroxyprogesterone groups were successfully weaned compared to 66.7% patients in the placebo group. However, this difference was not statistically significant (p-Value = 0.111). Changes in pH, mean duration of intubation, and weaning duration were statistically different in donepezil compared with the control group (p-Value &lt; 0.05). No significant difference in ABG, Hgb, GCS and SOFA score, and duration of intubation were seen in the medroxyprogesterone group, but weaning duration was significantly reduced to 1.429 days compared with the control group (p-Value = 0.038).Conclusion: The results of this clinical trial have demonstrated that the administered dose of medroxyprogesterone and donepezil can expedite the weaning process by reducing the weaning duration compared to placebo. Furthermore, the total duration of invasive ventilation was significantly lower in the donepezil group compared to the control group. Future clinical trials with a larger sample size will determine the exact role of medroxyprogesterone and donepezil in mechanically ventilated patients.Clinical Trial Registration:https://irct.ir/IRCT20190810044500N2 (April 1, 2020).

Background: In addition to the influence of diabetes on male reproduction lowers testosterone levels of FSH and LH, the second number of hormones is reduced will interfere with the process of spermatogenesis. If the spermatogenesis process is interrupted then the number of germ cells decreases, this will cause a decrease in the weight of the testes. Objective: To analyze the effect of extract grass administration to the weight of the testicles diabetes Mellitus mice between the treatment group and the control. Methods: The research used DM male mice which is divided into 5 groups each consisting of 6 mice. Day 1 till day 5 all STZ-induced groups. On the 11th day to the 45th day, the control group (K +) was given metformin 2mg/head/day. The control group (K-) was given CMC-Na 1%. Group P1, P2 and P3 was given Kebar grass with dose of 67, 5mg/day, 135mg/kg/day, and 270mg/kg/day and metformin 2mg/head/day. The 46th was done with the release and sampling of the right and left testicles. Testicular weighing used analytic scales 0.001gram. Results: Statistical tests showed that there was a discrepancy in the p-value = 0.000 (0.05) on testicular weight. Conclusion: Increased testicular weight model of diabetes mellitus that gets the grass extractions of Kebar. Proses spermatogenesis terganggu maka jumlah sel germinal mengalami penurunan, hal ini akan menyababkan berat testis menurun. Tujuan: menganalisisi pengaruh pemberian ekstrak rumput kebar terhadap berat testis mencit model diabetes mellitus antara kelompok perlakuan dan kontrol. Metode: penelitian menggunakan mencit DM yang dibagi menjadi lima kelompok setiap kelompok terdiri dari 6 ekor mencit. Hari ke 1 sampai hari ke 5 semua kelompok diinduksi STZ. Hari ke 11 sampai hari ke 45 kelompok kontrol (K+) disonde metformin 2mg/ekor/hari. Kelompok kontrol (K-) disonde CMC Na 1%. Kelompok P1,P2 dan P3 disonde ekstrak rumput kebar dengan dosis, 67,5mg/kgBB/hari, 135mg/kgBB/hari, 270mg/kgBB/hari dan metformin 2mg/ekor/hari. Hari ke 46 dilakukan pembedahan dan pengambilan sampel testis kanan dan kiri dan Penimbangan berat testis mencit menggunakan timbangan analitik dengan skala 0,001 gram. Hasil: uji statistik menunjukkan terdapat perbedaan bermakna dengan nilai p= 0,000 (p<0,05) pada berat testis mencit. Kesimpulan: berat testis meningkat dibandingkan kelompok kontrol Kata kunci: ekstrak rumput kebar; berat testis; diabetes mellitus
