Background: The objective of this micro-computed tomography (micro-CT)-based study was to compare the filling quality of endodontic treatment and endodontic Re-treatment between two sealers with matched obturation techniques: calcium silicate-based sealer (Endoseal TCS) with a single-cone technique (SCT) and resin-based sealer (AH Plus) with a continuous wave technique (CWT). Methods: Forty maxillary premolars were selected and assigned into four groups, according to the obturation methods of the first endodontic treatment and Re-treatment (n = 10). The AP/AP group was first treated with AH Plus/CWT, then re-treated with AH Plus/CWT. The AP/ET group was first treated with AH Plus/CWT, then re-treated with Endoseal TCS/SCT. The ET/AP group was first treated with Endoseal TCS/SCT, then re-treated with AH Plus/CWT, and the ET/ET group was first treated with Endoseal TCS/SCT, then re-treated with Endoseal TCS/SCT. The specimens were scanned using micro-CT at three time points: after the first endodontic treatment, after gutta-percha (GP) cone removal, and after Re-treatment. The void volume of root canal obturation and the volume of the remaining filling materials were calculated. Data were analyzed using Student’s t-tests and ANOVA. Results: The Endoseal TCS groups (ET/AP and ET/ET) showed a lower percentage of voids than the AH plus groups (AP/AP and AP/ET) on the whole canal and the apical third, after first obturation (p < 0.05). The AH plus group showed significantly fewer remnants than the Endoseal TCS group after GP removal (p < 0.05). Re-treated canals and initially treated canals had similar void volumes (p > 0.05). There was no significant difference in void volume after Re-treatment, regardless of whether the same or different sealers were used for the first treatment and Re-treatment (p < 0.05). Conclusions: Endoseal TCS sealer and AH Plus sealer had a similar Re-treatment efficacy, regardless of which sealer was used in the previous treatment.

ABSTRACT Objective: Aim of the present study was to evaluate the retreatability and reestablishment of apical patency of two calcium silicate-based sealers, TotalFill BC Sealer (BCS) and mineral trioxide aggregate Fillapex (MTA F), versus AH Plus, when used in combination with Gutta-percha (GP). Materials and Methods: The canals of 54 single-rooted anterior teeth were instrumented and filled with GP/AH Plus (Group A), GP/MTA F (Group B), or GP/BCS (Group C) using continuous wave obturation technique. The groups were subdivided into subgroups with the master-GP cone placed to the working length (WL) or intentionally 2 mm short. The retreatment procedures were performed using ultrasonics, chloroform, rotary, and hand files. The ability to establish the patency and reach WL was determined as well as the time taken to reach WL was calculated in minutes. Furthermore, the samples were observed under a dental, optical microscope, after vertically splitting them. Results: The WL and patency were reestablished in 100% of specimens in all groups. The Mann–Whitney U-test indicated that there was a significant difference in the amount of time required to reach WL between the groups (P < 0.05) with group GP/BCS short of the WL showing the most amount of time to be retreated. Conclusion: The novel calcium silicate-based sealers are negotiable under simple root canal anatomy. However, the conventional retreatment techniques are not able to fully remove them.

Aim: To evaluate flow, filling ability, and apical extrusion of three calcium silicate-based sealers, Neo MTA Plus (Avalon, USA), Bio-C Sealer (Angelus, Brazil) and Sealer Plus BC (MK Life, Brazil), in comparation with the gold standard based on epoxy resin, AH Plus (Dentsply, Germany). Methodology: The flow was evaluated based on ISO 6876/2012 standard (mm) and in area (mm2 ). For assessment of filling ability and apical extrusion of the sealers, curved artificial canals were prepared up to size 25.06, and filled with the sealers by the single cone technique. The samples were scanned in microcomputed tomography (SkyScan 1272. Bruker, Belgium) after preparation and after filling of the root canals. The percentage of voids throughout the entire extension of the root canals and in the apical third, besides the volume of apical extrusion of each sealer were calculated. Data were statistically analyzed using ANOVA/Tukey tests (α = 0.05). Results: Bio-C Sealer showed the highest flow and NeoMTA Plus had the lowest value (p < 0.05). Bio-C Sealer had lower percentage of total voids than NeoMTA Plus (p < 0.05). AH Plus had greater percentage of voids in the apical third than Bio-C Sealer and Sealer Plus BC (p < 0.05). Bio-C Sealer showed higher volume of extrusion (p < 0.05) than AH Plus and NeoMTA Plus. Conclusion: Bio-C Sealer and Sealer Plus BC had greater flow and proper filling ability in the apical third. However, these sealers presented high volume of apical extrusion. NeoMTA Plus provided less sealer extrusion, low flow, and more presence of voids.

Abstract Background The present study aimed to compare the volumetric changes of three calcium silicate cements after retrofilling and placing under different pH conditions via micro-computed tomography (micro-CT) scan. Methods Forty-two extracted human single-rooted teeth were randomly assigned to three groups according to the retrofilling materials used (Biodentine, Endocem MTA, and ProRoot MTA). Each group was divided into two subgroups according to the setting condition. The teeth in one group were immersed in normal saline for 5 days at room temperature, and the teeth in the other group were immersed in butyric acid (pH = 5.4) for 5 days at room temperature. The volume ratios of the retrofilling material were calculated via micro-CT imaging. Results The volume ratios of the Biodentine and Endocem MTA groups were significantly different between the two setting environment, and these groups had significantly lower filled volume ratio (Vf, %) in the acidic environment than in the saline environment (pH = 5.4). Meanwhile, the volume ratio of the ProRoot MTA group did not significantly differ between the two setting environments. All materials under the acidic setting condition had relative radiolucency in the area in contact with the acidic solution. Conclusion The Vf ratio of the Biodentine and Endocem MTA cements was significantly lower in the acidic environment than in the saline environment. No statistically significant difference was observed in the Vf ratio of ProRoot MTA between the two setting environments.

Background: This clinical trial aimed to compare the effects of bioceramic sealer and resin-based sealer on the incidence and intensity of postoperative pain. Methods: Patients with anterior teeth or premolars requiring root canal treatment were assigned to group 1 (n = 51). Those with molars requiring treatment were assigned to group 2 (n = 57). In groups 1En and 2En, root canals were obturated with Endoseal MTA using the single-cone technique. In groups 1AH and 2AH, the sealer used was AH Plus with the continuous wave technique. On the day of canal filling, each patient was instructed to indicate their pain intensity over the 7 day postoperative period, at rest and, while biting, using a visual analog scale. Results: There was no significant difference in the incidence or intensity of postoperative pain between the Endoseal MTA and AH Plus groups during the 7 day postoperative period (p > 0.05). Less time was needed to seal the root canals with Endoseal MTA, especially in group 2 (p < 0.05). Conclusions: Endoseal MTA and AH Plus had similar effects on the incidence and intensity of postoperative pain. The obturation time was shorter when using Endoseal MTA compared to AH Plus.

Purpose: This study compared the antimicrobial efficacy of calcium silicate sealers (BioRoot RCS and Total Fill BC) and conventional sealers (AH Plus and Tubli-seal) against planktonic bacteria and a nutrient-stressed multispecies biofilm. Methods: Antimicrobial properties of freshly mixed sealers were investigated using the direct contact test (DCT) and a nutrient-stressed multispecies biofilm comprised of five endodontic strains. Antimicrobial activity was determined using quantitative viable counts and confocal laser scanning microscopy (CLSM) analysis with live/dead staining. The pH of the sealers was analysed over a period of 28 days in Hanks Balanced Salt Solution (HBSS). Analysis of variance (ANOVA) with Tukey tests and the Kruskal–Wallis test were used for data analysis with a significance of 5%. Results: All endodontic sealers exhibited significant antimicrobial activity against planktonic bacteria (p < 0.05). BioRoot RCS caused a significant reduction in viable counts of the biofilms compared to AH Plus and the control (p < 0.05), while no significant difference could be observed compared to TotalFill BC and Tubli-seal (p > 0.05). CLSM analysis showed that BioRoot RCS and TotalFill BC exhibited significant biofilm inhibition compared to Tubli-seal, AH Plus and the control (p < 0.05). BioRoot RCS presented with the highest microbial killing, followed by TotalFill BC and Tubli-seal. Alkalizing activity was seen from the onset by BioRoot RCS, TotalFill BC and AH Plus. After 28 days, BioRoot RCS demonstrated the highest pH in HBSS (pH > 12). Conclusions: Calcium silicate sealers exhibited effective antimicrobial properties. This was demonstrated by superior biofilm inhibition capacity and microbial killing, with strong alkalizing activity compared to epoxy-based and zinc oxide-eugenol-based sealers.

The aim of this study was to quantify the residual filling material after filling removal, re-preparation with rotary or reciprocating files and passive ultrasonic irrigation (PUI). Twenty maxillary molars were prepared using ProTaper instruments up to F1. The teeth were filled with AH Plus and ProTaper gutta-percha points using the single-cone technique. Thereafter, the specimens were scanned using a micro-computed tomography system (Micro-CT #1). Then, the root canal filling was removed using ProTaper Retreatment files, and a new scan was performed (Micro-CT #2). The specimens were divided into two groups according to the instrument used for re-preparation: ProTaper rotary or WaveOne reciprocating files (Micro-CT #3). Finally, PUI was performed, and a new micro-CT scan was performed (Micro-CT #4). Intragroup and intergroup analyses were performed using Friedman and Dunn's post hoc test and the Kruskal-Wallis and Dunn post hoc tests, respectively. Palatal canal presented the highest volume of residual filling material in all stages of endodontic retreatment (p<0.05). The main reduction of filling volume was achieved after using ProTaper Retreament (p<0.05). The amount of remaining filling material after using ProTaper Retreatment was similar to that achieved with rotary and reciprocating files and after PUI (p>0.05). Rotary and reciprocating files achieved similar removal of the root canal filling (p>0.05). The greatest reduction in filling material was achieved after using ProTaper Retreatment files. Rotary and reciprocating instruments and PUI did not improve the removal of root canal filling materials.

Abstract Objective This study compared the ProTaper Next (PTN; Dentsply Sirona, Tulsa, Oklahoma, United States) and HyFlex EDM (HEDM; Coltene/Whaledent AG, Alstätten, Switzerland) systems using micro–computed tomography (CT). Materials and Methods Twenty-one mesial roots classified as Vertucci's type IV from extracted mandibular first molars with curvatures between 20 and 40 degrees were selected. The teeth were scanned using a micro-CT before and after root canal preparation by both systems, applied to the same root, in alternating canals. The following parameters were analyzed: canal centering, apical transportation, root canal diameter/root diameter. Results No statistically significant differences between both systems were observed for any of the assessed morphological parameters (p > 0.05). All canals presented diameter enlargement of more than 40% in relation to root diameter in the cervical and middle segments. No statistically significant difference was noted between the HEDM and PTN groups. The wear percentage for the HEDM group in the cervical and middle thirds were 49.66 ± 8.65 and 46.48 ± 14.29, respectively, and 51.02 ± 11.81 and 45.48 ± 10.79 for the PTN group, respectively. Conclusion Both systems displayed similar mandibular molar mesial canals preparation, with no differences noted for any of the assessed parameters. Both groups showed increased canal diameter in the cervical and middle thirds by more than 40%.

Background: The aim of this study was to compare the success rate of root canal treatments undertaken using a calcium silicate root canal sealer in combination with a single cone with non-calcium silicate cement and warm vertical condensation. Methods: 150 necrotic or pulpitic teeth were treated. (REC: 08/H0804/79). Following standardized root canal chemo-debridement. The canals were obturated using warm vertical condensation of gutta-percha and epoxy-based sealer (AH plus) or a calcium silicate sealer (BioRootTM RCS) with a single cone technique. Follow-up assessment was conducted at 12 months using Cone Beam Computed Tomography (CBCT). Results: At 1-year recall, 104 teeth were assessed (51 AH plus, 53 BioRootTM RCS). The success rate using loose criteria for the CBCT images and PA radiographs was respectively 80% and 89% in the AH plus/warm vertical condensation group, 84% and 90% in the BioRootTM RCS/single cone group. There was no statistically significant difference between the two groups (Fisher exact test p value 0.6099 for the CBCT images). Conclusion: Within the limitations of this non-randomized trial, a calcium silicate cement in combination with single cone resulted in a similar proportion of successful cases compared to warm vertical condensation and epoxy-based sealer.

The aim of this study was to evaluate the influence of calcium-silicate-based sealer (Endosequence-BC-Sealer) in roots, filled with thermo-plasticized injectable technique aided by Calamus-Flow-Delivery-System, on bond strength to radicular dentin, in comparison with conventional epoxy-resin-based sealer (AH-Plus) along with cold-lateral-compaction technique. Root canals of mandibular-premolar teeth (n = 80) were instrumented using Protaper Universal rotary files and were randomly divided into four experimental groups (n = 20) as follows: (1) AH-Plus + cold-lateral-compaction technique; (2) Endosequence-BC-Sealer + cold-lateral-compaction technique; (3) AH-Plus + thermo-plasticized injectable technique; and (4) Endosequence-BC-Sealer + thermo-plasticized injectable technique. Horizontal disc shaped samples from each group (n = 60/group) were obtained and push-out bond strength testing was performed at a cross-head speed of 0.5 mm/min. Data were analyzed statistically using nonparametric Kruskal-Wallis analysis and Mann-Whitney test (p < 0.001). The statistical analysis revealed a significant difference amongst the groups (p < 0.001). The highest bond strength values were found in group 1 compared with all the other experimental groups (p < 0.001), whereas the lowest bond strength values were found in group 4 (p < 0.001). It was concluded that thermo-plasticized injectable technique with Calamus-Flow-Delivery-System lowered the bond strengths of the sealers, especially Endosequence-BC-Sealer. Therefore, this technique is not recommended to calcium-silicate-based sealers. Further studies are needed to confirm the findings of this study.

Abstract Background Histology is considered as a gold standard for analyzing bone architecture. However, histomorphometry is a destructive method and only offers the bone information of a limited location. Micro-computed tomography (μCT) is a non-destructive technology and provides a slice at any site. The aim of this study was to compare the correlation of the Bone-to-Implant Contact ratio (BIC) between 2D micro-CT (μCT) and histomorphometry and to investigate a method for assessing the osseointegration of the implant by 2D μCT. Methods A total of 18 implants were divided into three groups (6 implants per group), and inserted into the rabbit tibia defects as follow: implant only (Implant group), implant with β-TCP/hydrogel (TCP group), implant with rhBMP-2 loaded β-TCP/hydrogel composite (BMP-2 group). After 4 weeks of implantation, the specimens were collected to take the micro-CT scan with an aluminum filter and performed H&E staining on the undecalcified sections. The 2D μCT slices were chosen at an angle of 0°, 45°, 90° and 135° with the representative histological section to measure BIC. And the correlations between BICs of 2D μCT and BICs of histology were evaluated. Results In each group, BICs at the same sites measured by histomorphometry and corresponding 2D μCT presented the same trend and shown no significant difference between the two methods (P > 0.05). BICs of histological sections and BICs of corresponding 2D μCT slices presented a strong correlation in the implant group (γ = 0.74, P = 0.09), a moderate correlation in the TCP group (γ = 0.46, P = 0.35), a weak correlation in the BMP-2 group (γ = 0.30, P = 0.56). In the implant group, the relationship between BIC-Mean-μCTs and BICs-Histology has presented a significant linear correlation (γ = 0.84, P = 0.04). Conclusions Integrating bone information of several 2D μCT slices in different sites to measure BIC is a feasible method for assessing the implant osseointegration.
