This study investigated which preparation strategy for root canals leads to the best technical preparation quality, and moreover, which is perceived to be performed best by novice students. Sixty-four students were recruited to prepare one simulated root canal with each of the following: FlexMaster files (F), Mtwo files (M), and Reciproc files (R). After preparation, the students assessed the different instrument systems through a questionnaire. The technical quality of the root canal preparations was evaluated by the centering ratio of the preparation. A total of 186 prepared root canals were submitted for evaluation. With R, significantly better centered preparations were achieved when compared to M and F (p < 0.001). The students evaluated R faster than M and F, and evaluated F significantly (p < 0.05) slower than R and M. M was rated as the easiest system to learn and to handle, as well as the best at reaching the working length; therefore, it was evaluated as the overall favorite of the students. A difference was found between the students’ perceptions and their achieved technical quality of root canal preparations.

ABSTRACTObjectives: The purpose of this study was to evaluate the radiographic quality of root canal fillings performed by first and second clinical year dental students at the Faculty of Dentistry in Süleyman Demirel University, Turkey. Methods: The technical quality of 833 root canal fillings was evaluated by three examiners according to the density of the filling and the distance between the end of the filling and the radiographic apex. A root canal with acceptable filling length and homogeneous root filling was defined as being good quality endodontic work (GQEW). A treated tooth was defined as having good quality endodontic work tooth (GQEW-T) when all its canals had a GQEW rating. Results: A Chi-square (χ2) test at the 0.05 significance level was used for the statistical analysis of the data. Of a total of 833 root canals, 662 (79.47%) root fillings were classed as GQEW. Two hundred fifty canals (73.4%) (135 teeth) treated by first clinical year dental students were defined as GQEW-T while 412 canals (204 teeth) (73.9%) treated by second clinical year dental students were defined as GQEW-T (P>.05). Most of the GQEW-T were defined in anterior teeth (90.1%), whereas the fewest were in molar teeth (46.6%) for both first and second clinical year dental students (P<.001). Conclusions: The quality of root canal fillings in anterior teeth performed by undergraduate dental students in Isparta, Turkey was satisfactory. However, to improve the success with molar teeth, education about newer techniques and instruments must be incorporated into the preclinical and clinical curriculum. (Eur J Dent 2011;5:324-330)

Abstract Background To investigate the efficacy of a nickel-titanium (NiTi) file with an automated computerized numerical control (CNC) system for root canal shaping. Methods The movement of the automated device and the insertion angle were investigated. In Experiment 1, simulated resin root canals were randomly divided into four groups (n = 20): manual downward movement using a handpiece (Group 1), vertical downward movement by CNC (Group 2), reciprocating up and down movement by CNC (Group 3), and spiral up and down movement by CNC (Group 4). In Experiment 2, five different insertion angles of the NiTi file were evaluated (n = 20). Four parameters were used to evaluate the shaping ability: change in the working length, central axis offset, curvature variation, and preparation time. Groups were compared using one-way analysis of variance (ANOVA) with significance was set at P < 0.05. Results The change in central axis position in the curved part of the root canal was found to be smaller in Group 4 than in other groups (P < 0.05). The curvature changes and preparation time of Groups 1 and 4 were significantly reduced compared with Groups 2 and 3 (P < 0.05). The variation in working length and curvature in the 5° insertion angle group was significantly smaller than in the other groups (P < 0.05). Conclusions A spiral up and down movement, controlled by the CNC machine, and 5° insertion angle, maintained original root canal shape more precisely than other methods.

Background: Effective root canal treatment involves cleaning and shaping root canals using a range of instrumentation procedures and techniques. The aim of this review was to provide an update on root canal hand instrumentation techniques and how to overcome iatrogenic errors. Methods: A comprehensive search was carried out using multiple databases, PubMed, Medline, Google scholar, and Scopus, to identify relevant studies addressing the objective of this study and to summarize the current evidence to readers. Keywords, i.e., endodontics, root canal preparation, hand instrumentation, techniques and procedural errors, as well as, MeSH terms, i.e. “endodontics”, “instrumentation”, “root canal preparation” were used to conduct our comprehensive search. Results: The hybrid technique is a commonly-used approach that combines the benefits of both the crown-down and step-back techniques and was reported to be one of the best techniques to produce an optimal root canal preparation outcome. Hand-instrument manipulation using the balanced forced technique is also favoured as it rapidly and safely permits removal of canal contents allowing irrigants and medications to reach deep inside canal spaces. Conclusion: The use of hand files is still recommended for initial canal negotiation and preparation and is essential for the correction of procedural errors. This study provided an update on several hand instrumentation techniques commonly used in endodontics. The correct and safe application of these techniques can prevent iatrogenic procedural errors from occurring which optimizes the quality of treatment outcome.

Aim. The aim of this study was to evaluate the radiographic technical quality of endodontic treatment performed by undergraduate students at the School of Dentistry, Faculty of Medicine, University of Novi Sad, Serbia.Materials and Methods. Electronic records of 220 patients treated by final-year undergraduate students during the school year 2011/2012 were examined, and the final sample consisted of 212 patients, 322 teeth, and 565 root canals. The criteria for overall radiographic adequacy of root canal fillings were defined as the presence of adequate length and density and absence of iatrogenic errors (ledge, fractured instrument, untreated canal, and apical transportation). Chi-square test was used to determine statistical significance between different parameters.Results. Adequate root canal fillings were found in 74.22% of the teeth. The percentage of root fillings with adequate length and density was 89.73% and 92.6%, respectively. Fractured instruments and ledges were present in 16 root canals (2.8%), while the presence of missed canal and apical transportation was observed in 2 cases, each (0.3%).Conclusions. Overall, the technical quality of root canal fillings performed by undergraduate students was satisfactory.

SUMMARYAim: To evaluate technical quality of root canal treatments performed in extracted anterior single-rooted teeth in an undergraduate dental clinic, and to record the associated iatrogenic errors.Material and Method: 287 root canal treatments performed by 114 preclinical undergraduate students in extracted anterior teeth at the Department of Endodontology were radiographically evaluated, based on the following parameters: access cavity dimensions and direction, root canal instrumentation, and root canal filling quality.Results: The access cavity was found of adequate dimensions in 28.9% and straight in direction in 71.1% of the material. Erroneous position, overextension of the access cavity and non-straight access were noted in 39.7 %, 21.6% and 28.9% of the cases, respectively. The frequency of ledged root canals was 10.1%, while 84.7% of the teeth had no instrumentation related iatrogenic errors. Regarding root canal filling, incomplete density apically and technical inadequacies in the middle and cervical thirds were observed in 76.3% and 82.3% of the cases. The percentage of flawless endodontic treatments was 3.1%.Conclusions: Most of the endodontic treatments evaluated were classified unacceptable. Inadequate root canal filling density in the middle and cervical third and ledge formation were the most common faults.

SummaryBackground/Aim: To investigate the incidence of procedural errors with the use of a novel nickel-titanium rotary system (Hyflex CM, Coltene/Whaledent, Altstätten Switzerland), evaluate the technical quality of root canal treatments and assess a questionnaire completed by the participants themselves in an undergraduate dental clinic between 2014 and 2017 (Department of Endodontology, School of Dentistry, Aristotle University of Thessaloniki).Material and Methods: 118 undergraduate students in their first year of clinical practice performed a root canal treatment on a patient’s molar (maxillary/mandibular). None of the participants had previous experience in rotary instrumentation. The periapical radiographs were taken with the use of the paralleling technique for standardization and were collected and evaluated by the investigator. After the root canal treatment was performed the students completed a questionnaire in order to evaluate their training on rotary instrumentation.Results: The overall incidence of instrument separation, apical perforation, root perforation, straightening and ledges was 0.8%, 4.4%, 2.3%, 5.5% and 29% respectively on root canal level. Ledges were detected more often in mandibular mesiobuccal canals. The frequency of root canals with an ‘acceptable’ filling was 68.4%, while overfilled and underfilled canals were found to be 8.6% and 16.2% respectively. The response rate was high (94.9%), 35% of the participants encountered no difficulty in the use of rotary instrumentation and 98.2% would use it again.Conclusions: The incidence of procedural errors was considerably low and the technical quality of the filled root canals was superior to that of similar studies. The responses of the questionnaire demonstrated a positive attitude toward rotary instrumentation.

The aim of this study was to investigate the shaping ability and cyclic fatigue resistance of Genius (GN) and WaveOne (WO) nickel-titanium (NiTi) instruments. Forty mesial root canals of mandibular molars were randomly divided into two groups, according to the instrument system used for root canal preparation (n = 20): GN and WO. Root canals were prepared to the full working length using the crown-down technique, with up to 40.04 instruments for GN group and 40.08 instruments for WO group. Specimens were scanned with a µCT before and after instrumentation. The changes in 3D parameters and degree of canal transportation were evaluated in the apical, middle and coronal thirds of root canals. Size 25, taper 0.04 (GN) and 0.08 (WO) instruments (n = 20) were used in simulated root canals until fracturing, and the number of cycles to failure (NCF) was recorded. The fractured files were investigated under a scanning electron microscope to characterize fractured surfaces. Data were analyzed using ANOVA post hoc Tuckey’s tests with the significance level set at 5%. The GN system was able to prepare the higher percentage of the canal surface with less canal transportation in all root canal thirds and displayed the higher resistance to cyclic fatigue in comparison to WO instruments (p < 0.05). However, the length of the fractured segments was not significantly different among the groups; meanwhile, SEM investigations revealed the typical topographic characteristics of fractured surfaces. Hybrid GN instruments demonstrated superior shaping ability and resistance to cyclic fatigue. However, both NiTi instrument systems prepared the root canals with no significant shaping errors, considerable changes in 3D parameters and no significant differences in the topographic appearances of the instrument surfaces after fractures.

INTRODUCTION Taurodontism is a morphoanatomical change in the shape of a tooth. An enlarged body of a tooth with smaller than usual roots is a characteristic feature. Internal tooth anatomy correlates with this appearance, which means that a taurodontal tooth has a large pulp chamber and apically positioned furcations. This dental anomaly may be associated with different syndromes and congenital discoders. CASE OUTLINE The case report presents the patient of a rare case of taurodontism in the mandibular second premolar with chronic periodontitis. Endodontic treatment was performed after dental history and clinical examination. Special care is required in all segments of endodontic treatment of a taurodontal tooth from the identification orifice, canal exploration, determining working length, cleaning and shaping and obturation of the root canal. Precurved K-file was used for canal exploration and location of the furcation. One mesial and one distal canal with the buccal position were identified in the apical third of the root canal. The working lengths of two canals were determined by radiographic interpretation with two K-files in each canal and verified with the apex locator. During canal instrumentation, the third canal was located in the disto-lingual position. The working length of the third canal was established using the apex locator. CONCLUSION Thorough knowledge of tooth anatomy and its variations can lead to lower percentage of endodontic failure. Each clinical case involving these teeth should be investigated carefully, clinically and radiographically to detect additional root canals. High quality radiographs from different angles and proper instrumentarium improve the quality of endodontic procedure.

Objective: The aim of the study was to assessment of technical quality of 9562 endodontic cases treated by heterogeneous groups with different clinical experience. Material and methods: This retrospective study reviewed the dental records of 8590 patients (9562 endodontic cases and 13203 root canals including 3340 retreatment root canals) treated by fourth-year undergraduates, fifth-year undergraduates, endodontic program students, and endodontic specialists between December 2017 and December 2018 at the Department of Endodontics of Necmettin Erbakan University Faculty of Dentistry in Konya. The length, density, and taper of root fillings, the acceptable technical quality of the root filling criteria, and the presence of procedural errors, were recorded. Chi-square tests with a significance level at p=0.05 were used for statistical analysis. Results: Acceptable root fillings were found in 71.5% of endodontic cases. Clinical experience showed statistical differences in the root filling quality among fourth-year undergraduates, fifth-year undergraduates, endodontic program students, and specialists (52, 63.1, 77.9 and 86.5% respectively). Fractured instruments and missed canals were noted in 9.8% and 0.57% of cases treated by endodontic program students significantly more than the other clinical experiences. No statistical differences were found among the clinical experiences in other procedural errors. No statistically significant differences were found between age range and gender in additional root canals. Statistically significant differences were observed between the number of root canal retreatments in terms of age range and gender. Conclusions: Clinical experience affects the technical quality of root canal treatments. Fractured instruments and missed canals, especially by endodontic program students, should be given more attention. KEYWORDS Dental radiography; Education; Undergraduate.

Abstract Objectives The aim of this study was to compare two teaching protocols according to the technical quality of root canal therapy (RCT) and the procedural errors occurred in preclinical training. Materials and Methods Two different groups of students were concerned. The first one (G1) performed a crown-down technique to shape the root canal systems and cold lateral condensation technique to fill them. The second one (G2) performed a step-down technique without initial manual scouting to shape the root canal systems, and cold hydraulic condensation technique, to fill them. G2 used clinical operative microscope to check the access cavity preparation. Statistical Analysis The quality of RCTs and procedural errors were recorded and analyzed using chi-squared test and t-test. Results Four hundred sixty-eight root canals from 152 maxillary molars were treated by the G1 students: 46.6% canals were judged as acceptable. Four hundred sixty-nine root canals from 152 mandibular molars were treated by G1: 58.8% canals were judged as acceptable. Five hundred fifteen root canals from 156 maxillary molars were treated by G2 students: 84.1% canals were judged as acceptable. Four hundred ninety-three root canals from 156 mandibular molars were treated by G2: 90.9% canals were judged as acceptable. Among the errors, the incidence of “ledges” and “fractured instruments” was statistically significant in G1 compared with G2, both on maxillary and on mandibular molars. Conclusions The molar RCTs performed by G2, who got benefit from the new teaching protocol, resulted in a better quality of root filling and in fewer procedural errors compared with the molar RCTs performed by G1.
