Penelitian ini memiliki tujuan yaitu mengetahui nilai risiko potensi bahaya kerja dan level risiko potensi bahaya kerja serta mengetahui potensi bahaya kerja dominan yang menyebabkan terjadinya kecelakaan kerja pada proyek Instalasi Vertical Drain dan Geotextile di Kawasan industri Kendal. Penelitian ini menggunakan pendekatan metode HIRA (Hazard Identification and Risk Assesment) dan FTA (Fault Tree Analysis). Hasil dari analisis dengan metode FTA didapat potensi bahaya kerja pada proyek tersebut yang dominan adalah melempar, melompat, memanjat, berlari, bergurau (X5), membetulkan mesin pada keadaan mesin masih berjalan (X11), jalan akses tidak bersih dan tidak rata (X15), membetulkan mesin pada keadaan mesin masih berjalan (X11), jalan akses tidak bersih dan tidak rata (X15), dan lokasi kerja licin (X17) dengan penyebabnya adalah terpeleset, terjatuh, terkena benda tajam (P2) dan menabrak atau tertabraknya pekerja dengan objek yang diam atau bergerak (P6).

In this qualitative paper, a method for technical and operational hazard identification of a water treatment plant is described. Here, fault tree analysis is applied to a physicochemical ultrafiltration (UF) membrane train, with the objectives of developing a systematic approach for organizing and improving our understanding of the hazards at the treatment plant operational level that affect the risk of infection from the pathogen Cryptosporidium parvum. The approach was successful in identifying many technical and operational hazards. The fault tree shows that water treatment plant operation is a complex task where many factors must be taken into account. Regarding the removal of C. parvum oocysts, most initiating events relate to the filtration step in the UF system. In the future, quantification of the probability of fault events may help to prioritize interventions at the operational level.

WL Aluminium industry is a small and medium enterprises which is engaged in the production of household needs and there is no unit that manages the Occupational Health and Safety (OHS).  Meanwhile the employee  were not encouraged  in OHS implementation. This study aims to determine the management system of occupational health and safety management, the dangers of work accidents and the factors causing occupational accidents that occur in the WL Aluminium company. The methods used are Fault Tree Analysis (FTA) approaches that encourage the employee based on Hazard Identification Risk Assessment (HIRA) and SWOT analysis. The results of the research can be seen in the HIRA method there are 3 (three) production activities that have a high enough risk rating number (RRN). The main cause of work accidents with the FTA method is the potential for workplace accidents that occur, namely the fingers are exposed to aluminium raw materials, the hands are exposed to hot printing containers, pain in the arms, fingers are affected by lathe eyes and back pain. For the SWOT matrix strategy, the methods used by the company to achieve good growth include employee participation through creating an OHS team for monitoring and evaluating the implementation of the OHS program, creating an administrative system aimed at managing the OHS management system, as well as socializing about OHS, empowerment of the employee in OHS implementation  and the goal of zero accident  can be achieved.

In engineering project, for a long time due to the particularity, risks and complex organization structure and management modes, etc. Construction safety has plagued the professionals and the general scholars, and highway construction is no exception. To ensure the safety of highway construction projects, hazard identification has become the top priority. Therefore, this paper proposes FTA of highway construction for hazard identification, introduces the basic steps and application process, and explain the application of this method with examples in detail. The application results of fault tree analysis verified this method is scientific and feasible.

Occupational accident is one thing that should be avoided, considering occupational accident has very harmful effects for boh the company and the labours. Dewa-Dewi Farm is a poultry farm which has four main divisions, which is mixing division is one of them. There are 5 main activities in mixing division, such as receiving and testing, weighing, grinding, mixing and distributing the feed. All work activities work in mixing division potentially bring out accidents. Identify Hazards is necessary to minimize accidents. Hazard identification and risk analysis (HIRA) is a method of identifying hazards potential that may occur in a job. By using HIRA known that the five main activities potentially face risk of occupational accident such as earaches, muscle injury, shortness of breath, sore eyes, slipping, falling, strucking, electric shock, burns and skin allergies. After identifying the problem with HIRA method is completed then carried out a risk assessment based on two criteria: likelihood (probability) and severity (impact) should be done. Based on the risk assessment there are 2 extreme-risk, 7 high risk, 4 medium- risk and 12 low-risk activities. The next method used is Fault Tree Analysis (FTA) which is a method of error analysis by direct observation and interviews, which then were visualized with a model of the fault tree. Analysis of the causes of accidents using FTA method shows the source of the risks posed by unsafe action and unsafe condition that is the wrong way of working and poor working environment. There are several recommendations given such as instructional the best way about manual lifting, provision of tools and personal protective equipment, and improve the working environment. Wireless communications became one of the most widespread means for transferring information. Speed and reliability in transferring the piece of information are considered one of the most important requirements in communication systems in general. Moreover, Quality and reliability in any system are considered the most important criterion of the efficiency of this system in doing the task it is designed to do and its ability for satisfactory performance for a certain period of time, Therefore, we need fault tree analysis in these systems in order to determine how to detect an error or defect when happening in communication system and what are the possibilities that make this error happens. This research deals with studying TETRA system components, studying the physical layer in theory and practice, as well as studying fault tree analysis in this system, and later benefit from this study in proposing improvements to the structure of the system, which led to improve gain in Link Budget. A simulation and test have been done using MATLAB, where simulation results have shown that the built fault tree is able to detect the system’s work by 82.4%.
