Technical hazard identification in water treatment using fault tree analysis

In this qualitative paper, a method for technical and operational hazard identification of a water treatment plant is described. Here, fault tree analysis is applied to a physicochemical ultrafiltration (UF) membrane train, with the objectives of developing a systematic approach for organizing and improving our understanding of the hazards at the treatment plant operational level that affect the risk of infection from the pathogen Cryptosporidium parvum. The approach was successful in identifying many technical and operational hazards. The fault tree shows that water treatment plant operation is a complex task where many factors must be taken into account. Regarding the removal of C. parvum oocysts, most initiating events relate to the filtration step in the UF system. In the future, quantification of the probability of fault events may help to prioritize interventions at the operational level.

Penelitian ini memiliki tujuan yaitu mengetahui nilai risiko potensi bahaya kerja dan level risiko potensi bahaya kerja serta mengetahui potensi bahaya kerja dominan yang menyebabkan terjadinya kecelakaan kerja pada proyek Instalasi Vertical Drain dan Geotextile di Kawasan industri Kendal. Penelitian ini menggunakan pendekatan metode HIRA (Hazard Identification and Risk Assesment) dan FTA (Fault Tree Analysis). Hasil dari analisis dengan metode FTA didapat potensi bahaya kerja pada proyek tersebut yang dominan adalah melempar, melompat, memanjat, berlari, bergurau (X5), membetulkan mesin pada keadaan mesin masih berjalan (X11), jalan akses tidak bersih dan tidak rata (X15), membetulkan mesin pada keadaan mesin masih berjalan (X11), jalan akses tidak bersih dan tidak rata (X15), dan lokasi kerja licin (X17) dengan penyebabnya adalah terpeleset, terjatuh, terkena benda tajam (P2) dan menabrak atau tertabraknya pekerja dengan objek yang diam atau bergerak (P6).

Water Treatment Plant (WTP) is an important infrastructure to ensure human health and the environment. In its development, aspects of environmental safety and health are of concern. This paper case study was conducted at the Water Treatment Plant Company in Semarang, Central Java, Indonesia. Hazard identification and risk assessment is one part of the occupational safety and health program at the risk management stage. The purpose of this study was to identify potential hazards using hazard identification methods and risk assessment methods. Risk assessment is done using criteria of severity and probability of accident. The results obtained from this risk assessment are 22 potential hazards present in the water purification process. Extreme categories that exist in the risk assessment are leakage of chlorine and industrial fires. Chlorine and fire leakage gets the highest value because its impact threatens many things, such as industrial disasters that could endanger human life and the environment. Control measures undertaken to avoid potential hazards are to apply the use of personal protective equipment, but management will also be better managed in accordance with hazard control hazards, occupational safety and health programs such as issuing work permits, emergency response training is required, Very useful in overcoming potential hazards that have been determined.

Fault tree analysis (FTA) is an effective method of ensuring the security and safety of the product by identifying all the possible causes of the problem and fixing them. However, it is not easy for a designer to construct a complete fault tree about various physical phenomena without any misunderstanding or oversight, and some computerized method of managing (i.e., storing, searching and utilizing) knowledge about FTA is needed. To solve the problem, the authors have proposed and studied a method and software tool for knowledge management of FTA based on quantity dimension indexing as a design knowledge management method to avoid ambiguity of literal expression about physical phenomena. In the previous method and software, however, fault values of quantities were limited as just above- and below-normal, and dynamic phenomena such as oscillation could not be described. In this paper, the authors introduce a systematically classified definition of fault values as above/below normal, one-side/both-sides, constant/varying, monotonic/non-monotonic and sudden/gradual, and expand the computerized systematic approach to FTA. Feasibility of the method was examined by applying it to fault tree examples made in a company.

WL Aluminium industry is a small and medium enterprises which is engaged in the production of household needs and there is no unit that manages the Occupational Health and Safety (OHS).  Meanwhile the employee  were not encouraged  in OHS implementation. This study aims to determine the management system of occupational health and safety management, the dangers of work accidents and the factors causing occupational accidents that occur in the WL Aluminium company. The methods used are Fault Tree Analysis (FTA) approaches that encourage the employee based on Hazard Identification Risk Assessment (HIRA) and SWOT analysis. The results of the research can be seen in the HIRA method there are 3 (three) production activities that have a high enough risk rating number (RRN). The main cause of work accidents with the FTA method is the potential for workplace accidents that occur, namely the fingers are exposed to aluminium raw materials, the hands are exposed to hot printing containers, pain in the arms, fingers are affected by lathe eyes and back pain. For the SWOT matrix strategy, the methods used by the company to achieve good growth include employee participation through creating an OHS team for monitoring and evaluating the implementation of the OHS program, creating an administrative system aimed at managing the OHS management system, as well as socializing about OHS, empowerment of the employee in OHS implementation  and the goal of zero accident  can be achieved.
