Transportasi laut merupakan sarana penting diberbagai bidang, antara lain adalah bidang ekonomi, sosial, keamanan, dan lain-lain. Prasarana paling penting dalam keberlangsungan transportasi laut adalah pelabuhan. Terminal Jamrud Pelabuhan Tanjung Perak merupakan terminal barang yang melayani domestik dan internasional. Dalam menjaga pelayanan pelabuhan agar berjalan tetap optimal, maka perlu menghindari waktu tunggu kapal. Penelitian ini bertujuan untuk membuat strategi optimasi waktu sandar kapal dengan mengoptimalkan pelayanan pengangkutan. Penelitian ini dilakukan dengan mengumpulkan dan mengevaluasi data bongkar muat selama 6 tahun terakhir (tahun 2011 sampai 2017). Analisa pelayanan di terminal Jamrud pelabuhan Tanjung Perak dilakukan dengan menggunakan metode pemrograman linier dan model metode model antrian. Hasil output yang utama adalah optimasi waktu sandar kapal. Hasil hasil analisis deskriptif kuantitatif dengan tujuan untuk meminimalkan waktu sandar kapal mendapatkan solusi yaitu dengan penambahan 50% NOT (Not Operation Time), 75% NOT (Not Operation Time), dan 100% NOT (Not Operation Time) pada ET (Efektif Time), maka efektivitas untuk semua jenis muatan di semua dermaga di Terminal Jamrud sesuai dengan SOP yaitu 70%. Dan juga dengan penambahan 50% IT (Idle Time) dan 50% NOT (Not Operation Time) pada ET (Efektif Time), penambahan 75% IT (Idle Time) dan 75% NOT (Not Operation Time) pada ET (Efektif Time) dan penambahan 100% IT (Idle Time) dan 100% NOT (Not Operation Time) pada ET (Efektif Time) efektivitas sudah sesuai dengan SOP yaitu 70%

The purpose of this paper is to improve line productivity using line balancing technique and a reduction of work variation using an automated part feeder. This is due to the fact that the conventional assembly line manually feeds a part on to the production line by a worker. The feeding cycle is not constant and varies from time to time. Thus, the production rate is not constant and varied by the worker feeding speed. In addition, the assembly line itself does not fully balance. The operation time of each station is not well designed; as a result, all of the stations but the bottle-neck one experience an idle time. The balance efficiency is only at 40.33 percent which means that 59.67 percent of the total production time is balanced delay. Once the automated feeding machine which a buffer is designed and implemented. The cycle time can be set at 6 sec. and with the line balance technique the operator can be reduced from 11 to 9 persons. As a result, the productivity is substantially increasing around 138 percent and the new balance efficiency is 85.12 percent.

Although the reason is not clear, mileage is commonly recommended as an indicator to evaluate the oil drain interval of a car. This paper tries to investigate whether the mileage is the most suitable indicator to evaluate the oil drain interval based on scientific experiments and data analysis method. Three civilian cars and motor oils (contained mineral oil, semisynthetic oil, and synthetic oil) were subjected to road tests under urban traffic condition. The real-time driving parameters comprising speed, mileage, idle time, engine operation time, and number of starts were obtained by on-board diagnostic system. Synchronous oil samples were collected about every 30 days. The physicochemical properties and component changes of experimental oils were analyzed in laboratory. The effects of driving parameters on oil properties were studied with the improved grey relational analysis method. The results show that the influencing factors of driving parameters on motor oil degradation were different in different experimental cars. The influence of mileage on oil degradation was not obvious. The idle time of engine, engine operation time, and number of starts were the three most significantly influential factors on oil deterioration after comprehensive analysis of the three experimental cars.

Introduction: Pancreaticoduodenectomy is the procedure of choice for periampullary neoplasms. It is considered as a major surgical procedure. It is associated with relatively higher postoperative mortality and morbidity rate, however, with development of technology, proper patient selection, meticulous operative technique, appropriate postoperative care, morbidity and mortality rate has decreased subsequently. Up to the 1970s, the operative mortality rate after pancreaticoduodenectomy approached 20% but it has been reduced to less than 5% in recent reports. This study is designed to evaluate the postoperative outcomes of pylorus-preserving pancreaticoduodenectomy procedure in our set up. Objective: To evaluate the outcome of the pylorus-preserving pancreaticoduodenectomy procedure with the intention to measure operation time and per-operative bleeding, observing postoperative anastomotic leakage and gastric emptying time. To find out postoperative wound infection and complications to detect the dumping syndrome. Materials and Methods: A prospective observational study was carried out in the Department of Hepatobiliary Surgery, Combined Military Hospital, Dhaka from July 2013 to January 2017. Fifty patients who underwent pylorus-preserving pancreaticodudenectomy procedure were included in this study. Results: Out of 50 postoperative patients, 12(24%) patients developed complications. Of these patients, 3(6%) developed wound infection, 2(4%) developed bile leakage and 2(4%) developed postoperative haemorrhage. Pancreatic fistula, vomiting, delayed gastric emptying and abdominal collection all were 1(2%) each. Postoperative mortality was 3(6%). Conclusion: The present study demonstrated the development of postoperative complications after pylorus-preserving pancreaticoduodenectomy is as similar as published in different studies. Better outcome can be achieved with meticulous pre-operative evaluation of risk factors and per-operative skill maneuvering. Journal of Armed Forces Medical College Bangladesh Vol.13(1) 2017: 42-45

<p><strong>Aim</strong>: To study the effect of age, gender, side and impaction types on surgical difficulty during mandibular third molar extraction through the data collected over a period of one year by single maxillofacial surgeon. All the extractions were performed under the same environment and conditions.</p><p><strong>Methods</strong>: Descriptive clinical study of 401 patients was carried out between the ages of 16 to 45years. Age, gender, impaction side and impaction types (according to the winter classification) were recorded on proforma. Duration of surgery for each patient was recorded after starting incision to the completion of suture which was divided into less than 10 minutes (mild), 11 to 20 minutes (moderate) and above 21 minutes (severe). Pearson’s Chi-square test was used for data analysis and significance level was less than or equal to 0.05.</p><p><strong>Results</strong>: Among 401 participants, 225 (56.1%) were male and 176 (43.9%) were female. Mean age was 31.5 years and mean operation time was 17.59 minutes. After statistical analysis there was a significant correlation among gender and side of impaction where <em>p </em>value is 0.043 0.048 respectively.</p><p><strong>Conclusion</strong>: There is a statistically significant correlation between gender and side of impaction with duration of surgery which is considered as the objective measure of surgical difficulty whereas age and impaction types didn’t show any significant correlation. </p>

Operation time of the well before stopping for investigating the pressure recovery curve in hydrodynamic studies is an important parameter affecting the quality and accuracy of results of research processing. Comparing the actual and theoretical pressure curves and the derivative, it’s possible to eliminate the uncertainty in the choice of previous history of the well operation.

Background: Conventional three–port laparoscopic appendectomy is becoming popular for the treatment of acute appendicitis. In this report, we present the early results of a new technique of laparoscopic appendectomy conducted through a single-port. Patients and Methods: From March 2011 to October 2013, we have performed 86 operations Single Port Laparoscopic Appendectomy at Hue University Hospital and Hue Central Hospital. SILS Port (Covidien) is used, it can be performed with basic laparoscopic instruments. Results: In this study, 86 patients underwent Single-Port laparoscopic appendectomy, among them 52.33% were femele, 47.67% were male, female/male ratio was 1.09. The mean age was 33.09. An orther trocar insertion was required in 2 patients (2.33%). The mean operation time was 42.03 minutes and mean postoperative hospital stay 3.37 days. Postoperative complication occurred in 2 case (2.33%) was of omphalitis. During 2 weeks follow up, 2 case (2.33%) was of omphalitis. Conclusions: Single - port intracorporeal appendectomy is a safe, minimal invasive procedure with excellent cosmetic results. Key words: Single Port Laparoscopic Appendectomy, appendectomy

Objectives: Laparoscopic appendectomy (LA) is becoming popular for the treatment of acute appendicitis. Since it was the first described, LA has been modified various times. We present the results of a new technique of LA conducted through a single port. Materials and methods: From March 2011 to November 2011, we have performed 28 operations Single Port Laparoscopic Appendectomy at the Surgical Department of Hue Central Hospital. Results: There were 28 patients, 57.1% were female, 42.9% were male, rate female/male was 1.3. The mean age is 36.4. The second port insertion was required in 2 patients (7.2%). Mean operation time was 44.6 minutes and postoperative hospital stay 3-5 days took 71.4%. Postoperative compications occurred in 1 case (3.6%) was of omphalitis. During 2-4 weeks follow up no problem related to the appendectomy have been reported. Conclusions: Single - port intracorporeal appendectomy procedure is a safe, minimal invasive procedure with excellent cosmetic results.

The results of treatment of postoperative intraabdominal complications in patients who underwent surgical interventions for colon cancer were studied. The effect of the re-operation time was assessed. The volume of surgical correction was discussed in case of insufficiency of anastomosis sutures. The results of the study made it possible to identify the most rational timing and scope of surgical correction of postoperative intraabdominal complications in patients with colorectal cancer.
