Objective: To prove that Meloxicam 15 mg only, combination of Meloxicam 15 mg and Tamsulosin 0.4 mg were more effective for the success of Trial Without Catheter (TWOC) in BPH patients with first episode of urinary retention compared to Tamsulosin 0.4 mg only. Material & methods: Benign Prostatic Hyperplasia (BPH) patients with first episode of urinary retention that met the inclusion criteria and did not fulfill the exclusion criteria were randomized to form 3 treatment groups, n=11 for each group. Group I was given Meloxicam 15 mg only, group II was given a combination of Meloxicam 15 mg and Tamsulosin 0.4 mg, and group III was given Tamsulosin 0.4 mg only. For each group the drug given once orally for 7 days. The success of TWOC assessed by an ability to spontaneous micturition after that each treatment in the first 24 hours after urethral catheter removal, accompanied by Qmax in uroflowmetry ≥ 5 cc/sec and PVR ≤ 100 cc. Results: All Meloxicam 15 mg only group samples have recurred urinary retention (100%). The success rate of TWOC for combination of Meloxicam 15 mg and Tamsulosin 0.4 mg group was 72.7%; while for the Tamsulosin 0.4 mg only one was 63.6%. The success rate of TWOC for combination of Meloxicam 15 mg and Tamsulosin 0.4 mg group was higher than Tamsulosin 0.4 mg only one (p=0.003). Conclusion: The combination of Meloxicam 15 mg and Tamsulosin 0.4 mg orally once daily for 7 days was more effective in the success of TWOC in BPH patients with first episode of urinary retention compared to Tamsulosin 0.4 mg only orally once daily for 7 days.

Mutans streptococci (MS) are one of the most virulent cariogenic pathogens in the oral cavity. The effects of three oral hygiene techniques on salivary mutans streptococci levels were evaluated to see which mechanism would be most efficient in reducing salivary MS levels. Sixty patients, all in the permanent dentition, were selected and randomly distributed into three groups. Group I was asked to use a tongue scraper to brush the tongue once daily in the morning after normal tooth brushing routine. Group II was asked to place and let dissolve a Listerine Oral Care StripTM on the tongue once daily after normal oral hygiene routine. Group III was asked to rinse once every morning, for thirty seconds, with a saturated saline solution after routine tooth brushing. The instructions were done for a period of seven days. Baseline, one hour, three day, and seven day saliva samples were obtained and plated on CRT" by Vivadent MS-sensitive agar. All treatments groups show a significant reduction in colony counts from baseline and one or more post treatment periods and at one or more time periods between treatment groups. The most effective treatment in reducing colony counts was seen within Group I "Tongue Scraping" which demonstrated the greatest change from baseline to each of the post treatment periods. The least effective was Group II "Listerine Strip" which showed a statistically insignificant increase in colony count from baseline to 1-Hour and a significant decrease from baseline at the 7-day period only.

The present study was conducted to investigate the efficacy of Ovsynch and Ovsynch plus protocol in postpartum (day 60) Sahiwal cows (n=18). Animals were randomly divided into three equal groups, viz., Ovsynch group Ovsynch plus group and Control group. Animals of group I (n = 6) were treated with traditional Ovsynch protocol. The animals (n = 6) of group II were treated with Ovsynch plus protocol which consisted of an initial intramuscular injection of eCG (Folligon) @ 250 IU on day 60 postpartum followed 3 days later by GPG (Ovsynch) protocol. In group-III Control, no treatment was given to animals (n = 6). Treated animals were inseminated at a fixed time between 14 and 20 hrs after second GnRH injection, irrespective of estrus detection. Blood samples were collected from each animal on days 50 and 60 postpartum to determine the status of cyclicity in animals based on serum concentrations of progesterone (P4). A third blood sample was collected on the day of prostaglandin treatment to determine the response of first GnRH injection. Four animals each were cyclic, and two were acyclic in both treatment groups. Four animals each responded to first GnRH treatment in both treatment groups. Similarly, two animals each got conceived giving conception rate of 50% (2/4) in each treatment. In the control group, one out of 6 animals got conceived yielding 16.66 % conception rate (1/6) during the study period. It may be thus concluded that Ovsynch and Ovsynch plus protocol may be used during the early postpartum period to improve the reproductive efficiency in postpartum Sahiwal cows.

Objectives To compare the clinical outcomes of patients undergoing additional procedures in endoscopic endonasal dacryocystorhinostomy (End-DCR) surgery and discuss the factors affecting the success of End-DCR surgery in light of relevant literature. Methods The study included 155 patients who underwent End-DCR surgery in our clinic due to epiphora. This was a prospective randomized, single-blind, controlled trial. Group I (control) included 54 patients who did not undergo silicone stent insertion or silver nitrate application, group II included 51 patients who underwent silicone stent insertion only, and group III included 50 patients who underwent silver nitrate application only. Statistical analysis was performed on follow-up data regarding anatomic and functional success rates. Results Revision surgery was performed in 16 patients who developed persistent epiphora in the postoperative period, including 6 in group I, 7 in group II, and 3 in group III ( P = .4). The most common reason for revision surgery was stenosis of the neo-ostium ( n = 8), followed by granulation tissue formation ( n = 5) and synechia formation ( n = 3). Granuloma formation was the most common postoperative complication, and a significant difference was found among the groups with regard to granuloma formation ( P = .04). At postoperative month 12, the functional success rate was estimated to be 88%, 86%, and 94%, and the anatomic success rate was estimated to be 94%, 92%, and 96% in groups I, II, and III, respectively, with no significant difference, found among the 3 groups with regard to both rates ( P = .79 and P = .76, respectively). Conclusion The results indicated that stenting and silver nitrate application did not affect surgical success. Our preliminary results on silver nitrate cauterization showed that it is an effective, inexpensive, and practical method to reduce granulation formation in the postoperative period.

CONTEXT: Management of patients with obstructed defecation syndrome is still controversial. OBJECTIVE: To analyze the efficacy of clinical, clinical treatment followed by biofeedback, and surgical treatment in patients with obstructed defecation, rectocele and multiple dysfunctions evaluated with echodefecography. METHODS: The study included 103 females aged 26-84 years with obstructed defecation, grade-II/III rectocele and multiple dysfunctions on echodefecography. Patients were distributed into three treatment groups and constipation scores were assigned. Group I: 34 (33%) patients with significant improvement of symptoms through clinical management only. Group II: 14 (14%) with improvement through clinical treatment plus biofeedback. Group III: 55 (53%) referred to surgery due to treatment failure. RESULTS: Group I: 20 (59%) patients had grade-II rectocele, 14 (41%) grade-III. Obstructed defecation syndrome was associated with intussusception (41%), mucosal prolapse (41%), anismus (29%), enterocele (9%) or 2 dysfunctions (23%). The average constipation score decreased significantly from 11 to 5. Group II: 11 (79%) grade-II rectocele, 3 (21%) grade-III, associated with intussusception (7%), mucosal prolapse (43%), anismus (71%) or 2 dysfunctions (29%). There was significant decrease in constipation score from 13 to 6. Group III: 8 (15%) grade-II rectocele, 47 (85%) grade-III, associated with intussusception (42%), mucosal prolapse (40%) or 2 dysfunctions (32%). The constipation score remained unchanged despite clinical treatment and biofeedback. Twenty-three underwent surgery had a significantly decrease in constipation score from 12 to 4. The remaining 32 (31%) patients which 22 refused surgery, 6 had low anal pressure and 4 had slow transit. CONCLUSIONS: Approximately 50% of patients with obstructed defecation, rectocele and multiple dysfunctions presented a satisfactory response to clinical treatment and/or biofeedback. Surgical repair was mainly required in patients with grade-III rectocele whose constipation scores remained high despite all efforts.

The aim of the 21 day toxicity study was to evaluate the pathomorphological effect of flunixin meglumine in layer chicks. The chicks of Group I were kept as control while groups II, III and IV were fed with diet containing flunixin meglumine @ 10 ppm, 25 ppm and 50 ppm respectively for 21 days. Clinical signs viz. anorexia, dullness, lethargy, lameness and uneven growth were noticed in chicks of treatment groups III and IV only. Maximum mortality was observed in group IV (12%) followed by group III (4%). A dose dependant reduction in body weight was observed in all the treatment groups. The mean values of Kidney: Body weight ratio was significantly increased in group IV. The plasma uric acid, creatinine and BUN values were significantly increased in group III whereas increase in group IV was highly significant. Grossly, there was deposition of chalky white urates on serosal surface of kidney, heart and liver in chicks of group IV which died during experiment. Microscopically, lesions were characterized by varying degrees of congestion, haemorrhages, degeneration, necrosis and deposition of urate crystals in visceral organs of group III and group IV chicks. The intensity and distribution of pathological lesions were more severe in chicks of group IV, followed by chicks of group III. The overall lesions gave an impression that flunixin meglumine was nephrotoxic in nature.

Background: Nephrotoxicity is characterized by raised serum urea and creatinine levels and kidneys being one of the primary organs of drug concentration and excretion are vulnerable to many widely used marketed drugs, including anti-cancer drugs, antibiotics, immunosuppressants, and radio contrast agents, are nephrotoxic. Aim: To determine the nephroprotective effect of ethanolic extract of Carica papaya seeds in Aminoglycoside induced acute nephrotoxicity. Study design: Quasi Experimental study. Methodology: Thirty Sprague Dawley rats were sub divided into 3 groups i.e., I, II and III with 10 rats in each group. Group II and III were administered Aminoglycoside drug; Gentamycin in 80 mg/kg) via intraperitoneal route once daily for 5 consecutive days to induce acute nephrotoxicity. At day 6, nephrotoxicity was confirmed by measuring serum urea and creatinine. Ethanolic extract of Carica papaya seeds (1000 mg/kg) was started once daily through oral route in group III for 5 consecutive days to see the nephroprotective effects of seed extract after causing acute kidney injury. All animals were given standard diet pellets manufactured at NIH. Results: Mean serum urea and creatinine for Group I (Control Group) at day 0 were 24.90 mg/dL ± 1.633 and .750 mg/dL ± .0619 respectively. Mean serum urea and creatinine for Group II (Disease Control Group) was 81.00 mg/dL ± 1.247 and 1.980mg/dL ± .0467 at day 6th. This suggested induction of nephrotoxicity by Gentamycin. Mean serum urea and creatinine for Group III (Ethanolic Extract Treated Group) at 11 day was 72.40mg/dL± .991 and 1.680 mg/dL± .0467 after 5 days treatment with ethanolic extract of Carica papaya seeds. Conclusion: We concluded that ethanolic extract of Carica papaya seeds has significant nephroprotective effects on Aminoglycoside induced acute nephrotoxicity in rats. Background/Aim. There are many specificities of merital infertility and sometimes surprising connections between some thinks with no connections at first sight. Examinations of these patients imply diagnostic actions such as the blood basal hormone sample, doing hysterosalpingography, ultrahysterosonography, ultrasound examinations, and sometimes laparoscopy and hysteroscopy if there are necessary. The aim of the study was to determine the characteristics of the connection between policystic ovary (PCO) syndrome (Sy) and congenital M?llerian ducts abnormalities. Methods. This study included 356 patients treated in the period from January 1, to December 31, 2009, in the Department of Infertility of the Clinic for Obstetrics and Gynecology in Nis, Serbia. Exclusion criteria were no myoma, ovary cysts, tubal and male factors of infertility. Results. A total of 180 patients were divided into 3 groups: the group I with PCO sy, the group II with uterine congenital malformation and the group III with a combination of these disorders. The middle age of patients was 29.6 ? 4.8, body mass index (BMI) was 26.1 ? 4,8 kg/m2 the middle thicknes of endometrium was 5.2 + 2.7 mm, and there were no significant differences between the examined groups. There were no significant among in a number of miscarriages in the examined groups. We found that PCO Sy and congenital abnormalities of M?llerian ducts were conjoint in 30% of examined patients. Conclusion. Conjoined PCO Sy and congenital abnormalities of M?llerian ducts do not result in a higher number of misscarriages than only either PCO Sy or abnormalities of M?llerian ducts. It is important to check BMI, basal level of follicle stimulating hormone and number of antral follicles because the induction protocol and concentracion of inductors depends on these characteristics, thus, the successful cycles and pregnancy.

Performing a block under ultrasound guidance effectively requires skill; however, inexperienced anesthesiologists often use high-dose LA to ensure success. We aimed to share our experience with the ultrasound-guided infraclavicular brachial plexus block (USGICB) for upper extremity surgeries and to determine changes in failure rate and local anesthetic dose administered with gaining adequate experience. With approval from the local ethics committee, a retrospective review of records of 2953 patients who underwent USGICB between November 2011 and March 2015 was performed for evaluating the following data: age, sex, height, weight, operation type, American Society of Anesthesiologists physical status score, local anesthetic volume, complications, and success of USGICB. The patients were divided into 4 groups of 10 months each from November 2011 to March 2015: first 10-month period, 628 cases (group I); second 10-month period, 672 (group II); third 10-month period, 720 (group III); and the fourth 10-month period, 933 cases (group IV). Nine anesthesiologists with the same baseline experience in USG performed the blocks. During the initial period, when anesthesiologists had insufficient experience, local anesthetic (LA) dose, success rate, failed blocks, and complications were investigated. The LA volume administered in group I (33.7 ± 4.2 ml) was significantly higher than that in groups II, III, and IV (p<0.05). Although a reduction in LA volume administered with increasing anesthesiologist experience was not statistically significant, a volume reduction of over 30 ml was observed in groups II, III, and IV compared with group I. Furthermore, in group I, failure rate (3.2%) was higher than that in groups II, III, and IV (p<0.05). We concluded that sonographic guidance ensures a high success rate and that increased experience of anesthesiologists is associated with reduced complications and failure rate of blocks, in addition to prevention of LA overdose.

A total of 18 bitches in the period in the period of six month diagnosed for transmissible venereal tumor in Teaching Veterinary Clinical Complex, Nagpur by per vaginal examination and vaginal cytology. These bitches were divided randomly into three groups, viz., Group I (Inj. Vincristine @ 0.025mg/kg slow IV in DNS and Injection Botropase 1ml IM for 1st week), Group II (Inj. Vincristine @ 0.025mg/kg slow IV in DNS and Tab. Amoxycillin and Potassium Clavinate for first 7 days orally) and Group III (treated with autohaemotherapy@10 ml whole blood IM) for once in week and continue for 5 week. At the time of treatment, liver and kidney function tests were performed. During the course of treatment significant variation was noted in Group I and II in BUN values while creatinine values changes significantly in all treatment groups. In case of SGPT only significant changes observed in Group I. Rest of the changes in serum prolife found to be non significant. Also from recovery point of view we noted that, it was faster in Group I and II compare than Group III. Side effects observed only in Group I and II. From the study we concluded that autohaemotherapy will be an alternate option for treatment of TVT in bitches who shows the excessive side effects of Vincristine Sulphate.
