Introduction. Cavernous hemangiomas are benign vascular tumours rarely described in the lungs. Symptoms include respiratory distress, cardiac failure and massive haemoptysis, but they are mostly asymptomatic. Case report. A 67-year-old woman was referred to our institute and treated for pneumonia. A computed tomography scan of the thorax showed an infiltrative mass about 46mm in its greatest dimension in the right upper lobe. The mass was in contact with the mediastinal pleura. Since bronchial biopsy and FNAC did not reveal the aetiology of the lesion, the video-assisted thoracic surgery with right anterolateral thoracotomy and enucleation was performed. The intraoperative and postoperative patohistological study showed cavernous hemangioma and this diagnosis was confirmed on immunohistochemical staining. Conclusion. In spite of its benign behaviour and mostly asymptomatic clinical course it is necessary to consider cavernous hemangioma in the differential diagnosis of other pulmonary lesions. The definitive diagnosis can be made only by histopathological and immunohistochemical examination.

AbstractPhrenic nerve provides the major motor supply to diaphragm. Various anatomical variations in the course and distribution of the phrenic nerve have reported before. Here we report a rare bilateral asymmetric variation in the roots of origin of the phrenic nerve and absence of fibrous pericardium in an old male cadaver. Specifically, the right phrenic nerve was arising from the upper trunk of the brachial plexus (C5) and the left side nerve originated from the supraclavicular nerve and a tiny branch from C5. In the same cadaver both sides phrenic nerve were buried in the mediastinal pleura. Another interesting finding was absence of the fibrous pericardium. To the best of our knowledge the presented case showed a very rare variation in the roots of origin of the phrenic nerve accompanied with pericardial anomaly which has been less reported. We think such case is of practical importance during supraclavicular block during anesthesia

Synovial sarcoma is a malignant mesenchymal tumour which mostly affects young adults and is mostly seen in extremities. Primary synovial sarcoma arising from the lung is rare, accounting for less than 0.5% of all lung carcinomas. Most commonly it arises from the lung followed by pleura and mediastinum. Primary pulmonary synovial sarcoma is an extremely aggressive malignant tumour that can invade adjacent organs and give distant metastasis. Histologically it is of two main types – monophasic and biphasic. IHC is a must for diagnosis following clinical examination and imaging. Here we report a case of an elderly male with right lung mass lesion infiltrating the visceral and mediastinal pleura. PET – computerized tomography (CT) guided biopsy was s/o synovial sarcoma monophasic type which was further confirmed by IHC. The multimodality treated for this includes wide resection, chemotherapy and radiotherapy. Synovial sarcoma is relatively chemosensitive though it is considered as a high grade tumour with a poor prognosis. Because of the advanced stage of the disease our patient was not a candidate for surgery and was taken up for chemotherapy. He had a survival of 6 months but had succumbed due to non – cancer related cause.
